Clinical and laboratory examinations were carried out on a total of 338 monkeypox patients in Zaire from 1981 to 1986. An animal source of infection was suspected in 245 (72%) and interhuman transmission for the remaining 93 patients. Among those whose infection was presumably acquired from an animal source, the most affected groups were children aged 3-4 years (27%) and 5-6 years (20%), while only 4% of cases were over 15 years old; there was a considerable preponderance of males (58%) over females (42%), especially in the age group 5-14 years. Among those presumably infected by person-to-per

BACKGROUND: Little information exists in the literature regarding the burden and impact of mpox infection in the newborn population since th … immunodeficiency virus. Vesicular fluid polymerase chain reaction testing confirmed the presence of monkeypox virus (MPXV) DNA and was negative for varicella-zoster virus. Despite the diagnosis of mpox in the early neonatal period with the high likelihood of a transplacental route of transmission, the possibility of a postnatal transmission could not be entirely …

The control of smallpox was achieved using live vaccinia virus (VV) vaccine, which successfully eradicated the disease worldwide. As the variola virus no longer exists as a natural infection agent, mass vaccination was discontinued after 1980. However, emergence of smallpox outbreaks caused by accidental or deliberate release of variola virus has stimulated new research for second-generation vaccine development based on attenuated VV strains. Considering the closely related animal poxviruses that also arise as zoonoses, and the increasing number of unvaccinated or immunocompromised people, a s

There is a concern that there may be unregistered stocks of smallpox that can be used for bioterrorism or biological warfare. According to the WHO Advisory Committee on Variola Research, there is a need to develop strategies to treat smallpox infections should they reappear. It would also be important to have an effective drug at hand for the treatment of monkeypox disease in humans. We show here that 5-iodo-2'-deoxyuridine (IDU) is a potent inhibitor of vaccinia virus (VV) replication and that IDU inhibits VV DNA synthesis in a dose-dependent way. The in vivo protective effect of IDU was asse

In May 2022, a large outbreak of monkeypox virus (MPXV) occurred in several non-endemic regions worldwide. Another outbreak in 2024 raised global concerns, particularly for immunocompromised individuals like people with HIV (PWH). Since the latent HIV-1 reservoir remains a major barrier to a cure, we investigated how past mpox infection affected reservoir dynamics in this population. PWH who had mpox at an average of 9 months before sampling showed a significantly smaller HIV-1 reservoir than MPXV-unexposed PWH. This reduction was accompanied by enhanced antigen-driven proviral reactivation in…

BACKGROUND: Recent mpox outbreaks highlight the need for novel, effective, and scalable vaccines against monkeypox virus (MPXV). The quadrivalent mRNA vaccine candidate BNT166a encoding MPXV antigens A35, B6, M1, and H3 was selected for clinical development based on evidence of immunogenicity and protective efficacy against multiple orthopoxviruses in preclinical models. We aimed to investigate the safety and immunogenicity of BNT166a in healthy adults.
METHODS: In this open-label, uncontrolled, non-randomised, dose-escalation, first-in-human, phase 1 trial, we included healthy adults with no…

RNA N6-methyladenosine (m6A) demethylase AlkB homolog 5 (ALKBH5) plays a crucial role in regulating innate immunity. Lysine acylation, a widespread protein modification, influences protein function, but its impact on ALKBH5 during viral infections has not been well characterized. This study investigates the presence and regulatory mechanisms of a previously unidentified lysine acylation in ALKBH5 and its role in mediating m6A modifications to activate antiviral innate immune responses. We demonstrate that ALKBH5 undergoes lactylation, which is essential for an effective innate immune response

Human immunodeficiency virus (HIV)-infected individuals have an increased risk of various infections due to their impaired host immune system, resulting in higher morbidity and mortality rates. These patients severely suffered during the COVID-19 epidemic, the influenza epidemic and the spread of monkeypox. Reducing serious infections is one of the most important measures to improve HIV-infected individuals' quality and length of life. Based on the preparation processes and their antigenic properties, vaccines are divided into several types, including inactivated vaccines, attenuated live vacc

Monkeypox disease (mpox) is an endemic zoonosis, previously restricted to Central and Western African countries. Since 2022, it is the most common orthopoxvirus (OPXV) zoonotic disease observed in non-endemic countries, including Europe and America, and 333 cases of mpox were already confirmed in the Balkan countries. To monitor and control the spread of mpox, a surveillance program was established in line with WHO guidelines. Although the risk of disease has diminished, sporadic cases still occur. The reemergence of mpox cases at the end of 2024 in Greece, where the first case was registered

Monkeypox (Mpox) is a zoonotic viral infection endemic to Africa, which has caused a global outbreak since April 2022. The global Mpox outbreak is related to Clade IIb. The disease has primarily affected men who have sex with men. Skin lesions are concentrated in the genital area, with lymphadenopathy as well as concurrent sexually transmitted infections (STIs). This is an observational study of adult patients with a recent development of skin lesions and systemic symptoms, which could not be explained by other diseases present. Fifty-nine PCR-positive patients with prominent skin lesions in t

In 1958, several monkeys in a Copenhagen laboratory developed a skin rash from which an orthopoxvirus could be isolated, which was named monkeypox virus (MPXV). However, the natural animal reservoir for MPXV is thought to be a rodent. The first human case occurred in 1970, and the incidence has increased progressively throughout the years. Starting May 2022, the number of cases outside Africa has soared, especially in Western Europe. There are two clades of MPXV, Congo Basin, with higher virulence and mortality, and Western Africa (WA). MPXV from the present outbreak has been proposed to be cl…

SUMMARY: Mpox (MPX) formerly known as monkeypox was declared a public health emergency of international concern, following an outbreak that commenced in May 2022. We report a case of subacute thyroiditis following MPX infection. To our knowledge, it is the first documented incidence of this complication in humans. A 51-year-old male, with a well-controlled human immunodeficiency virus (HIV) infection on antiretroviral therapy, was reviewed 3 weeks after a positive test for MPX. The acute skin lesions and initial systemic symptoms had resolved, but he described significant neck discomfort, fati

BACKGROUND: Poxviruses comprise a group of large double-stranded DNA viruses and are known to cause diseases in humans, livestock animals, and other animal species. The Mpox virus (MPXV; formerly Monkeypox), variola virus (VARV), and volepox virus (VPXV) are among the prevalent poxviruses of the Orthopoxviridae genera. The ongoing Mpox infectious disease pandemic caused by the Mpox virus has had a major impact on public health across the globe. To date, only limited repurposed antivirals and vaccines are available for the effective treatment of Mpox and other poxviruses that cause contagious d

AIMS: To evaluate whether antibodies specific for the vaccinia virus (VV) are still detectable after at least 45 years from immunization. To confirm that VV-specific antibodies are endowed with the capacity to neutralize Mpox virus (MPXV) in vitro. To test a possible role of polyclonal non-specific activation in the maintenance of immunologic memory.
METHODS: Sera were collected from the following groups: smallpox-vaccinated individuals with or without latent tuberculosis infection (LTBI), unvaccinated donors, and convalescent individuals after MPXV infection. Supernatant of VV- or MPXV-infec

The vaccinia virus (VACV) has been used for prophylactic immunization against smallpox for many decades. However, the VACV-based vaccine had been highly reactogenic. Therefore, after the eradication of smallpox, the World Health Organization in 1980 recommended that vaccination against this infection be discontinued. As a result, there has been a rise in the occurrence of orthopoxvirus infections in humans in recent years, with the most severe being the 2022 monkeypox epidemic that reached all continents. Thus, it is crucial to address the pressing matter of developing safe and highly immunoge

BACKGROUND: In May 2022, a case of monkeypox (currently known as "mpox") with no history of overseas travel was reported in the United Kingdom, followed by reports of infections reported in Europe, the United States, and other countries worldwide. Due to the significant overlap in immune responses among viruses of the genus Orthopoxvirus (including smallpox virus, mpox virus, and vaccinia virus), it is believed that cross-immunity can be achieved by administering the smallpox virus vaccine. In Japan, a smallpox vaccine (LC16m8 strain vaccine) has been approved; however, there was no regulatory

INTRODUCTION: non-exanthematous presentations in mpox infection are increasingly reported, posing a risk of underestimating the true burden of the disease. In Nigeria, border towns play a unique role in shaping the dynamics of mpox transmission, yet they have received relatively little research attention. This study aims to address this gap by investigating the prevalence of mpox infection in a potential hotspot region of Nigeria.
METHODS: a cross-sectional study was conducted during a free medical outreach in Iboko, Izzi Local Government Area, Ebonyi State, Nigeria, from April 15 to May 20,
