Substance-induced manic psychosis in which delusions were corroborated by a chatbot - case report.

BACKGROUND: This case describes a substance-induced manic episode with psychotic features in which interaction with an AI (artificial intelligence) chatbot appeared to corroborate and reinforce the patient's delusional thought content and to contradict medical advice. Excerpts from the patient's interactions with the AI chatbot provide novel clinical insight into this phenomenon, which to date has primarily been reported in news media. CASE PRESENTATION: A man in his 30s presented to the emergency department with a one-week history of escalating behavioural disturbance, severe insomnia, press

A Deep Learning Approach to Predict Conductive Hearing Loss in Patients With Otitis Media With Effusion Using Otoscopic Images.

IMPORTANCE: Otitis media with effusion (OME) is one of the most common causes of acquired conductive hearing loss (CHL). Persistent hearing loss is associated with poor childhood speech and language development and other adverse consequence. However, to obtain accurate and reliable hearing thresholds largely requires a high degree of cooperation from the patients. OBJECTIVE: To predict CHL from otoscopic images using deep learning (DL) techniques and a logistic regression model based on tympanic membrane features. DESIGN, SETTING, AND PARTICIPANTS: A retrospective diagnostic/prognostic study

Yokukansan (TJ-54) for treatment of pervasive developmental disorder not otherwise specified and Asperger's disorder: a 12-week prospective, open-label study.

BACKGROUND: Numerous medications have been tested on patients with pervasive developmental disorder not otherwise specified (PDD-NOS) and Asperger's disorder. Although many of these medications have been demonstrated to be useful, no clear primary treatment for PDD-NOS and Asperger's disorder has emerged. Despite the efficacy of some of the medicines, the acceptability and side effects have proven to be barriers to their use. Recent studies indicate that the traditional Japanese herbal medicine yokukansan (TJ-54) may be safe and useful in treating behavioral and psychological symptoms in demen

Differential physiological and behavioral cues observed in individuals smoking botanical marijuana versus synthetic cannabinoid drugs.

CONTEXT: Synthetic cannabinoid use has increased in many states, and medicinal and/or recreational marijuana use has been legalized in some states. These changes present challenges to law enforcement drug recognition experts (DREs) who determine whether drivers are impaired by synthetic cannabinoids or marijuana, as well as to clinical toxicologists who care for patients with complications from synthetic cannabinoids and marijuana. Our goal was to compare what effects synthetic cannabinoids and marijuana had on performance and behavior, including driving impairment, by reviewing records genera

Outcomes of Children with Hearing Loss: Data Collection and Methods.

OBJECTIVES: The primary objective of this article was to describe recruitment, data collection, and methods for a longitudinal, multicenter study involving children with bilateral mild to severe hearing loss. The goals of this research program were to characterize the developmental outcomes of children with mild to severe bilateral hearing loss during infancy and the preschool years. Furthermore, the researchers examined how these outcomes were associated with the child's hearing loss and how home background and clinical interventions mediated and moderated these outcomes. DESIGN: The partici

Redo double-opposing Z-plasty is effective for correction of marginal velopharyngeal insufficiency.

OBJECTIVE: Double-opposing Z-plasty (DOZ) of the Furlow method has been successfully used for the correction of marginal velopharyngeal insufficiency (VPI) by reconstructing the levator muscle and lengthening the soft palate. For a recurrent or residual marginal VPI, it was questioned whether redo DOZ could be applied to correct the problem and minimize surgical morbidity. This study reported the outcome of the redo DOZ. MATERIALS AND METHODS: The DOZ procedure was the method of choice for marginal VPI in the authors' center. To evaluate the effectiveness of the redo operation, patients with

Acquisition Sequence of Grammatical Structures in Azerbaijani-Turkish-speaking Children Aged 4-6 Years: A Cross-Sectional Study.

BACKGROUND: Grammatical development in children follows a relatively predictable sequence across languages; however, most evidence comes from Indo-European languages, particularly English. Azerbaijani-Turkish, an agglutinative Turkic language, remains understudied in terms of the order in which grammatical structures are acquired. Understanding typical acquisition sequences is essential for identifying children with developmental language disorder (DLD) in linguistically diverse populations. AIMS: This study aimed to describe the receptive comprehension sequence of 28 grammatical structures i

Interventions to enable communication for adult patients requiring an artificial airway with or without mechanical ventilator support.

BACKGROUND: Inability to communicate in a manner that can be understood causes extreme distress for people requiring an artificial airway and has implications for care quality and patient safety. Options for aided communication include non-vocal, speech-generating, and voice-enabling aids. OBJECTIVES: To assess effectiveness of communication aids for people requiring an artificial airway (endotracheal or tracheostomy tube), defined as the proportion of people able to: use a non-vocal communication aid to communicate at least one symptom, need, or preference; or use a voice-enabling communicat

Two-stage palatoplasty using a modified Furlow procedure.

A two-stage palatal repair using a modification of Furlow palatoplasty is presented. The authors investigate the speech outcome, fistula formation and maxillary growth. In a prospective, successive cohort study, 40 nonsyndromic patients with wide cleft palate were operated on between March 2001 and June 2006 by a single surgeon. 10 patients in the first cohort underwent a Furlow palatoplasty (control group). In 30 patients in the second cohort a unilateral myomucosal cheek flap was used in combination with a modified Furlow palatoplasty (study group). The hard palate was closed in both groups

Evaluation of postoperative function in patients undergoing reconstruction following resection of superior and lateral oropharyngeal cancer: long-term outcomes of reconstruction with the Gehanno method.

Resection of the superior or lateral wall of the cancer-affected oropharynx can often lead to disturbed nasal breathing, dysphagia, and dysarthria. The authors used the Gehanno method to reconstruct these surgical defects and achieved favourable functional recovery soon after surgery. The present study was undertaken to analyze the long-term outcome and usefulness of this method. Reconstruction was carried out using the Gehanno method in 36 patients during the 10-year period between 1997 and 2007. Both short-term and long-term evaluations of the postoperative function were performed in 12 of t

Altered Paired-Click Auditory Brainstem Responses in Normal-Hearing Young Adults With Frequent Loud Sound Exposure.

This study investigated differences in auditory brainstem responses (ABRs) using the paired-click paradigm between young adults frequently exposed to loud sounds and those not. Young adults with normal hearing were divided into two groups: an exposed group (n = 23), who were frequently exposed to loud sounds through music practice, and a control group (n = 21). Sound exposure levels (measured via smartwatch), hearing thresholds (0.125-16 kHz), distortion product otoacoustic emission (DPOAE) magnitudes, word intelligibility, and ABRs to single and paired clicks were obtained. For paired-click A

Pedicled Supraclavicular Artery Island Flap Versus Free Radial Forearm Flap for Tongue Reconstruction Following Hemiglossectomy.

This study investigated the tongue function and donor-site morbidity of patients with malignant tumors who had undergone immediate flap reconstruction surgery. Twenty-seven patients who had undergone immediate reconstruction after hemiglossectomy were observed. Twelve patients were reconstructed using the pedicled supraclavicular artery island flap (PSAIF) and 15 patients using the free radial forearm flap (FRFF). Flap survival, speech and swallowing function, and donor-site morbidity at the 6-month follow-up were evaluated. All the flaps were successfully transferred. No obvious complications

A functional evaluation of patients treated for oral cancer. A prospective study.

The oral function of 81 consecutive patients with intraoral squamous cell carcinoma was assessed at diagnosis and during the follow-up 1-2 years after diagnosis. Patients received either radiotherapy, surgery, or radiotherapy and surgery. Speech function and tongue mobility were better among patients who received radiotherapy alone than among patients treated with surgery or combined therapy. Subjective complaints about mucositis, poor dental status, and loss of teeth were most pronounced among patients who received radiotherapy. Patients with tumor recurrence reported a poorer quality of life

Grommets (ventilation tubes) for hearing loss associated with otitis media with effusion in children.

BACKGROUND: Otitis media with effusion (OME), or 'glue ear', is very common in children, especially between the ages of one and three years with a prevalence of 10% to 30% and a cumulative incidence of 80% at the age of four years. OME is defined as middle ear effusion without signs or symptoms of an acute infection. OME may occur as a primary disorder or as a sequel to acute otitis media. The functional effect of OME is a conductive hearing level of about 25 to 30 dB associated with fluid in the middle ear. Both the high incidence and the high rate of spontaneous resolution suggest that the p

The effect of early versus late onset of major motor epilepsy upon cognitive-intellectual performance.

Performance on the Wechsler Adult Intelligence Scale and Halstead neuropsychological measures were investigated in two groups of adult subjects with major motor epilepsy of early (0 to 5 yr) and later (17 to 50 yr) onset, and in two groups with early and later onset of brain damage without epilepsy. The two groups with early onset earned poorer scores on most tests than did the two groups with later onset. The findings suggest an adverse effect of cerebral dysfunction of early onset, whether accompanied by major motor seizures or not. Subjects with early onset of major motor seizures scored si

Palliative care symptoms of people living with rapidly progressive prion diseases: a systematic review.

BACKGROUND: Creutzfeldt-Jakob disease (CJD) is a rapidly progressive, fatal dementia with an average prognosis of four to six months. Palliative care, as a holistic person-centred approach to supporting people and families affected by life-limiting illness, may be beneficial. However, despite the rapid disease trajectory, relatively little is known about the palliative care symptoms and concerns of this population. This systematic review identifies and reports the palliative care symptoms of people living with sporadic, iatrogenic, and acquired CJD. METHOD: Studies were included if they report

Pursuit of Digital Innovation in Psychiatric Data Handling Practices in Ireland: Comprehensive Case Study.

BACKGROUND: Ireland is ranked among the most disadvantageous European countries in terms of mental health challenges. Contrary to general health services that primarily focus on diagnosis and treatment, the mental health sector in Ireland deals with highly sensitive psychiatric case notes based on patient-doctor conversations. Such data, therefore, must be collected, analyzed, and stored with an approach customized specifically for psychiatry. OBJECTIVE: This study's objective involves examining the state of data handling practices in the Irish Mental Health Services (MHS), identifying the sh

Revealing the Roles of Part-of-Speech Taggers in Alzheimer Disease Detection: Scientific Discovery Using One-Intervention Causal Explanation.

BACKGROUND: Recently, rich computational methods that use deep learning or machine learning have been developed using linguistic biomarkers for the diagnosis of early-stage Alzheimer disease (AD). Moreover, some qualitative and quantitative studies have indicated that certain part-of-speech (PoS) features or tags could be good indicators of AD. However, there has not been a systematic attempt to discover the underlying relationships between PoS features and AD. Moreover, there has not been any attempt to quantify the relative importance of PoS features in detecting AD. OBJECTIVE: Our goal was
