Double-inlet ventricle presenting in infancy. III. Outcome and potential for definitive repair.

The fate of 191 infants with double-inlet ventricle was studied to determine the influence of morphologic characteristics at presentation and subsequent management on the potential for, and timing of, definitive repair by the Fontan operation or ventricular septation. At presentation, 136 patients (71%) were potential candidates for a Fontan procedure. Actuarial survival was better than for those deemed unsuitable for either definitive option (n = 55; 68% versus 28% at 1 year; p less than 0.001), but still, only 78 patients (57%) were known to be alive and suitable candidates at 2 years of age

[Differential diagnosis of traumatic skull findings in infants: residues after vacuum extraction].

If autopsy findings in an infant show traumatic changes on the skullcap, these are not always the result of a postnatal trauma due to child abuse, a fall or another accidental event. With regard to differential diagnosis a birth trauma should also be kept in mind, so that the history of the delivery is important. The spectrum of possible residues after vacuum extraction (circular fracture and/or elevation of the outer table of skull bones, subperiostal and intraossal haematoma, extradural and subdural haemorrhage) is demonstrated by means of three examples from the forensic autopsy material.

Postmortem diffusion of tracheal lidocaine into heart blood following intubation for cardiopulmonary resuscitation.

This study investigated the postmortem diffusion of tracheal lidocaine into the blood after intubation in three individuals whose heart beat was not restored by cardiopulmonary resuscitation. The results are compared with those obtained in animal experiments using rabbits. The first human subject was a 3.5-month-old female baby who died of sudden infant death syndrome. She was autopsied approximately 20 h after death. A toxicological examination revealed the presence of 0.349 mg/L and 0.102 mg/L of lidocaine in the blood in the left and right ventricles of the heart, respectively. No lidocaine…

Longevity of neonatal ductal stenting for congenital heart diseases with duct-dependent pulmonary circulation.

INTRODUCTION: Ductal stent (DS) in duct-dependent pulmonary circulation is less morbid than neonatal Blalock-Taussig shunt. However, there is concern if DS provides an adequately long palliation before definitive repair. METHODS: This is a retrospective review of clinical follow-up of all consecutive infants after successful DS performed by a single operator. They were divided into three anatomic groups. Group A neonates had balloon valvotomy for critical pulmonary stenosis or pulmonary atresia with intact ventricular septum, who needed DS patency until the right ventricle was adequate to pro

Brief Resolved Unexplained Events: Analysis of an Apparent Life Threatening Event Database.

OBJECTIVE: To identify the proportion of patients previously diagnosed with apparent life-threatening events (ALTE) who would meet criteria for brief resolved unexplained events (BRUE) and to identify rates of adverse outcomes in subgroups: ALTE not meeting criteria for BRUE, lower-risk BRUE, and higher-risk ALTE. METHODS: We performed a secondary analysis of a single-center prospective registry of patients diagnosed with ALTE in a tertiary care emergency department from March 1, 1997 to October 31, 2007. We identified the proportion of patients meeting criteria for BRUE, and the proportion o

Risk markers for sudden unexpected death in epilepsy: an observational, prospective, multicentre cohort study.

BACKGROUND: Sudden unexpected death in epilepsy (SUDEP) is the leading cause of epilepsy-related mortality. Generalised-particularly nocturnal-convulsive seizures, longstanding epilepsy, and solitary living have been identified retrospectively as risk factors. No definitive electroclinical biomarkers have been prospectively ascertained. This study aimed to identify SUDEP risk markers using multimodality data with long-term follow-up. METHODS: This prospective, multicentre, observational cohort study, conducted at nine centres (eight in the USA and one in the UK), recruited children and adults

The New South Wales Sudden Cardiac Arrest Registry: A Data Linkage Cohort Study.

BACKGROUND: Sudden cardiac arrest (SCA) in young people aged 1 to 50 years often occurs with no presenting symptoms or risk factors prompting screening for cardiovascular disease prior to their cardiac arrest. Approximately 3,000 young Australians suffer from sudden cardiac death (SCD) each year, making this a major public health issue. However, there is significant variation in the way incidence is estimated resulting in discrepancy across reporting which impacts our ability to understand and prevent these devastating events. We describe the New South Wales (NSW) Sudden Cardiac Arrest Registr

Risk of cardiovascular disease in family members of young sudden cardiac death victims.

AIMS: Descriptive and genetic studies suggest that relatives of sudden cardiac death (SCD) victims have an increased risk of several cardiovascular diseases (CVDs). Given the severe consequences of undiagnosed CVD and the availability of effective treatment, the potential for prevention in this group is enormous if they do have an increased CVD risk. This nationwide prospective population-based cohort study described the risk of CVDs in relatives of young SCD victims, compared with the general population. METHODS AND RESULTS: All SCD victims aged 1-35 years in Denmark, 2000-2006, were identif

[The investigation of frequency of cystic echinococcosis in the autopsies committed in the Speciality Department of Istanbul Forensic Medicine Institute].

Cystic echinococcosis, which is caused by a parasite called Echinococcus granulosus, is a disease frequently find in the liver and in the lungs. In many countries, it is an important public-health problem. This study was done to find out the prevalence of Cystic echinococcosis in forensic autopsies performed for sudden death cases. This investigation was realized between July 2003-January 2004 at Istanbul Forensic Medicine Institution. During this six month period, total of 1687 autopsies were performed. Of those, 56 autopsy cases had macroscopic unknown cysts. The sera obtained from these 56

Severe endocrine and nonendocrine manifestations of the McCune-Albright syndrome associated with activating mutations of stimulatory G protein GS.

McCune-Albright syndrome (MCAS) is a sporadic disease classically including polyostotic fibrous dysplasia, café au lait spots, sexual precocity, and other hyperfunctional endocrinopathies. An activating missense mutation in the gene for the alpha subunit of GS, the G protein that stimulates cyclic adenosine monophosphate formation, has been reported to be present in these patients. The mutation is found in variable abundance in different affected endocrine and nonendocrine tissues, consistent with the mosaic distribution of abnormal cells generated by a somatic cell mutation early in embryogen

Pulmonary artery banding in infants and young children with end-stage left ventricular dilated cardiomyopathy: cohort study.

BACKGROUND: Dilated cardiomyopathy (DCM) is the most common cardiomyopathy, and 40--50% of patients may die or need a heart transplant in 5 years after diagnosis. Although heart transplantation is the most effective life-saving option of end-stage DCM, scarcity of donors and series of complications prevent many patients from receiving timely treatment. Pulmonary artery banding (PAB) is recently described as an alternative strategy for end-stage DCM, with low left ventricular function (LVEF) but preserved right ventricular function, may potentially restore heart function and delay the need for

Exploring the Discussion of Risk of Sudden Cardiac Death.

Sudden arrhythmic death syndrome (SADS), where death is secondary to cardiac arrhythmia, is associated with several cardiac ion channelopathies, including long QT syndrome and Brugada syndrome, as well as cardiomyopathies such as hypertrophic cardiomyopathy and dilated cardiomyopathy. Many of these conditions often present in childhood or adolescence. This study investigates how diagnoses of cardiac diseases associated with SADS are communicated within families. A questionnaire was distributed through cardiac disease-focused support groups and organizations. Data from 114 parents who have a ch

Pulmonary hypertensive vascular changes in lungs of patients with sudden unexpected death. Emphasis on congenital heart disease, Eisenmenger syndrome, postoperative deaths and death during pregnancy and postpartum.

AIMS: Pulmonary hypertension (PH) in asymptomatic patients is a rare cause of sudden death. This study aims to determine the incidence of this entity and raise awareness among pathologists. METHODS: We retrospectively investigated 44 cases of sudden unexpected death in relation to PH in patients not on antihypertensive therapy. This is the largest pathological study reported. RESULTS: We report 44 cases of sudden death due to PH in which 28 (63.63%) were female and 16 (36.36%) were male, and the age range was from 5 days to 93 years old (mean age: 24±20). The majority had no clinical evidenc

Clinical implication of hiatal hernia in patients with right isomerism.

OBJECTIVE: Despite a reported association between hiatal hernia in patients with heterotaxy syndrome, the clinical significance has remained unclear. Based on large patient cohorts, this study specifically aimed at defining the implication of hiatal hernia in patients with right isomerism. METHODS: From 1994 to 2002, 143 patients were identified as having right isomerism. Among them, 119 received ultrafast computed tomography (CT) to determine the presence of hiatal hernia as well as any cardiovascular anomalies. RESULTS: Hiatal hernia was found in 17 patients (seven females and 10 males, 14

International Survey of Growth Hormone and Sleep Evaluation Practices in Infants With Prader-Willi Syndrome.

OBJECTIVE: Guidelines encourage polysomnogram (PSG) prior to starting growth hormone (GH) in individuals with Prader-Willi Syndrome (PWS) to avoid adverse effects; however, this presents a challenge in infancy due to barriers regarding access and lack of normative data to guide recommendations. This international survey study assessed clinician practices regarding GH initiation and sleep evaluation in infants with PWS. The primary objective was to determine the prevalence of clinicians reporting they do not always perform a baseline PSG prior to GH Initiation and whether responses were associa

Investigating N-3 Fatty Acids to prevent Neonatal Tobacco-related outcomeS (INFANTS): study protocol for a double-blind, randomized, placebo-controlled parallel clinical trial of n-3 polyunsaturated fatty acids in pregnant smokers.

BACKGROUND: Tobacco use during pregnancy is the most important modifiable risk factor associated with adverse pregnancy outcomes, increasing the risk of preterm birth, intrauterine growth restriction and sudden infant death syndrome. Fewer than half of pregnant smokers can quit on their own. Identifying safe and effective therapies to prevent tobacco-related adverse pregnancy outcomes and/or increase smoking cessation in pregnant women would have a substantial public health impact. Cigarette smoking is associated with a relative deficiency in circulating n-3 long-chain polyunsaturated fatty ac…
