Repair of hand defects by transfer of free tissue flaps from toes.

INTRODUCTION: There are very few reports on the use of a free composite flap from the toe to repair small tissue defects in the hand. Here, we describe our experience using a free composite flap from the great toe and second toe. METHOD: Fifteen patients sought surgical treatment for tissue defects of the hand at our medical institution from July 2008 to December 2010. These defects included: dorsal defect of the distal thumb, dorsal-radialis defect of the proximal thumb, degloving injury of distal index finger, pulp defect of the middle finger and these were repaired with toe side pulp flaps

Sono-Anatomy and Histology of the Deep Fasciae in the Lower Limb.

The deep fasciae of the lower limb represent a highly specialized network essential for efficient force transmission, segmental stability and coordinated mechanics of locomotion. Its collagen-dense architecture forms a resilient and adaptive continuum that varies in thickness, fiber orientation and anchoring patterns from the thigh to the foot. Recently, the use of high-resolution ultrasonography, supported by anatomical and histological evidence, revealed more intricate organization than traditionally illustrated. This is particularly true in the distal leg and ankle, where lattice-like fiber

A constellation of anatomical variation: middle scalene, wrist extensor, and aortic arch variants share embryological origins in cervical somites.

During dissection of a 70-year-old male donor, several anatomical variations were observed, highlighted by a bilateral variant middle scalen … embryological origin. Clinically, these observations have implications in thoracic outlet syndrome, tendon transfer, and vascular surgery.

EditorialCommentary: In Situ Biceps Tenodesis Is Useful in the Treatment of Irreparable Rotator Cuff Tears.

For irreparable posterior superior rotator cuff tears (IPSRCTs), techniques other than reverse shoulder arthroplasty and tendon transfer are still under research; among these, reusing the long head of the biceps (LHB) is noteworthy. Various techniques of LHB reuse have been reported and can be divided into 3 biomechanical categories: restoration of the force chain of the rotator cuff, augmentation of the superior capsule, and combined force-chain restoration and superior capsule augmentation, among which in situ biceps tenodesis may be the simplest. LHB reuse for IPSRCTs is always combined wit…

Longitudinal Epiphyseal Bracket as an Overlooked Etiology of Congenital-Residual Hallux Varus: A Case Report.

Longitudinal epiphyseal bracket is a rare ossification disorder of the short tubular bones. The affected bone becomes deformed as a result of the bracket. The normal growth pattern cannot occur, and when it affects the first metatarsal bone, hallux varus may develop with the abnormal growth pattern. We present such a 6-year-old patient who had undergone surgery at 6 months of age for hallux varus and polydactyly. The deformity had worsened gradually after the initial operation because of the overlooked longitudinal epiphyseal bracket. The patient was treated with surgical excision of the epiph

[Clubfoot].

Clubfoot. The clubfoot is secondary to an early trouble of foot development at the end of the first trimester of pregnancy. This explains the structural disorders which concern all the tissues of mid-hindfoot and the leg and the tendency to relapse during growth. The diagnostic is triple: positive, etiological and scoring classification. A clinical and radiological checking is mandatory until the end of growth. The treatment is conservative combining casts, splints and physiotherapy. The indication of Achilles tenotomy, not systematic, is evaluated on clinical and radiological criteria. Noctur

Soft tissue release of the spastic hip by psoas-rectus transfer and adductor tenotomy for long-term functional improvement and prevention of hip dislocation.

The purpose of this study was to assess the long-term development of children with cerebral palsy treated with soft tissue releases of the hip and, if necessary, also of the hamstrings and the Achilles tendon. The follow-up had to consider the functional status and the hip centration. Seventy-one patients (46 with tetrapareses, 24 with dipareses, and one with triparesis) who underwent soft tissue releases of the hip by the so-called psoas-rectus transfer at an average age of 7 years were assessed preoperatively, after 1 year and at an average age of 19 years. Functional status and the radiolog

Shotgun arterial injuries of the extremities.

Even though shotgun injuries are often grouped with gunshot wounds, the former are ballistically and clinically far different from gunshot w … with shotgun injuries of the extremities often required secondary reconstructive procedures such as tendon transfer or bone and joint fusion. Despite these measures the outlook for the ultimate rehabilitation of the extremity after shotgun injury was dismal, whereas after gunshot wounds complete rehabilitation was the rule.

An unstable second metatarsophalangeal joint may produce pain in the forefoot. Eighteen patients (20 feet) had a transfer of the flexor digi … rative stiffness in the joint provided some of the joint stability seen in our patients. The flexor tendon transfer appears to be a satisfactory method to treating the unstable metatarsophalangeal joint and of relieving patients' pain, but may not, however, restore a normal alignment of the second toe. Correction of other forefoot deformities as hallux valgus and …

Hypoxia and hydrothoraces in a case of liver cirrhosis: correlation of physiological, radiographic, scintigraphic, and pathological findings. A case is reported of liver cirrhosis complicated by cyanosis and recurrent right hydrothorax. A diagnostic pneumoperitoneum demonstrated that direct movement of ascites through a diaphragmatic defect was responsible for the hydrothoraces. Pulmonary function tests between episodes of hydrothorax showed severe arterial hypoxaemia, a 23% right-to-left

The rectus abdominis free flap as an emergency procedure in extensive upper extremity soft-tissue defects.

Stable wound coverage after extensive soft-tissue loss of the upper extremity remains a difficult problem in the management of large defects of the upper limb. To prevent further tissue loss owing to infection or inadequate cover when important structures such as vessels, tendons, nerves, joints, and bones are exposed, various free flaps have been introduced into the therapeutic armamentarium of acute plastic surgical management options. Emergency or delayed early reconstruction has been proposed to prevent chronic infection and further tissue loss. We report a series of 12 emergency and delay

Role of the coracoid bone block in the modified Bristow procedure.

In 20 shoulders (19 patients) with a Bankart lesion, the isolated coracoid process was transferred as a bone graft to the anterior rim of the glenoid. The graft was positioned directly at the site of the Bankart lesion without displacing the insertion of the conjoined tendon or subscapularis. The average age of the patients was 22.5 years (range, 15 to 42). Follow-up examination at a minimum of 3 years (average, 55 months) showed 17 of 20 shoulders (85%) presented with good or excellent results. One recurrent dislocation was noted. There were no complications related to screw fixation of the c

Combination of Two Monoclonal Anti-Citrullinated Protein Antibodies Induced Tenosynovitis, Pain, and Bone Loss in Mice in a Peptidyl Arginine Deiminase-4-Dependent Manner.

OBJECTIVE: The appearance of anti-citrullinated protein antibodies (ACPAs) in the circulation represents a major risk factor for developing rheumatoid arthritis (RA). Patient-derived ACPAs have been shown to induce pain and bone erosion in mice, suggesting an active role in the pathogenicity of RA. We undertook this study to investigate whether ACPAs can induce tenosynovitis, an early sign of RA, in addition to pain and bone loss and whether these symptoms are dependent on peptidyl arginine deiminase 4 (PAD4). METHODS: Monoclonal ACPAs generated from plasma cells of RA patients were transferr

Combined medial patellofemoral and medial patellotibial reconstruction for patellar instability: a PRISMA systematic review.

BACKGROUND: The medial patellofemoral ligament (MPFL) works in association with the medial patellotibial ligament (MPTL) and the medial patellomeniscal ligament (MPML) to impart stability to the patellofemoral joint. The anatomy and biomechanical characteristics of the MPFL have been well described but little is known about the MPTL and MPML. Several reconstruction procedures of the MPFL with semitendinosus, gracilis, patellar and quadriceps tendons, allografts and synthetic grafts have been described. No clear superiority of one surgical technique over another is evident. METHODS: A systemat

Methods for Congenital Thumb Hypoplasia Reconstruction. A Review of the Outcomes for Ten Years of Surgical Treatment.

BACKGROUND AND OBJECTIVES: Congenital thumb hypoplasia is a rare deformity of upper extremity. The incidence for thumb hypoplasia grade II-V is 1:10,000 newborns per year in Latvia. A technique for extensor indicis proprius (EIP) tendon transfer with subperiosteal fixation was developed and used for thumb hypoplasia grades II and IIIa. Pollicization or second-toe-to-hand transplantation with metatarsophalangeal (MTP) joint arthrodesis was used for the reconstruction of hypoplasia grade IIIb-V. The aim of this retrospective cohort study is to evaluate the outcomes for reconstruction techniques …

Procurement of Extended Vascularized Skin Flaps from the Donor Enables Hand Transplantation in Severe Upper Extremity Burns: An Anatomical Study.

BACKGROUND: Hand transplantation in patients with severe upper extremity burns can be associated with an increased risk of exposure of vessels, tendons, and nerves because of extensive skin and soft-tissue deficit. This study evaluated how to reliably transfer additional extended skin flaps with a standard hand allograft. METHODS: Twenty-five upper extremities were used. Sixteen were injected with latex to map the perforating branches of the brachial, superior ulnar collateral, radial, ulnar, and posterior interosseous arteries. Nine hand allografts were procured, injected with blue ink throu
