Skull tap induced vestibular evoked myogenic potentials: an ipsilateral vibration response and a bilateral head acceleration response?

OBJECTIVE: To explore the mechanisms for skull tap induced vestibular evoked myogenic potentials (VEMP). METHODS: An electro-mechanical "skull tapper" (that provided a constant stimulus intensity) was used to test the effects of different midline stimulus sites/directions in healthy subjects (n=10) and in patients with severe unilateral loss of vestibular function (n=8). RESULTS: The standardized midline skull taps caused highly reproducible VEMP. There were highly significant differences in amplitude and latency in both normals and patients depending on site/direction of tapping (suggesting

Distortion product otoacoustic emissions after intratympanic gentamicin therapy for unilateral Ménière's disease.

The treatment of patients with Ménière's disease that do not respond to conventional therapy becomes complicated, particularly when taking into account the issue of hearing damage as well as the control of vertigo. Treatment often involves the intratympanic administration of gentamicin, for which different protocols are used. Hence, it is important that we better understand how this treatment influences hearing, beyond mere audiometric assessments. The aim of this prospective study was to evaluate the effect of intratympanic gentamicin treatment for Ménière's disease on cochlear function, as a

Changes in the three-dimensional angular vestibulo-ocular reflex following intratympanic gentamicin for Ménière's disease.

The 3-dimensional angular vestibulo-ocular reflexes (AVOR) elicited by rapid rotary head thrusts were studied in 17 subjects with unilateral Ménière's disease before and 2-10 weeks after treatment with intratympanic gentamicin and in 13 subjects after surgical unilateral vestibular destruction (SUVD). Each head thrust was in the horizontal plane or in either diagonal plane of the vertical semicircular canals, so that each head thrust effectively stimulated only one pair of canals. The AVOR gains (eye velocity/head velocity during the 30 ms before peak head velocity) for the head thrusts exciti

Deviation of the subjective vertical in long-standing unilateral vestibular loss.

We evaluated changes in the subjectively perceived gravitational vertical as an index of imbalance in the function of the right and left otolith organs. In addition to normal subjects (n = 25), we measured patients with a longstanding (mean 4.5 year +/- 3.2 SD; range 0.5-11.5 years) unilateral vestibular loss after surgery for acoustic neuroma (n = 32), patients with partial unilateral vestibular loss (n = 7) and patients with bilateral vestibular hyporeflexia (n = 8). Normal subjects could accurately align a vertical luminous bar to the gravitational vertical in an otherwise completely dark r

Simulation of Space-Adaptation Syndrome on Earth.

The three Spacelab D-1 Scientist Astronauts were exposed to a 1.5 h centrifuge run in the supine position, resulting in a linear 3 g acceleration. They used their space experience to evaluate their readaptation to normal gravity and compared their observations with 'Space Adaptation Syndrome'. After the centrifuge runs, the vestibular visual system appeared to be modified in a very specific and reproducible manner. Readaptation to the normal 1 g environment took at least 6 h. During this period there was a striking similarity to the astronauts' experience during adaptation to weightlessness in

Eye movements in patients with Wallenberg's syndrome.

We studied electro-oculographic and videotape recordings of eye movements in six patients with Wallenberg's syndrome. With fixation, all patients had a spontaneous rotatory nystagmus with the fast phase directed toward the intact side. With loss of fixation, the patients' eyes deviated tonically toward the side of the lesion. Voluntary and involuntary saccades had larger amplitude when directed toward the side of the lesion than away from it. The spontaneous nystagmus predictably interacted with all slow eye movements, producing asymmetric smooth-pursuit, optokinetic, and vestibular responses.

Brain control of conjugate horizontal and vertical eye movements: a survey of the structural and functional correlates.

(1) It appears that all oculomotor pathways originating within the cerebrum and mediating stimulations and lesions, project from the two sides of the brain through the diencephalon to the brain-stem. (2) The pathways subserving horizontal movements decussate at the level of the oculomotor and trochlear nuclei, across the midsagittal plane. The direction of vector action within the brain above the 'electroanatomical' oculomotor decussation is predominately contraversive; below this levelit is ipsiversive. (3) The pontine reticular formation, the abducens and oculomotor nuclei and the median lon

Hypoplasia of the vestibular aqueduct and endolymphatic sac in endolymphatic hydrops.

Four temporal bones from three previously reported cases showing endolymphatic hydrops and pathology of the vestibular aqueduct (VA) and endolymphatic sac (ES) were investigated further. Pathology of the VA and ES was studied by measuring the sizes of the VA and ES, paying particular attention to the proximal rugose portions. A medial-view graphic reconstruction was created to delineate the course and size of the VA, as well as its correlation with neighboring structures (posterior canal and cochlea). In addition, 20 control temporal bones were selected and measured for quantitative study of t

Sudden deafness in relapsing polychondritis. A scanning electron microscopy study.

The left ear of a 57-year-old female who suffered sudden deafness during the course of relapsing polychondritis was examined under a scanning electron microscope. Marked degeneration of the organ of Corti in all turns and dislocated and encapsulated tectorial membrane were found in the cochlea. Marked decrease in number of the sensory cells in the utricular and saccular maculae and total loss of sensory hair bundles in the ampullary cristae of the semicircular canals were seen in the vestibule. These findings strongly suggest that the cause of sudden deafness in this case might be viral. The u

Vestibular function in panic disorder patients: a vestibular-evoked myogenic potentials and video head impulse test study.

PURPOSE: To evaluate the function of the utriculus and sacculus and their central connections by ocular and cervical vestibular-evoked myogenic potentials (oVEMPs and cVEMPs), and the function of high-frequency VOR of the semicircular canals by video head impulse test (vHIT) in patients with panic disorder (PD). METHODS: Forty-eight patients with PD (21 with agoraphobia) and 20 sex- and age-matched healthy controls took part in the investigation. The vestibulo-ocular reflex (VOR) gains and latencies and peak-to-peak amplitudes of sound-induced VEMPs were measured and compared with those of he

Handedness-dependent functional organizational patterns within the bilateral vestibular cortical network revealed by fMRI connectivity based parcellation.

Current evidence points towards a vestibular cortex that involves a multisensory bilateral temporo-parietal-insular network with a handedness-dependent hemispheric lateralization. This study aimed to identify handedness-dependent organizational patterns of (lateralized and non-lateralized) functional subunits within the human vestibular cortex areas. 60 healthy volunteers (30 left-handed and 30 right-handed) were examined on a 3T MR scanner using resting state functional MRI (fMRI). The data was analyzed in four major steps using a functional connectivity based parcellation (fCBP) approach: (1
