A 53-year-old man with a history of lung cancer mestastic to the brain and pelvis presented with hypotension and massive GI bleeding. He had no past history of gastrointestinal disease or bleeding. The patient was found to have an unusual focal source of bleeding--a large ulcerated Meckel's diverticulum with acute hemorrhage. After investigating an outbreak of Lyme Disease among counselors at a summer camp in Kent County, Maryland in 1994, we wanted to determine the incidence of Lyme Disease (LD) at the camp the following summer and identify risk factors for tick exposure. Any ticks that were detected on campers' skin or clothing were collected by the camp nurse and we studied them for infection with Borrelia burgdorferi. This report describes the cases of two patients who underwent laparoscopic repair for treatment of Morgagni-Larrey hernia at United Hospital Center in Clarksburg. The first patient was a 40-year-old woman complaining of epigastric discomfort and tenderness. Emphysematous cystitis (EC), a rare form of cystitis, is often an incidental radiological finding but it can be associated with diffuse abdominal or suprapubic pain. The clinical course can vary from asymptomatic infection to fulminant sepsis. We present the case of a 79-year-old woman with diffuse abdominal pain, back pain accompanied with low-grade fevers, urinary frequency, urinary urgency, and emesis who was ultimately found to have emphysematous cystitis. A review of the etiology, pathogenesis, diagnosis and treatment options follows.

Penile strangulation is a devastating injury with a spectrum of causes and possible outcomes. Causes can be intentional or unintentional. Objects such as ball bearings, rings, condoms, and rubberbands have caused this condition, as have hair and plastibell rings. This article describes the cases of five patients with strangulation injuries of the penis. For each case, the cause of strangulation, the treatment and the outcome are presented.

The finding of toxic shock syndrome (TSS) is a rare, potentially fatal illness that physicians often associate with young, menstruating females. However, TSS is not exclusively a disease of females. We report the unusual case of an adult male patient who presented to the Emergency Department at Ruby Memorial Hospital in Morgantown with toxic shock syndrome secondary from a fish tooth suffered while trout fishing. The diagnostic features of TSS and treatment are also discussed in this article.

An extremely rare case of pulmonary epitheloid hemangioendothelioma (PEH), previously known as intravascular bronchoalveolar tumor (IVBAT), in a 38-year-old female is presented. This patient had a history of rheumatoid arthritis and bilateral multiple small pulmonary nodules which progressed over the years. The histopathological diagnosis of PEH was confirmed by immunohistochemical stains. Prognosis of this tumor is very unpredictable. There is no effective treatment for pulmonary epitheloid hemangioendothelioma.

Spontaneous extracranial internal carotid artery dissection (ICAD) is a rare vascular event. Even in the presence of existing signs and symptoms, the diagnosis can be missed by experienced clinicians of all specialties. This is a report of a patient who presented with symptomatic bilateral dissection manifested as partial Horner's syndrome. The diagnosis was made after several clinical evaluations with the use of duplex ultrasound and angiography.

A 42-year-old multiparous pregnant woman presented with swelling and pain of the left arm at 34 weeks gestation. She had no discoloration of her arm nor a loss of radial pulse. Latex is a natural product obtained from rubber trees. Sensitization and subsequent exposure to latex products can lead to allergic (immediate hypersensitivity) reactions including anaphylaxis. Gloves are the largest source of latex exposure, and the majority of serious latex reactions occur in medical settings. Allergen testing and avoidance in history positive or high risk groups (medical personnel, spina bifida patients and rubber workers) is presently the best treatment option.

Autoimmune hepatitis (AIH) is usually diagnosed after other viral, metabolic or drug-induced etiologies have been ruled out. Although uncommon in children, the disease can manifest itself with severe hepatocellular damage and early fibrosis. Early diagnosis and aggressive treatment may improve morbidity and decrease mortality. In other cases, liver transplantation is the final rescue for these children.
