Patients with painful bladder syndrome have altered response to thermal stimuli and catastrophic reaction to painful experiences.

AIMS: To compare cutaneous sensory thresholds, habituation to somatic stimuli, and tendency towards catastrophic reaction to painful stimuli in patients with Painful Bladder Syndrome (PBS) to controls without PBS. METHOD: Thermal and vibratory sensory thresholds were established in 11 PBS patients and 10 controls at C5, T1, T12, and S3 dermatomes. Supra-threshold thermal stimuli were then applied at T12 and S3 for 60 sec while patients periodically rated the intensity of stimuli using a visual analog scale. A Pain Catastrophizing Scale (PCS) questionnaire was also completed by all participant

Molecular Signatures of Bladder Pain Syndrome: A Systematic Review Towards Phenotype-Informed Diagnostics.

INTRODUCTION AND HYPOTHESIS: Bladder painsyndrome (BPS) is a disabling condition diagnosed clinically, characterised by marked heterogeneity in cystoscopic phenotype and treatment response. Molecular mechanisms remain incompletely defined, and no validated biomarkers currently support diagnosis or phenotypic stratification. The objective was to synthesise molecular evidence underpinning BPS and evaluate biomarker candidates relevant to phenotype-informed diagnostics. METHODS: A Preferred Reporting Items for Systematic reviews and Meta-Analyses-guided search of PubMed and the Cochrane Library

Pulsed Radiofrequency of the Sacral Dorsal Root Ganglia as a Potential Rescue Therapy for Refractory Chronic Pelvic Pain: A Case Series.

BACKGROUND Chronic pelvic pain (CPP) is a prevalent, multifactorial condition with neuropathic, visceral, myofascial, and psychogenic components; a subset of patients remain refractory to multimodal pharmacotherapy and conventional interventional procedures. Pulsed radiofrequency (PRF) of the dorsal root ganglia (DRG) is a minimally invasive neuromodulatory technique that can attenuate pathological nociceptive transmission while preserving neural integrity. We describe 3 patients with severe, treatment-refractory CPP of predominantly neuropathic origin who underwent sacral DRG PRF as rescue th

Green Cystoscopy: Does Minimizing the Use of Drapes Increase Infection Rates?

IMPORTANCE: The U.S. health care system has an enormous carbon footprint made worse by the escalating use of single-use supplies. Emerging evidence suggests that smaller surgical fields ("green" draping) may represent a safe alternative to traditional draping. OBJECTIVES: The aim of the study was to determine if the proportion of cases treated for culture-proven urinary tract infection (UTI) within 2 weeks of operating room cystoscopy after the green draping protocol implementation is noninferior to preprotocol cases. Secondary objectives included risk factors for UTI and waste and cost savin

Intravesical substance P enhances bladder afferent nerve activity without the influence of the micturition reflex.

PURPOSE: Substance P (SP) is important in the physiological and pathophysiological functions of the bladder, including bladder sensory disorders like interstitial cystitis/bladder pain syndrome (IC/BPS) and overactive bladder (OAB). However, the impact of SP on sensory function is not fully understood. The aim of this study was to determine the effects of SP on bladder function in relation to capsaicin, specifically on bladder afferent nerve activity. METHODS: We used female Sprague-Dawley rats. The in vitro organ bath study assessed SP-induced detrusor contraction in intact and inside-out pr

Systemic ozone therapy improves the quality of life in patients with bladder pain syndrome.

BACKGROUND: Bladder pain syndrome (BPS), also known as interstitial cystitis, is a chronic condition characterized by pelvic pain and urinary symptoms that severely impair patients' quality of life (QoL). The current therapeutic options often yield suboptimal results, prompting interest in complementary treatments. Systemic ozone therapy, known for its anti-inflammatory, immunomodulatory, and analgesic effects, may represent a promising adjunctive treatment for BPS. OBJECTIVE: This study aimed to evaluate the efficacy and safety of systemic ozone therapy in improving the self-perceived QoL in

Pentosan Polysulfate Sodium Causes Diminished Function and Subtle Morphological Changes in Retina and RPE of Mice.

PURPOSE: There are numerous reports of a distinctive maculopathy in adults exposed to pentosan polysulfate sodium (PPS), a drug prescribed to treat bladder discomfort associated with interstitial cystitis. We tested whether PPS treatment of mice injures RPE or retina to provide insight into the etiology of the human condition. METHODS: Mice were fed PPS-supplemented chow over 14 months. RPE and retinal function was assessed by electroretinography (ERG) regularly. Following euthanasia, one eye was used for sagittal sectioning and histology, the contralateral for RPE flatmounting. ZO-1 positive

Latent bladder hypersensitivity induced by neonatal cystitis in rats unmasked by segmental but not hetero-segmental inflammation.

Rats which experienced neonatal bladder inflammation (NBI) have been demonstrated to exhibit latent bladder hypersensitivity with a nociceptive component that becomes unmasked by a second inflammatory insult as an adult. Manifested as augmented reflex and neuronal responses to urinary bladder distension (UBD), these NBI-induced changes are revealed by using inflammation of nearby structures as an adult pretreatment. The effect of inflammation in distant structures is not known. These studies examined visceromotor reflex responses and lumbosacral spinal dorsal horn neuronal responses to UBD in

Associations Between Urological Chronic Pelvic Pain Syndrome Symptom Flares, Illness Impact, and Health Care Seeking Activity: Findings From the Multidisciplinary Approach to the Study of Chronic Pelvic Pain Symptom Patterns Study.

PURPOSE: Most studies on interstitial cystitis/bladder pain syndrome and chronic prostatitis/chronic pelvic pain syndrome use typical or average levels of pelvic pain or urological symptom intensity as their outcome, as both are associated with reduced quality of life. Symptom exacerbations or "flares" have also been found to be associated with reduced quality of life, but no studies, to our knowledge, have investigated whether these associations are independent of typical pelvic pain levels and thus might be useful additional outcome measures (or stated differently, whether reducing flare fre

Pentosan polysulfate exerts anti-inflammatory effect and halts albuminuria progression in diabetic nephropathy: Role of combined losartan.

Pentosan polysulfate sodium (PPS) is a polysulfated glycosaminoglycan approved for the treatment of interstitial cystitis. It also showed renoprotective effects in chronic kidney injury models, for example, 5/6 nephrectomy and diabetic nephropathy (DN). In the present study, we addressed to evaluate the therapeutic value of PPS in DN of rats in combination with losartan (LSR). Sixty male Sprague-Dawley rats were randomly divided into six groups: control group, untreated diabetic groups (8 and 16 weeks after diabetes induction by streptozotocin "STZ", 60 mg/kg, intraperitoneal [I.P.]), LSR-trea

Could urinary nerve growth factor be a biomarker for overactive bladder? A meta-analysis.

OBJECTIVE: The previously reported association between urinary nerve growth factor (NGF) and overactive bladder (OAB) was controversial. We performed this meta-analysis based on current available studies to sum up all evidence on this association. METHODS: Studies were identified by searching PubMed, Embase, and Cochrane library from inception to September 2016. Pooled standardized mean difference (SMD) with their corresponding 95% confidence intervals (CIs) were calculated using a random-effects model. RESULTS: A total of 17 published studies were included in this meta-analysis. Among the s

A multiplexed analysis approach identifies new association of inflammatory proteins in patients with overactive bladder.

Overactive bladder (OAB) is a common debilitating bladder condition with unknown etiology and limited diagnostic modalities. Here, we explored a novel high-throughput and unbiased multiplex approach with cellular and molecular components in a well-characterized patient cohort to identify biomarkers that could be reliably used to distinguish OAB from controls or provide insights into underlying etiology. As a secondary analysis, we determined whether this method could discriminate between OAB and other chronic bladder conditions. We analyzed plasma samples from healthy volunteers (n = 19) and p

Evaluation of the efficacy of postmortem human bladder tissue as a normal comparator for case-controlled gene expression studies in urology.

AIMS: Interstitial cystitis/bladder pain syndrome (IC/BPS) is a poorly understood disease with no absolute diagnostic marker. A molecular-based tool (biomarker) for IC/BPS diagnosis would have immediate clinical utility. We have generated a bank of bladder biopsy tissue from IC/BPS patients and require a control group for comparative gene expression studies. The objective of this pilot study was to investigate the feasibility of cadaveric bladder specimens as a viable source of control tissue. METHODS: Cryopreserved cadaveric bladder specimens were obtained through the National Disease Resear

Increased Risk of Stroke in Patients With Fibromyalgia: A Population-BASED Cohort Study.

Neuropsychiatric diseases might enhance stroke development, possibly through inflammation and atherosclerosis. Approximately 25% to 40% of patients with stroke, largely younger patients, are not associated with any conventional stroke risk factors. In this research, we explored whether fibromyalgia (FM), a neuropsychosomatic disorder, increases stroke risk. From a claims dataset with one million enrollees sourced of the Taiwan National Health Insurance database, we selected 47,279 patients with FM and randomly selected 189,112 age- and sex-matched controls within a 3-year period from January 1,

Urological chronic pelvic pain syndrome flares and their impact: qualitative analysis in the MAPP network.

INTRODUCTION AND HYPOTHESIS: Although in-depth qualitative information is critical to understanding patients' symptom experiences and to developing patient-centered outcome measures, only one previous qualitative study has assessed urological chronic pelvic pain syndrome (UCPPS) symptom exacerbations ("flares"). METHODS: We conducted eight focus groups of female UCPPS (interstitial cystitis/bladder pain syndrome) patients at four sites from the MAPP Research Network (n = 57, mean = 7/group) to explore the full spectrum of flares and their impact on patients' lives. RESULTS: Flare experiences

The Glt1 glutamate receptor mediates the establishment and perpetuation of chronic visceral pain in an animal model of stress-induced bladder hyperalgesia.

Psychological stress exacerbates interstitial cystitis/bladder pain syndrome (IC/BPS), a lower urinary tract pain disorder characterized by increased urinary frequency and bladder pain. Glutamate (Glu) is the primary excitatory neurotransmitter modulating nociceptive networks. Glt1, an astrocytic transporter responsible for Glu clearance, is critical in pain signaling termination. We sought to examine the role of Glt1 in stress-induced bladder hyperalgesia and urinary frequency. In a model of stress-induced bladder hyperalgesia with high construct validity to human IC/BPS, female Wistar-Kyoto
