Effect of High-Flow Nasal Cannula on Expiratory Pressure and Ventilation in Infant, Pediatric, and Adult Models.

BACKGROUND: Heated and humidified high-flow nasal cannula (HFNC) is a widely used form of respiratory support; however, data regarding optimal flows for a given patient size or disease state are lacking. A comprehensive study of the physiologic effects of HFNC is needed to better understand the mechanisms of action. The objective of the current study was to quantify the effect of HFNC settings in age-specific, anatomically correct nasal airways and spontaneously breathing lung models. We hypothesized that there is an effect of flow on pressure and ventilation. METHODS: Three-dimensionally pri

Cough and expiration reflexes elicited by inhaled irritant gases are intensified in ovalbumin-sensitized mice.

This study was designed to determine the effect of active sensitization with ovalbumin (Ova) on cough responses to inhaled irritant gases in mice. Conscious mice moved freely in a recording chamber, while the pressure change in the chamber and audio and video signals of the mouse movements were recorded simultaneously to measure the frequencies of cough reflex (CR) and expiration reflex (ER). To further verify the accuracy of cough analysis, the intrapleural pressure was also recorded by a telemetry sensor surgically implanted in the intrapleural space in a subgroup of mice. During the irritan

Is tidal expiratory flow limitation predictive of sleep-related disorders in the elderly?

Sleep-related disorders represent an important health burden and their prevalence increases with age. In patients with snoring or sleepiness, the presence of expiratory flow limitation (EFL), determined via the negative expiratory pressure (NEP) method, is related to the apnoea/hypopnoea index (AHI). In this study, we examined whether EFL can be used to predict obstructive sleep apnoea syndrome (OSAS) in healthy asymptomatic older subjects. A group of 72-yr-old subjects (n = 448, 44% males) with a mean body mass index of 25.5±3.8 kg·m(-2) were examined. All subjects underwent spirometry, NEP (

Asthma in athletes: diagnostic challenges and management strategies.

PURPOSE OF REVIEW: Exercise-induced bronchoconstriction (EIB) and exercise-induced asthma (EIA) are common conditions among athletes and represent an important clinical issue with potential implications for both respiratory health and athletic performance. The prevalence of asthma in athletes is higher than in the general population, likely due to the combined effects of increased ventilatory demand and environmental exposures. RECENT FINDINGS: The pathophysiology of EIB is multifactorial and involves osmotic and thermal mechanisms related to airway dehydration during exercise-induced hyperve

A Pt/Pd-Decorated SnO2-Based Electronic Nose for High-Precision Breath Analysis in Diagnosing SIBO.

Exhaled human breath contains disease-related biomarkers, enabling non-invasive medical diagnostics. Small intestinal bacterial overgrowth (SIBO) is traditionally diagnosed through hydrogen measurement, a method that offers limited specificity and sensitivity. Herein, we introduce a simplified Pt/Pd-SnO2 electronic composed of using only two sensing materials to achieve multi-gas detection. By simultaneously monitoring H2, CO, ethanol, and acetone, the system provides a more informative diagnostic profile than conventional single-marker tests. Furthermore, the electronic nose employs dual oper

Electrochemical Biosensing Strategies for Oral Health: From Engineered Interfaces to Advanced Transistor Architectures.

Oral diseases represent a major global health burden, underscoring the need for sensitive, accessible, and noninvasive diagnostic technologies. Electrochemical biosensing offers a powerful route for point-of-care oral health monitoring by translating biomolecular interactions in saliva, gingival crevicular fluid, and exhaled breath condensate into quantifiable electrical signals. This review systematically discusses electrochemical biosensing strategies for oral disease diagnosis, beginning with the structure and operating mechanisms of electrochemical sensors and then summarizing their applic

Superhydrophilic Silica-Coated Au Nanoislands for Efficient Capture and Enhanced SERS Detection of Airborne Biomarkers.

Exhaled breath aerosols contain critical biomarkers for noninvasive and cost-effective early detection of respiratory and other infectious diseases. However, current methods still struggle with a low capture efficiency and limited detection sensitivity for trace biomarkers in complex aerosols. Here we report superhydrophilic ultrathin silica-coated gold nanoislands (scAuNIs) for the highly efficient capture and SERS detection of airborne biomarkers. The scAuNIs are wafer-level fabricated by repeated dewetting of gold thin films, followed by a 2 nm silica film deposition using atmospheric press

A baseline study of interpretable machine learning using GC-MS breath VOCs for classifying asthma, bronchiectasis, and COPD.

Accurate differentiation among asthma, bronchiectasis, and chronic obstructive pulmonary disease (COPD) remains a critical challenge due to overlapping clinical symptoms and limitations of conventional diagnostic tools. This study establishes a transparent, reproducible baseline using gas chromatography-mass spectrometry (GC-MS) data derived from exhaled breath to classify asthma, bronchiectasis, and COPD. Using a publicly available clinical dataset comprising 121 breath samples and 76 shared volatile organic compounds (VOCs), we evaluated seven supervised classifiers under nested cross-valida

A model for maxilloturbinate morphogenesis in seals.

The nasal cavities of mammals contain the maxilloturbinate bones, which are involved in reducing heat and water losses. The maxilloturbinates of Arctic seals develop into particularly elaborate labyrinthine patterns, which are well adapted to retain heat and moisture from exhaled gas. These structures develop prenatally and continue to grow postnatally. The developmental mechanism of labyrinthine patterning is unknown. Here we report a model of maxilloturbinate pattern formation in prenatal and juvenile seals based on a simple algorithmic description and three key parameters: target turbinate

Targeted Biologic Therapies in Severe Asthma: Mechanisms, Biomarkers, and Clinical Applications.

Asthma represents a heterogeneous disorder characterized by a dynamic balance between pro-inflammatory and anti-inflammatory forces, with allergic sensitization contributing substantially to airway hyperresponsiveness and remodeling. Central to its pathogenesis are cytokines such as IL-4, IL-5, IL-13, IL-17, and IL-33, which drive recruitment of eosinophils, neutrophils, and other effector cells, thereby precipitating episodic exacerbations in response to viral and environmental triggers. Conventional biomarkers, including blood and sputum eosinophil counts, IgE levels, and fractional exhaled

State of the Art Review: Glucagon-Like Peptide-1 in Obesity-Related Asthma.

Asthma is a heterogeneous condition characterized by chronic airway inflammation, airway hyperresponsiveness, and mucin hypersecretion. Obesity-related asthma is one phenotype of asthma with significant metabolic dysregulation. A complete understanding of obesity-related asthma remains elusive, but it is most often characterized by the absence of hallmark features of Type-2 (T2) high asthma, such as eosinophilia or elevated exhaled nitric oxide (NO) levels. Patients with obesity-related and T2 low asthma with or without type 2 diabetes mellitus (T2DM) experience worse clinical outcomes, includ

Correlation of cervical-inspiratory-muscle electromyography and oxygen uptake during treadmill walking.

For measurements of exercise intensity, an individual's oxygen uptake (V̇O2) is measured with an exhaled gas analyzer that involves a mask, but exercise coaching would benefit if an individual's V̇O2 could be estimated with more easily obtained predictors. We investigated the predictability of V̇O2 by electromyography (EMG) of the neck inspiratory muscles. We analyzed the EMG results of the sternocleidomastoid (EMGst) and scalene (EMGsc) muscles of 14 healthy adults who performed a treadmill exercise load test. Their V̇O2, inspiratory flow rate, and heart rate were simultaneously recorded duri

Exploring the Potential Role of Metabolomics in COPD: A Concise Review.

Chronic Obstructive Pulmonary Disease (COPD) is a pathological condition of the respiratory system characterized by chronic airflow obstruction, associated with changes in the lung parenchyma (pulmonary emphysema), bronchi (chronic bronchitis) and bronchioles (small airways disease). In the last years, the importance of phenotyping and endotyping COPD patients has strongly emerged. Metabolomics refers to the study of metabolites (both intermediate or final products) and their biological processes in biomatrices. The application of metabolomics to respiratory diseases and, particularly, to COPD

Highly sensitive and real-time detection of acetone biomarker for diabetes using a ZnO-coated optical fiber sensor.

This work presents a ZnO-coated no-core optical fiber sensor (OFS) designed for the highly sensitive detection of acetone vapor. Acetone is a key biomarker for diabetes, which is linked to blood glucose levels and can be detected non-invasively through breath analysis. The OFS utilizes a no-core fiber (NCF) as the sensing region, coated with a thin layer of ZnO nanoparticles to enhance evanescent field interaction with the VOCs at the fiber interface. The NCF segment, optimized to 3.4 cm, maximizes coupling efficiency through multi-mode interference (MMI). The OFS was tested with six different

Microfluidic Chip-based Enrichment and Nucleic Acid Extraction for Quantitative Detection of Mycobacterium Smegmatis in Aerosols.

Tuberculosis (TB) is ranked as the third most prevalent infectious disease globally. Early detection and treatment are crucial for effective management. Conventional diagnostic methods primarily rely on sputum samples, which present challenges in accessibility and have limited accuracy in certain populations such as children, individuals with HIV, and those with extrapulmonary TB. To address the need for point-of-care diagnostics, this study introduces a rapid diagnostic approach for TB using exhaled breath aerosol as a more easily obtainable specimen. Mycobacterium smegmatis, a non-pathogenic

Investigation of uncertainty in internal target volume definition for lung stereotactic body radiotherapy.

This study evaluated the validity of internal target volumes (ITVs) defined by three- (3DCT) and four-dimensional computed tomography (4DCT), and subsequently compared them with actual movements during treatment. Five patients with upper lobe lung tumors were treated with stereotactic body radiotherapy (SBRT) at 48 Gy in four fractions. Planning 3DCT images were acquired with peak-exhale and peak-inhale breath-holds, and 4DCT images were acquired in the cine mode under free breathing. Cine images were acquired using an electronic portal imaging device during irradiation. Tumor coverage was eva

Non-Coding RNAs in Pulmonary Diseases: Comparison of Different Airway-Derived Biosamples.

Due to their structural conservation and functional role in critical signalling pathways, non-coding RNA (ncRNA) is a promising biomarker and modulator of pathological conditions. Most research has focussed on the role of microRNAs (miRNAs), long non-coding RNAs (lncRNAs) and circular RNAs (circRNAs). These molecules have been investigated both in a cellular and an extracellular context. Sources of ncRNAs may include organ-specific body fluids. Therefore, studies on ncRNAs in respiratory diseases include those on sputum, bronchoalveolar lavage fluid (BALF) and exhaled breath condensate (EBC).

A pattern-recognition-based clustering method for non-invasive diagnosis and classification of various gastric conditions.

Conventional endoscopic biopsy tests are not suitable for early detection of the acute onset and progression of peptic ulcer as well as various gastric complications. This also limits its suitability for widespread population-based screening and consequently, many people with complex gastric phenotypes remain undiagnosed. Here, we demonstrate a new non-invasive methodology for accurate diagnosis and classification of various gastric disorders exploiting a pattern-recognition-based cluster analysis of a breathomics dataset generated from a simple residual gas analyzer-mass spectrometry.
