A pilot study to evaluate the effectiveness of small intestinal submucosa used to repair spinal ligaments in the goat.

BACKGROUND CONTEXT: Destabilization of the lumbar spine results from sacrifice of the anterior longitudinal ligament and disc when removed for graft or cage placement. In a similar fashion, transection of the interspinous ligament during surgical approaches to the posterior spine may result in segmental instability. Such instability can cause abnormal motion or implant migration resulting in a higher incidence of pseudarthrosis. Small intestinal submucosa (SIS) is a naturally occurring extracellular collagen-based matrix, which is derived from porcine small intestine. SIS contains cytokines an

Complications of endocapsular tension ring implantation in a child with Marfan's syndrome.

A 4-year-old boy with Marfan's syndrome had severe visual impairment after subluxation of the crystalline lens with bisection of the pupil. In the first eye, a capsular tension ring and an intraocular lens (IOL) were uneventfully placed in the capsular bag after phacoemulsification. During implantation of the endocapsular tension ring in the second eye, an inadvertent tear of the anterior capsule occurred, and the posterior chamber IOL was placed in the sulcus. In both eyes, severe lens epithelial proliferation with secondary IOL decentration developed postoperatively. Several surgical revisio

Ultrasonographic detection of very thin percutaneous central venous catheter in neonates.

To assess the ability of ultrasonography to detect the tip of a very thin (0.4 mm outer diameter) percutaneous central venous catheter (PCVC) in neonates, the PCVC tip location was assessed by ultrasonography (US) and compared to the location estimated by standard radiography for 57 PCVCs in 44 neonates. Of 57 occasions, the examiner could not find the PCVC tip in three cases (5%). In the remaining 54 instances, in 87% of cases, the PCVC tip position was consistent with the location implied by skeletal landmarks on standard radiographs. On 24 occasions we also assessed catheter tip dislodgemen

Evaluation of efficacy and complications: primary pediatric orbital implants after enucleation.

BACKGROUND: Orbital implants are used routinely in pediatric patients at the time of enucleation. Complications, such as exposure, ptosis, and infection, may occur after implantation. Controversy continues regarding the rate of complications with newer implants in the pediatric population. OBJECTIVE: To examine the effects of orbital implants on children whose eyes have been enucleated. METHODS: Records of orbital implantation after enucleation performed by 5 surgeons on 120 pediatric patients (123 eyes) over a 10.5-year period were reviewed retrospectively. Demographic data, ocular diagnosi

Retained surgical sponge: an unusual cause of malabsorption.

Retained surgical sponge is an unpleasant surprise in clinical practice. Intraluminal migration of the retained sponge, though rare, can lead to intestinal obstruction and other complications. We describe two cases of retained surgical sponge, both following gynaecological surgery, presenting several years after surgery with features of subacute intestinal obstruction, malabsorption and several years after surgery with features of subacute intestinal obstruction, malabsorption and sever hypoproteinemia which reverted after surgical removal.

Posterior extradural lumbar disk fragment.

We present the unique case of a patient with a sequestered disk fragment posterior to the thecal sac producing symptoms of spinal stenosis with neurogenic claudication. The majority of sequestered disk fragments migrate in either a cranial or caudal direction. In only a few cases have disk fragments been identified posterior to the thecal sac. Our patient had a sudden onset of bilateral groin and anterior thigh pain. Magnetic resonance imaging showed relatively severe stenosis at L4-5 with mild disk bulging. Intraoperatively, a large posteriorly placed encapsulated mass of soft tissue was foun

Tarsal patch-flap for orbital implant exposure.

The authors determine the efficacy of a new technique, the tarsal patch-flap, in the management of postenucleation and postevisceration porous orbital implant exposures that are recalcitrant to other surgical approaches. All patients treated during a 30-month period with recurrent orbital implant exposures who failed at least one surgical attempt at defect closure were treated using a tarsal patch-flap, a fornix-based tarsoconjunctival flap from the upper eyelid. Eight patients, six after enucleation and two after evisceration, were treated with a tarsal patch-flap. Five implants were porous p

Modified cups.

Late results of structural allografting of the acetabulum are quite variable. In order to reduce or avoid the necessity for bone grafting, some modified cups have been developed. The deep cup with or without a lateralized polyethylene liner reduces the problem of the protrusio socket. Oblong or bilobed ingrowth cups significantly reduce the need for grafting of the roof-deficient acetabulum. Constrained or capture polyethylene liners are useful in the unstable hip.

A severe complication of the new self-expanding spiral nitinol biliary stent.

The new spiral nitinol biliary stent offers some very promising features, including its long patency and retrievability. We report here the case of a patient in whom this type of stent was inserted for palliative treatment of a periampullary tumour. The stent migrated distally, and could not be withdrawn. The further course was complicated by duodenal obstruction, repeated bouts of cholangitis, and fatal hemorrhage after surgical removal of the endoprosthesis. The design of this new stent should be improved.

Late neurological complications of Harrington-rod instrumentation.

From our patients who had idiopathic scoliosis, we identified a subset of eighteen in whom Harrington rods were used for fixation down to the fifth lumbar vertebra. In five of these patients, low-back pain, sciatica, and other neurological problems developed at two to thirty-two months after arthrodesis. These complications were caused by migration of the caudad hook into the spinal canal. The migration was probably caused by a combination of lumbosacral lordosis and mobility of the fifth lumbar vertebra (the most caudad mobile segment) on the segment below, resulting in weakening of the lamin

Intraoperative Guidewire Breakage During Proximal Femoral Nailing Managed Through Direct Anterior Approach: A Case Report and Review of Literature.

CASE: We report a rare intraoperative complication during proximal femoral nailing for an intertrochanteric femur fracture in a 54-year-old male. During helical blade preparation, the guidewire migrated intra-articularly and fractured during withdrawal. Retention risked cartilage damage, while retrieval through the blade tract was unsuitable and threatened implant stability. With the nail in situ, safe surgical dislocation was not feasible. The fragment was successfully removed through an direct anterior approach. At 14 months follow-up the patient had returned to his pre injury activity level

PICC migration and perforation of internal jugular vein after neck dissection.

Peripherally inserted central catheters (PICCs) were introduced as an alternative to centrally inserted central catheters (CICCs) inserted into the jugular vein to provide central venous access. In head and neck reconstructive surgery, PICCs are preferred over CICCs because they do not obstruct surgical access, such as during neck dissection. Additionally, PICCs offer a safer option than peripheral access for the infusion of vesicants, irritants, hyperosmolar solutions, antibiotics, prolonged parenteral nutrition, and chemotherapy agents. We present a case of perforation of the internal jugula

Autopsy findings of inferior vena cava filter perforation: a forensic analysis of retroperitoneal hemorrhage and medical liability.

The inferior vena cava filter (IVCF) reduces the risk of fatal pulmonary embolism but carries risks of severe complications. A 63-year-old hypertensive female with lower-extremity deep vein thrombosis underwent IVCF placement and anticoagulation. On postoperative day 9, the patient developed sudden nausea and abdominal distension, which progressed rapidly to hemorrhagic shock and death. This led to lawsuits for medical malpractice. Autopsy revealed IVCF migration and perforation, which caused inferior vena cava (IVC) perforation and retroperitoneal hematoma. Hemorrhagic shock was confirmed as

Obstructive biliopathy with acute cholangitis secondary to post-cholecystectomy clip migration: a rare complication managed with ERCP.

Post-cholecystectomy clip migration (PCCM) is a rare but recognised complication that can occur early or years after surgery. Given its potential to cause biliary obstruction, PCCM should be considered in the differential diagnosis of post-cholecystectomy patients presenting with obstructive biliopathy or cholangitis. Imaging, particularly contrast-enhanced CT (CECT) or magnetic resonance cholangiopancreatography, is crucial for diagnosis, while endoscopic retrograde cholangiopancreatography (ERCP) is the gold-standard modality for diagnosis and treatment. We report a case of a male patient in

A mediation analysis of laboratory indicators affecting seed migration after 125I brachytherapy in hepatic malignancies.

BACKGROUND: 125I seed migration is a persistent safety concern in hepatic brachytherapy. Inflammatory and metabolic biomarkers may influence both tissue vulnerability and implantation strategies, contributing to migration risk. METHODS: In this retrospective study of 625 patients undergoing 125I seed implantation for primary or metastatic liver tumors, clinical, laboratory, and procedural data were analyzed. Migration was assessed via follow-up chest radiography or CT. Univariate and multivariate Cox regression identified predictors of migration. Kaplan-Meier analysis evaluated seed migration

Ultrasound-guided tip location and detection of tip migration of neonatal peripherally inserted central catheters: a prospective, multicenter study (DISLOTIP study).

BACKGROUND: Incorrect neonatal peripherally inserted central catheter (n-PICC) tip positioning and secondary migration lead to serious complications. Identifying an accurate and non-invasive tip location method is crucial. This study primarily aimed to evaluate the feasibility of ultrasound-guided n-PICC tip location in a multicenter setting. Secondary aims were to determine the incidence and timing of tip migration and to identify factors associated with migration. METHODS: Multicenter, prospective, cross-sectional study conducted at 10 Neonatal Intensive Care Units from January 2024 to Janua

Ectopic origin of bronchial arteries: still a potential pitfall in embolization.

PURPOSE: To evaluate the influence of ectopic origin of bronchial arteries (BAs) on bronchial artery embolization (BAE) for hemoptysis. METHODS: CT and angiography images of 50 consecutive sessions in 39 patients (aged 26-93 years; mean, 70.6 years) who underwent BAE for hemoptysis from April 2010 to December 2019 were reviewed. We defined ectopic BA as a systemic artery originating from other than the T5-T6 vertebral level of the descending aorta with course along the major bronchi. The background of patients, number of BAs, culprit arteries, and treatment outcomes were compared between the

Migration and sac expansion as modes of midterm therapeutic failure after endovascular aneurysm sealing.

BACKGROUND: Endovascular aneurysm sealing (EVAS) is a disruptive technology to treat abdominal aortic aneurysm (AAA). The use of sac filling rather than endograft fixation was designed to treat aortic aneurysms in a wide range of morphologic appearances and to reduce endoleaks. There are few data reporting outcomes beyond postoperative follow-up. This study reports outcomes up to 5 years for Nellix (Endologix, Irvine, Calif) EVAS. METHODS: Data were prospectively collected for EVAS patients from the time of adoption of EVAS in 2013. All patients treated with the Nellix device are included in
