[Preliminary results with Cuénod's osteoligamentoplasty and capsulodesis for treatment of chronic scapholunate dissociation].

Until now, the ideal treatment of scapholunate dissociation has not yet been found. Cuénod described an operation with reconstruction of the dorsal part of the scapholunate ligament using a bone-ligament-bone autograft (dorsal trapezoidometacarpal II ligament) and an additional dorsal capsulodesis using the dorsal intercarpal ligament. From December 2000 until January 2002, we operated on twelve patients with symptomatic chronic scapholunate dissociation without signs of osteoarthrosis using Cuénod's procedure. Preoperatively as well as at follow-up patients were evaluated using a traditional

Mid-Term Results of the Use of a Suture Anchor Fixation for Chronic Reducible Scapholunate Dissociation.

Background: Scapholunate dissociation (SLD) is a common and sometimes disabling ligamentous injury of the wrist. The aim of the treatment is to restore carpal alignment and to prevent joint degeneration. In the current study, we reported mid-term results of our technique using suture anchor fixation within the SL interface. Methods: Nine male patients with an average age of 35 years underwent scapholunate suture anchor fixation (SLAF) for chronic and symptomatic dynamic or reducible static SLD from 2011 to 2016 with a mean follow-up of 36 months. After dorsal wrist exposure, a 2.8 mm suture an

Chronic scapholunate instability treated with temporary screw fixation.

PURPOSE: To determine the efficacy of scapholunate (SL) temporary screw fixation to maintain the reduction of SL interval after ligament debridement, repair, or reconstruction. METHODS: A total of 36 patients who had chronic SL instability were included. Arthroscopic debridement was performed for dynamic instability, and primary repair or reconstruction of the SL ligament was performed for static instability. We then used SL screw fixation for the protection of the repair or reconstruction. Screws were removed when loosening was noted on x-ray. RESULTS: The average patient age was 43 years.

Optimization of volar percutaneous screw fixation for scaphoid waist fractures using traction, positioning, imaging, and an angiocatheter guide.

Percutaneous screw fixation of nondisplaced or reducible scaphoid fractures has become more popular as techniques and implants have improved. Many authors have advocated for the dorsal approach, citing difficulties with adequate screw placement from the volar approach. We have developed a straightforward and reproducible technique for volar percutaneous scaphoid screw fixation that mitigates most of the drawbacks of the approach. The wrist is held in extension and ulnar deviation with traction through the thumb. A 14-gauge angiocatheter needle is then used to localize the starting point and as

Is Scaphoid Reconstruction Still an Option for Scaphoid Nonunion With Established Arthritis? A Retrospective Study of a Group of Patients.

PURPOSE: Although many studies have supported salvage procedures as the best option for scaphoid nonunions with established arthritis, there remains uncertainty regarding when procedures to gain union are favored over salvage procedures. The purpose of this study was to evaluate whether the short-term results from scaphoid nonunion with established arthritis treated by scaphoid reconstruction are acceptable. METHODS: We retrospectively studied the clinical outcomes of scaphoid nonunions with established arthritis treated by scaphoid reconstruction between January 2019 and December 2021. Eight

An endpoint adjudication committee for the assessment of computed tomography scans in fracture healing.

INTRODUCTION: Endpoint Adjudication Committees (EACs) benefit the quality of randomized control trials (RCTs) where outcomes depend on subjective interpretations. However, assembling a committee to adjudicate large datasets is cumbersome. In a recent RCT, the primary outcome was time to union following operative fixation of scaphoid non-union, with real or placebo adjunctive ultrasound treatment. Union status was determined with computed tomography (CT) scans interpreted by treating surgeons and radiologists. An EAC was established to deliberate discrepancies between radiologists' and surgeons

Reconstruction of chronic scapholunate dissociation with the modified scapholunate axis method (SLAM).

INTRODUCTION: Injury to the scapholunate (SL) ligament represents a common trauma of the wrist but is frequently misdiagnosed because of non-specific pain. Established methods for SL reconstruction mainly focus on reducing pain and maintaining the reposition result at the expense of mobility and strength. This study aimed at restoring stability and reducing pain while simultaneously maintaining mobility and strength using the scapholunate axis method (SLAM). MATERIAL AND METHODS: 22 patients (19 male and 3 female) aged between 26 and 64 years with an SL ligament lesion underwent SLAM reconstr

A modified Blatt dorsal capsulodesis in the treatment of dynamic scapholunate instability.

We report a case of scapholunate dissociation which was initially missed and presented late. A modification of Blatt dorsal capsulodesis performed using dorsal intercarpal ligament (DICL) and extra tunnel appears not only to add to dorsal stability but also address the volar problem as well. This modification may be a better alternative to the current technique of using a single flap.

[Primary arthroscopic treatment of TFCC tears in fractures of the distal radius].

The primary simultaneous diagnosis and therapy of TFCC tears in distal radius fractures is still the exception. We present our results of arthroscopic treatment of these injuries. From January 1998 until September 1999, we treated 21 patients with a type C fracture (AO classification) of the distal radius and one patient with a scaphoid fracture and TFCC tears. The Palmer 1A (n = 5) and 1 C (n = 4) tears have been arthroscopically shaved as well as the meniscal tear (n = 1). The Palmer 1B tears were refixed either in an outside-inside technique (n = 6) or in a new all-inside technique (n = 6).

Electrophysiologic findings in two patients with digital neuropathy of the thumb.

Repetitive trauma to a finger can result in compressive neuropathy of one or more digital nerves. The thumb is commonly affected in bowlers, hence the term "bowler's thumb". However, this condition can involve other fingers and non-bowlers. To our knowledge, none of the cases reported had undergone neurophysiologic testing. We describe the neurophysiologic findings in two patients who had a history consistent with digital neuropathy of the thumb.

Two-Column Intercarpal Arthrodesis Using Headless Compression Screws.

INTRODUCTION: Two-column midcarpal arthrodesis (2CMA) after scaphoidectomy is a well-established procedure for treating advanced wrist arthritis, particularly scapholunate advanced collapse and scaphoid nonunion advanced collapse grade III wrists. This study evaluates the clinical and radiographic outcomes of 2CMA using headless compression screws (HCS) over a 21-year period. MATERIALS AND METHODS: From 2001 to 2022, 128 patients underwent 2CMA at two institutions. Preoperative arthroscopy was performed to confirm the integrity of the radiolunate joint. Outcomes were assessed using the Modifi

Fracture Gap Reduction With Variable-Pitch Headless Screws.

PURPOSE: Fully threaded, variable-pitch, headless screws are used in many settings in surgery and have been extensively studied in this context, especially in regard to scaphoid fractures. However, it is not well understood how screw parameters such as diameter, length, and pitch variation, as well as technique parameters such as depth of drilling, affect gap closure. METHODS: Acutrak 2 fully threaded variable-pitch headless screws of various diameters (Standard, Mini, and Micro) and lengths (16-28 mm) were inserted into polyurethane blocks of "normal" and "osteoporotic" bone model densities

Satisfactory function 12 years after triscaphoid arthrodesis for chronic scapholunate ligament injury.

Scapholunate ligament (SLL) injury represents a hazard to the wrist and the treatment of these injuries has been the focus of much debate. We performed a long-term follow (>10 years) of triscaphoid arthrodesis for symptomatic chronic SLL injuries in 10 (8 men) patients 37 (22-49) years of age. All arthrodesis healed. One patient was converted to total wrist arthrodesis during the follow-up period, and three had minor additional surgeries. At final follow-up, the patients had reduced active range of motion (AROM, 55%) and grip strength (85%) compared to the opposite side, but reported low Quick

Wrist motion analysis in scaphoid nonunion.

INTRODUCTION: The motion of human wrist is a complex and multidirectional process. The aim of this study was to develop a reliable and practicable method to measure motion impairment of the wrist in patients who incurred a scaphoid nonunion. A scaphoid nonunion in computed tomography as well as the consent in this study was required. METHODS: A total of nine patients with unilateral scaphoid nonunion accomplished maximal circumferential wrist movements. The wrist movements were measured with an electrogoniometer (Biometrics Ltd.). To quantify maximal wrist motion we constructed the maximal bo

Acute nondisplaced fractures of the scaphoid.

BACKGROUND: The optimal management for undisplaced scaphoid fractures has been the focus of much debate. Investigators have advocated internal fixation of these fractures; however, whether surgery improves clinically relevant patient outcomes compared with casting remains unknown. OBJECTIVE: To determine the effect of internal fixation versus casting for undisplaced scaphoid fractures on the rates of union, return to work, grip strength, and range of motion.
