Stability of somatization disorder and somatization symptoms among primary care patients.

BACKGROUND: Diagnostic criteria for somatization disorder emphasize its early onset and long-term stability. Research assessments of somatization disorder depend on lifetime recall of medically unexplained somatic symptoms. METHODS: Longitudinal data from the World Health Organization Psychological Problems in General Health Care study were used to examine stability of somatization disorder and somatization symptoms over 12 months. At 15 study sites in 14 countries, consecutive primary care patients (N = 25916) were screened using the 12-item General Health Questionnaire. A stratified random

Psychosocial factors associated with risky sexual behavior among HIV-seropositive gay men.

The present study examines sociodemographic characteristics, levels of psychological distress, and coping styles among HIV-seropositive (HIV+) gay men who either did or did not engage in sexual activity which placed others at risk for HIV infection. Risky sexual behavior was defined as engaging in insertive anal intercourse. Respondents were 156 HIV+ men enrolled at the Pittsburgh site of the Multicenter AIDS Cohort Study in 1989-90. HIV+ men who engaged in risky sexual behavior, while similar to remaining HIV+ men on most sociodemographic characteristics, showed lower levels of psychological

Three distinct patterns of mental health response following accidents in mountain sports: a follow-up study of individuals treated at a tertiary trauma center.

The restorative effect of physical activity in alpine environments on mental and physical health is well recognized. However, a risk of accidents and post-accident mental health problems is inherent to every sport. We aimed to characterize mental health in individuals following mountain sport accidents requiring professional medical management. Adult victims of mountain sport accidents treated at the hospital of the Medical University of Innsbruck (Austria) between 2018 and 2020 completed a cross-sectional survey at least 6 months following the admission (median 44 months, n = 307). Symptoms o

Functional limitations in functional somatic syndromes and well-defined medical diseases. Results from the general population cohort LifeLines.

OBJECTIVE: Functional somatic syndromes (FSS), defined as physical syndromes without known underlying organic pathology, are sometimes regarded as less serious conditions than well-defined medical diseases (MD). The aims of this study were to evaluate functional limitations in FSS, and to compare the results to MD patients with the same core symptoms. METHODS: This study was performed in 89,585 participants (age: 44.4±12.4 years, 58.5% female) of the general-population cohort LifeLines. Quality of Life (QoL) and work participation were examined as indicators of functional limitations. QoL was

Psychological and behavioral variables associated with the somatic symptom severity of general hospital outpatients in China.

OBJECTIVE: In high-income countries, the number and severity of somatic symptoms - irrespective of etiology--are associated with adverse psychobehavioral and functional characteristics. This study aimed to assess these key features among Chinese general hospital outpatients with high levels of somatic symptoms. METHODS: This multicenter, cross-sectional study evaluated four outpatient departments of internal medicine and Traditional Chinese Medicine in Beijing and Kunming and enrolled a total of 281 consecutive patients. The patients answered questionnaires concerning somatic symptom severity

Gender-related differences in symptoms of patients with suspected breathing disorders in sleep: a clinical population study using the sleep disorders questionnaire.

BACKGROUND: Gender-related differences in the symptom profile of patients with suspected sleep disordered breathing (SDB) may be one explanation of the clinical underrecognition of SDB in women. STUDY OBJECTIVES: The aim of this study was to prospectively assess gender-related differences in presenting symptoms in a clinical sample of patients with suspected sleep disordered breathing. DESIGN: Administration of the Sleep Disorders Questionnaire prior to clinical and polysomnographic evaluation. Responses obtained from the questionnaire were used to construct 4 independent symptom scales: sle

Psychiatric consequences of stress after a vehicle accident.

BACKGROUND: Vehicle accidents are a common cause of disease and death among people over 30 years of age. Essentially, reaction to stress due to the vehicle accident does not differ from the reaction to other stress factors. There are still no uniform viewpoints about the kind of sequels and their percentage representation after vehicle accidents. SUBJECTS AND METHODS: The research was provided as a prospective study, included 150 subjects who had vehicle accident minimum 2 years prior to the examination. A questionnaire adjusted to the needs of the research and a battery of psychological test

Affective dysregulation and sex differences in somatic symptom disorder: A 6-month longitudinal and mediation analysis.

OBJECTIVES: Somatic symptom disorder (SSD) is characterized by persistent physical symptoms and excessive emotional responses, but the influence of emotional factors such as anger and alexithymia on symptom severity and course remains unclear. This study examined whether baseline levels and six-month changes in emotional factors are associated with somatic symptom severity and course in SSD, considering sex. METHODS: Eighty-three SSD patients and 80 age- and sex-matched healthy controls completed self-report measures of emotional and somatic symptoms at baseline and six-month follow-up. Chang

The prevalence and its correlates of somatization disorder at a quaternary mental health centre.

INTRODUCTION: Somatization Disorder (SD) is known to cause disability and impact the quality of life due to psychological and somatic distress. Though it is a Common Mental Disorder (CMD), the disorder tends to visit all levels of health care, among which prevalence of SD at tertiary care is 10.1%. AIMS: To estimate current prevalence and its correlates of socio-clinical factors in SD at a quaternary mental health centre (QMHC). METHODS: Total of 422 adult subjects were selected through systematic random sampling at first contact psychiatry outpatient clinic. Subjects were interviewed with M

Evidence of reduced complexity in self-report data from patients with medically unexplained symptoms.

Physical symptoms which cannot be adequately explained by organic disease are a common problem in all fields of medicine. Reduced complexity, shown using nonlinear dynamic analysis, has been found to be associated with a wide range of illnesses. These methods have been applied to short time series of mood but not to self-rated physical symptoms. We tested the hypothesis that self-reported medically unexplained physical symptoms display reduced complexity by measuring the approximate entropy of self-reported emotions and physical symptoms collected twice daily over 12 weeks and comparing the re

Does two-year stability for scale scores of psychosocial factors differ by socioeconomic position?

Previous longitudinal studies have demonstrated the importance of measuring stability of risk factors over time to correct for attenuation bias. The present aim was to assess the stability of scores for eight psychometric scales over a 2-yr. period and whether stability differed by socioeconomic position. Baseline data were collected during 2003-2004 from 1,007 men and women ages 45 to 69 years. Follow-up data were collected in 2006 from a total of 795 men and women. Analysis showed that stability over 2 yr. was moderate and tended to be lower in groups of low socioeconomic position. It is sug

Do fluctuations in ovarian hormones affect gastrointestinal symptoms in women with irritable bowel syndrome?

BACKGROUND: An increase in gastrointestinal (GI) symptoms, including bowel discomfort, abdominal pain/discomfort, bloating, and alterations in bowel patterns, has been reported during premenses and menses menstrual cycle phases and the perimenopause period in women with and without irritable bowel syndrome (IBS). OBJECTIVE: This article reviews the literature related to one possible physiological mechanism-declining or low ovarian hormone levels--that may underlie the occurrence or exacerbations of abdominal pain/discomfort at times of low ovarian hormones (menses, menopause) in women with or

The triad of multiple metacarpal fractures and/or dislocations of the fingers, severe hand swelling and clinical evidence of acute median nerve dysfunction.

Twenty-five cases of crush injury to the hand resulting in the triad of multiple finger metacarpal fractures/dislocations of the fingers, a balloon hand appearance due to severe swelling and clinical evidence of acute median nerve dysfunction were retrospectively reviewed. All were men with a mean age of 33 (range 20-45) years. All injuries occurred following industrial or major car accidents. The fracture site included metacarpal head/neck fractures, metacarpal shaft fractures and carpometacarpal joint fracture-dislocations. All patients underwent fasciotomies, open carpal tunnel release and

[Rehabilitation, social medical evaluation and counselling in HIV-infected patients].

An inpatient rehabilitation programme for HIV-infected patients is described. Since the early nineties some 1,200 patients with HIV infection have been treated in our internal and orthopaedic rehabilitation clinic. Beside internal and orthopaedic diagnostics, supporting compliance with antiretroviral therapy, motivating the patients for regular moderate exercises and specific nutritional counselling are major issues of the programme. From the psychological point of view, the patients are offered to aquire relaxation and stress coping techniques, to take part in non-smoking courses and to use i

Physical "phantasies" and family functions: overcoming the mind/body dualism in somatization.

In this article, we examine some of the ways in which family therapists have conceptualized the experience of illness of unexplained physical origin. We argue that opinions about the etiology of somatic symptoms should not be the primary focus of therapeutic work with people who share the prototypical characteristics of what has been defined as "somatization disorder." We suggest that current research in neurobiology can expand the linguistic resources of clinicians and help them avoid perpetuating unhelpful dichotomies between the mind and the body.

An analysis of panel data. The impact of a psychiatric consultation letter on the expenditures and outcomes of care for patients with somatization disorder.

In this study, the cost and health outcomes of a psychiatric consultation letter to primary care physicians caring for a sample of patients diagnosed with somatization disorder, a psychiatric condition associated with multiple, unexplained medical complaints, was assessed. To accommodate the small sample size of 73 patients, outcome effects were calculated using panel analysis. Study patients were randomized to a consultation or noconsultation group, and were repeatedly assessed at equal time intervals. Data were analyzed using parsimonious regression models derived from economic theory. Durin

Somatization in the community.

We examined the prevalence of somatization disorder symptoms elicited with the Diagnostic Interview Schedule in 3132 community respondents interviewed in Los Angeles by the Epidemiologic Catchment Area program. The variables age, gender, ethnic background, and the presence of a psychiatric diagnosis significantly influenced the number of somatization symptoms reported. An introductory review on conceptual and nosological aspects of somatization phenomena led to the formulation of a less-restrictive operational definition of the somatizer. We found that 4.4% of the respondents met criteria for
