Eighty-seven cases of a nail gun injury to the extremity.

OBJECTIVES: The aims of this research were to characterise the injury patterns associated with nail guns, highlight their potential complications and review treatment options. METHODS: A retrospective case series was conducted of all patients with a nail gun injury to the upper or lower limb who presented to the Princess Alexandra Hospital from 1 January 2007 to 30 July 2012. RESULTS: Young men in the work environment were at most risk of sustaining a nail gun injury to their non-dominant hand. Out of 87 cases identified, 29% were intra-articular, 16% had evidence of neurovascular injury, 24

Free anterolateral thigh flap for reconstruction of car tire injuries of children's feet.

BACKGROUND: Grade IV and V car tire injuries occurring in children cause extensive soft tissue defects with exposure or loss of tendons and bone on the dorsum of the foot. Free tissue transfer is indicated for reconstruction of these defects because of the limited local tissue available. We describe our management of high-grade car tire foot injuries in children with free anterolateral thigh flap (ALT). MATERIALS AND METHODS: Five pre-school children with car tire injuries (one grade IV and four grade V) were treated with free ALT flap in the last 4 years. The mean age was 4.8 years. In four

An Unusual Accessory Muscular Belly of the Flexor Carpi Radialis Muscle.

Variations of the upper limb musculature are of clinical interest, potentially impacting diagnostic interpretation and surgical outcomes. Th … ellies may be misidentified as soft tissue tumors on imaging or complicate surgical procedures like tendon transfers and volar approaches to the radius. Furthermore, their proximity to the superficial branch of the radial nerve creates potential sites for nerve compression and entrapment syndromes.

Strategies to Maximize the Success of the Distally Based Peroneus Brevis Muscle Flap: Lessons Learnt from the Experience in a Single Health care Center.

Lower limb reconstruction, particularly in the distal third, is challenging due to limited local flap options, poor skin laxity, and the direct exposure of critical structures such as bone and tendons due to the thin skin in the region. For larger defects, a microvascular free flap transfer is often required. For small to medium defects, the distally based peroneus brevis muscle flap is a viable option, though it may risk distal flap necrosis over the critical defect area. This study presents the authors' experience using this flap in 9 patients at a Singapore health care center. Key considera

Management of Brachial Plexus Birth Injuries at British Columbia Children's Hospital.

A multidisciplinary team at BC Children's Hospital provides brachial plexus birth injury management for our provincial catchment area. Altho … tients have better shoulder outcomes, require less brachial plexus surgery, and need fewer shoulder tendon transfers than those patients treated in our clinic prior to 2008. We have also found that the rate and degree of elbow flexion contractures are reduced compared with results reported in the literature.

Burn trauma from a space heater requiring paraspinal muscle flap back reconstruction with skin grafting: A case report.

Full-thickness burns damage all layers of skin and may also damage underlying tissue including bones, muscles, and tendons. Full-thickness burns almost always require immediate medical and surgical management. Some may require extensive bone, muscular, and other reconstructive surgery depending on the depth of involvement of surrounding tissues. Bone exposure in burn patients can lead to unique complications including osteomyelitis. We present the case of an elderly patient with a history of dementia who presented with full-thickness burns to the back with exposed spinal elements who later dev

Tenomodulin expression in the periodontal ligament enhances cellular adhesion.

Tenomodulin (Tnmd) is a type II transmembrane protein characteristically expressed in dense connective tissues such as tendons and ligaments. Its expression in the periodontal ligament (PDL) has also been demonstrated, though the timing and function remain unclear. We investigated the expression of Tnmd during murine tooth eruption and explored its biological functions in vitro. Tnmd expression was related to the time of eruption when occlusal force was transferred to the teeth and surrounding tissues. Tnmd overexpression enhanced cell adhesion in NIH3T3 and human PDL cells. In addition, Tnmd-

Metatarsophalangeal joint instability of the lesser toes.

Metatarsophalangeal joint instability of the lesser toes is a common finding and a common cause of metatarsalgia. The clinical presentation can include swelling without digital deformity; however, often, this can progress to the development of coronal and transverse plane malalignment. In some cases, frank metatarsophalangeal joint dislocation can develop. The treatment regimen has historically focused on indirect surgical realignment using soft tissue release, soft tissue reefing, tendon transfers, and periarticular osteotomies. An improved understanding of the plantar plate has recently led …

A case of acute motor and sensory axonal neuropathy following hepatitis a infection.

Acute motor and sensory axonal neuropathy (AMSAN) are recently described subtypes of Guillain-Barre syndrome characterized by acute onset of distal weakness, loss of deep tendon reflexes, and sensory symptoms. A 21-yr-old male was transferred to our hospital due to respiration difficulties and progressive weakness. In laboratory findings, immunoglobulin M antibodies against hepatitis A were detected in blood and cerebrospinal fluid. The findings of motor nerve conduction studies showed markedly reduced amplitudes of compound muscle action potentials in bilateral peroneal, and posterior tibial

[Obstetric brachial plexus palsy--clinics, pathophysiology and surgical treatment].

Obstetric brachial plexus palsy is a rare but sometimes severe traction injury. Peripheral nerve microsurgery (neurolysis, interfascicular grafting) and secondary procedures including muscle and tendon transfers altogether with contracture releases have improved the prognosis over the last 20 years. This article includes a historical review, the ongoing discussion about the pathophysiology (frequent traction injury vs. rare intrauterine maladaptation) and the clinics. Based on a patient group of 500 children with 100 microsurgical plexus reconstructions, we describe the surgical technique and …

Mangling upper limb injuries in industry.

The salvage of upper limbs mangled by industrial machinery became possible with the development of predictable techniques of vascular and microvascular anastamosis. Unfortunately, many of these salvaged limbs are associated with fair and poor functional outcomes. The quality of the skeletal fixation can have a substantial effect on the functional outcome and should be a major focus of the limb repair process. Internal plate fixation facilitates wound care and limb mobilization without tethering muscle-tendon units and is safe in the majority of severe upper limb injuries provided that all devi

Superior detachment of a glenoid labrum variant resembling an incomplete discoid meniscus in a wheelchair ambulator.

Symptoms of instability and painful clunking in the shoulder were found associated with superior detachment of an incomplete discoid glenoid labrum and biceps origin in a 25-year-old woman. The patient was paraplegic below the T4 level secondary to resection of a spinal cord astrocytoma at age of 17. Symptoms began after 3 years of wheelchair use and repetitive transfer injuries. On arthroscopic examination, only the central surface of the articular glenoid cartilage was visible because of labral coverage. The entire labrum was meniscoid without distinguishable variation. The superior labrum w

Pes cavovarus. Review of a surgical approach using selective soft-tissue procedures.

This study reviews the surgical treatment of twenty-seven patients (thirty-nine feet) with cavovarus foot deformity. The patients' ages ranged from six months to fifteen years. Only those patients who had at least a two-year follow-up were selected for statistical analysis. This group comprised twenty patients (twenty-seven feet). A standardized treatment program was used, based on the flexibility of the deformity. A radical plantar or plantar medial release was employed as indicated, in combination with tendon transfers or osteotomy of the fore part of the foot, or both. All patients were ava…

The role of flaps in preventing lower extremity amputations.

The increasing prevalence of chronic limb-threatening ischemia and diabetes mellitus has led to a surge in lower extremity amputations, driven by the combination of peripheral arterial disease and extensive wounds. Although revascularization often addresses ischemia, severe wounds pose a significant risk of amputation. Flaps, which involve the transfer of vascularized tissue, can provide immediate closure of complex wounds, particularly those involving bone or tendon exposure, where skin grafts are insufficient. Flap reconstruction of wounds can prevent amputation in cases when revascularizati
