Vestibular recovery and central compensation after acute unilateral vestibulopathy - changes in saccadic response patterns and gains over time.

INTRODUCTION: Acute unilateral vestibulopathy (AUVP), or vestibular neuritis, disrupts the vestibulo-ocular reflex (VOR), leading to impaired gaze stability during head movements. Vestibular function typically recovers to some extent. However, if recovery is incomplete, corrective eye saccades must compensate for the deficient VOR function. The aim was to analyze the progression of VOR gain recovery and development of different corrective eye saccade patterns after AUVP. METHODS: A prospective longitudinal study was performed, including 43 patients with AUVP, that were examined using the vide

Respiratory vulnerability to vehicle buffeting.

OBJECTIVE: Buffeting in a jerky ride in a bus or ambulance normally provokes a sustained tachypnoea driven by vibration and sensory mechanisms including vestibular signals. Tachypnoea reinforces the torso against mechanical shocks but results in overbreathing, causing a mild fall in CO(2). However, normal CO(2) is rapidly restored by a reduction in depth of breathing. We test the hypothesis that vulnerable subjects, exemplified by elderly individuals and patients with vestibular disorders, may fail to adapt to buffeting. METHODS: Respiratory and cardiovascular functions were recorded from fiv

A case of patulous Eustachian tube associated with dizziness induced by nasal respiration.

OBJECTIVE: To determine the relationship between Eustachian tube function and inner ear function in patulous Eustachian tube (pET). METHODS: We encountered a patient with pET accompanied by dizziness that was induced by nasal respiration. Eye movements were recorded using video-oculography, and Eustachian tube function was assessed using a Eustachian tube function analyzer. Horizontal and vertical components of pupil position were assessed to test fixation, positional, and positioning nystagmus. Impedance testing with a Eustachian tube function analyzer was performed to confirm tympanometry r

Evaluation of the Peripheral Vestibular System due to Inner Ear Barotrauma in the Hyperbaric Environment.

OBJECTIVES: The aim of this study was to investigate changes that may occur in the peripheral vestibular system due to inner ear barotrauma in self-contained underwater breathing apparatus (SCUBA) divers exposed to hyperbaric environments. METHODS: This prospective study was conducted on 45 healthy volunteers. In this study, after otologic and audiological examinations were performed on two groups as control (n = 24) and SCUBA (n = 21), cervical vestibular evoked myogenic potentials (c-VEMP) were evaluated for peripheral vestibular system functions. The SCUBA group comprised individuals who h

Influence of BPPV and Meniere's Disease on Cognitive Abilities: A Questionnaire-Based Study.

The vestibular system connects the inner ear to the midbrain and subcortical structures and can affect cognition. Patients with vertigo often experience cognitive symptoms such as attention deficits, memory problems, and spatial perception difficulties. This study aimed to explore the cognitive impairments associated with Benign paroxysmal positional vertigo (BPPV) and Meniere's Disease (MD). A non-experimental group comparison design was used with 107 participants divided into three groups: Group I (clinically normal), Group II (BPPV), and Group III (MD). Participants completed a questionnair

A prospective study on the application of HINTS in distinguishing the localization of acute vestibular syndrome.

BACKGROUND: Acute vestibular syndrome (AVS) is a common clinical syndrome in neurology clinics and emergency department. Canonical standard for AVS diagnosis requires the presence of persistent vertigo for more than 24 h. HINTS (head impulse-nystagmus-test of skew) is an emerging scheme in the diagnosis of AVS. In this prospective study, we evaluated the specificity and sensitivity of HINTS in distinguishing between central and peripheral AVS. METHODS: A cohort of 239 cases with complete clinical record was recruited in the study. All patients completed emergency brain CT examination to exclu

Salient and placebo vibrotactile feedback are equally effective in reducing sway in bilateral vestibular loss patients.

This study explores the effect of vibrotactile biofeedback on body sway in stance in patients with severe bilateral vestibular losses in a placebo-controlled study. A tilt sensor mounted on the head or trunk is used to detect head or body tilt and activates via a microprocessor 12 small vibrators that are placed around the waist with a mutual distance of 30 degrees. Two positions of the tilt sensor (head and trunk) and three types of biofeedback (normal, full and random) were evaluated, besides no biofeedback. Body sway during stance was assessed in 10 patients with bilateral vestibular arefle

Cochlear implantation in the neurofibromatosis type 2 patient: long-term follow-up.

OBJECTIVE: To evaluate the long-term hearing outcomes of neurofibromatosis type 2 (NF2) patients with cochlear implants. METHODS: Retrospective analysis of cochlear implant performance in NF2 patients using open- and closed-set speech perception testing. RESULTS: Patients with NF2-associated bilateral vestibular schwannomas frequently become profoundly deaf. The aim of surgical resection should be to preserve serviceable hearing in at least one ear; however, this goal can be difficult to achieve. Frequently, tumor size or poor preoperative hearing status can require a surgical approach that

Reliability and minimal clinically important differences of gait characteristics in peripheral vestibular disorders.

OBJECTIVE: Sensory gait ataxia is a core feature of chronic peripheral vestibular disorders, leading to impaired mobility, reduced quality of life, and increased fall risk. Quantitative gait metrics capturing this deficit are valuable endpoints for clinical trials, yet data on their reliability and clinically meaningful change remain limited. METHODS: Gait was recorded in 60 patients with chronic bilateral (n = 30) or unilateral (n = 30) vestibular failure on an instrumented walkway during self-selected, slow, and eyes-closed walking. Test-retest reliability was quantified using intraclass co

Impact of the intronic RFC1 expansion size in CANVAS phenotype: an oculomotor study.

The identification of the RFC1 homozygous intronic expansion in cerebellar ataxia neuropathy and vestibular areflexia syndrome (CANVAS) highlighted that genetically determined CANVAS patients exhibit a wide range of clinical presentations and natural course. Previous studies suggested a link between disease severity and the size of the intronic expansion. The aim of our study was to obtain quantitative data related to vestibular and cerebellar impairments using oculomotor recordings to provide further evidence of a link between RFC1 intronic expansion size and the phenotype. This study recruit

Earth-vertical motion perception assessment using an elevator: a feasibility study.

A feasible, inexpensive, rapid, and easy-to-use method to measure vestibular vertical movement perception is needed to assess the sacculus-mediated low-frequency otolith function of dizzy patients. To evaluate the feasibility of reaction time assessment in response to vertical motion induced by an elevator in healthy young individuals. We recorded linear acceleration/deceleration reaction times (LA-RT/LD-RT) of 20 healthy (13 female) subjects (mean age: 22 years ± 1 SD) as a measure of vertical vestibular motion perception. LA-RT/LD-RT were defined as the time elapsed from the start of elevato

Hearing Rehabilitation in Neurofibromatosis Type 2.

Bilateral vestibular schwannomas are almost pathognomonic of neurofibromatosis type 2 (NF2). As a result of these tumors, hearing loss is the presenting symptom in 60% of adults and 30% of children with NF2. It is often bilateral. The best means of preserving hearing in patients with NF2 is conservative management. Even so at least 28% of patients have progression of hearing loss following diagnosis. The likelihood of progression of hearing loss is, at least in part, determined by the type of mutation. Treatment of vestibular schwannomas often has a detrimental effect on hearing. Only 41% of p

Resting discharge patterns of macular primary afferents in otoconia-deficient mice.

Vestibular primary afferents in the normal mammal are spontaneously active. The consensus hypothesis states that such discharge patterns are independent of stimulation and depend instead on excitation by vestibular hair cells due to background release of synaptic neurotransmitter. In the case of otoconial sensory receptors, it is difficult to test the independence of resting discharge from natural tonic stimulation by gravity. We examined this question by studying discharge patterns of single vestibular primary afferent neurons in the absence of gravity stimulation using two mutant strains of

Surgical treatments for a case of superior canal dehiscence syndrome associated with patulous Eustachian tube.

OBJECTIVES: The patulous Eustachian tube (PET) and superior semicircular canal dehiscence syndrome (SCDS) have similarity in their symptoms and similar effects caused by positional changes, causing difficulty in the differentiation between the two disorders. This report describes a case of both SCDS and PET that was eventually successfully treated. METHODS: A 68-year-old man presented with hyperacusis to his own footsteps and gait disturbance. He had been diagnosed as PET two years before and had been treated by insertion of a silicone plug (Kobayashi plug) at the other hospital. Clinical cas

Saccular Impairment in Alzheimer's Disease Is Associated with Driving Difficulty.

BACKGROUND/AIMS: Patients with Alzheimer's disease (AD) experience increased rates of vestibular loss. Recent studies suggest that saccular impairment in mild cognitive impairment (MCI) and AD patients is associated with impaired spatial cognitive function. However, the impact of saccular impairment on everyday behaviors that rely on spatial cognitive function is unknown. METHODS: We recruited 60 patients (21 MCI and 39 AD) from an interdisciplinary Memory Clinic. Saccular function was measured, and a visuospatial questionnaire was administered to assess whether participants experienced impai
