Eye screening in the elderly.

Poor vision is considered to be a common unreported illness in the elderly population. To determine its prevalence we carried out an eye screen on 150 randomly selected co-operative elderly subjects attending St James's Hospital. A database [mean age 78.2 years (range 65 to 97)], medical history, mini-mental test score [mean score 9.1 (range 2 to 10)], mobility assessment and history of eye problems was gathered. A visual test of both eyes together (standard N6 and N8), inspection of the eyes, tonometry (Perkin's handheld tonometer) and fundoscopy were performed on each subject and the need an

Colposcopy, cervicography, speculoscopy and endoscopy. International Academy of Cytology Task Force summary. Diagnostic Cytology Towards the 21st Century: An International Expert Conference and Tutorial.

ISSUES: The colposcope was developed in 1925 and is well established in clinical gynecologic practice for defining and delineating cytologically detected lesions mainly of the cervix but also the vagina and vulva. Additionally, various endoscopic procedures in gastroenterology, pulmonary and urologic lesions enhance the cytologic detection and histologic verification of precancerous and cancerous lesions. The cost-effectiveness of all these devices and their applicability, particularly in countries with a limited health budget, is a major issue. This task force considered aspects of the presen

Efficiency of a video-autorefractometer used as a screening device for amblyogenic factors.

BACKGROUND: High ametropia is known as a risk factor in developing amblyopia. The aim of the study was to evaluate the efficiency of a commercially available infrared videorefractometer (PowerRefractor) in examining children, and to evaluate its effectiveness in detecting amblyogenic factors of the refraction. The influence of cycloplegia was of interest. METHODS: A total of 255 eyes (-28.25 D to +7.88 D spherical equivalent) of 128 patients (1-81 years) were examined by cycloplegic retinoscopy. All of them were measured with the PowerRefractor under cycloplegia. Fifty-four of these patients

Analyzing Satellite Imagery to Target Tuberculosis Control Interventions in Densely Urbanized Areas of Kigali, Rwanda: Cross-Sectional Pilot Study.

BACKGROUND: Early diagnosis and treatment initiation for tuberculosis (TB) not only improve individual patient outcomes but also reduce circulation within communities. Active case-finding (ACF), a cornerstone of TB control programs, aims to achieve this by targeting symptom screening and laboratory testing for individuals at high risk of infection. However, its efficiency is dependent on the ability to accurately identify such high-risk individuals and communities. The socioeconomic determinants of TB include difficulties in accessing health care and high within-household contact rates. These

Exploring geographic variation of and influencing factors for utilization of four diabetes management measures in Swiss population using claims data.

INTRODUCTION: Four strongly recommended diabetes management measures are biannual glycated hemoglobin (HbA1c) testing, annual eye examination, kidney function examination, and low-density lipoprotein (LDL) testing in patients below 75 years. We aimed to describe regional variation in the utilization of the four measures across small regions in Switzerland and to explore potential influencing factors. RESEARCH DESIGN AND METHODS: We conducted a cross-sectional study of adult patients with drug-treated diabetes in 2014 using claims data. Four binary outcomes represented adherence to the recomme

Multi-Comparison of Different Ocular Imaging Modality-based Deep Learning Models for Visually Significant Cataract Detection.

PURPOSE: Age-related cataract is the leading cause of vision impairment. Researchers have utilized various imaging modalities, including slit beam, diffuse anterior segment, and retinal imaging, to develop deep learning (DL) algorithms for automated cataract analysis. However, the comparative performance of these algorithms across different ocular imaging modalities remains unevaluated, mainly due to the absence of standardized test sets across studies. DESIGN: Retrospective study. PARTICIPANTS: Across all the models, the Singapore Malay Eye Study data set was used for training (N = 7093 eye

Can symptom-severity phenotypes identify depression risk after mild traumatic brain injury? A cluster-based approach.

BACKGROUND: Mild traumatic brain injury (mTBI) affects millions worldwide and frequently leads to secondary depression. Early identification of high-risk individuals is critical for targeted mental-health screening in this population. Data-driven phenotyping offers a promising avenue to unmask hidden symptom patterns, but few studies have combined unsupervised clustering of post-concussion profiles with established clinical and psychosocial metrics. We aimed to classify post-concussion symptom-severity profiles in adults with mTBI and to evaluate their association with secondary depression ris

Deep learning based diagnostic quality assessment of choroidal OCT features with expert-evaluated explainability.

Various vision-threatening eye diseases including age-related macular degeneration (AMD) and central serous chorioretinopathy (CSCR) are caused due to the dysfunctions manifested in the highly vascular choroid layer of the posterior segment of the eye. In the current clinical practice, screening choroidal structural changes is widely based on optical coherence tomography (OCT) images. Accordingly, to assist clinicians, several automated choroidal biomarker detection methods using OCT images are developed. However, the performance of these algorithms is largely constrained by the quality of the

Recessive variants in mitochondrial complex I nuclear subunits are an underrated cause of optic atrophy.

Our understanding of the genetic landscape of inherited optic neuropathies (ION) has grown significantly over the past decades, and it is now known to involve many genes found in both the nuclear and mitochondrial genomes, exhibiting all possible inheritance patterns. Furthermore, pathogenic variants in nuclear genes encoding mitochondrial respiratory complex I (CI) subunits have been identified in some cases of ION, in addition to the more common severe presentation of CI deficiencies, which usually have an early onset. We conducted next-generation sequencing screening of CI genes to identify

Predicting alcohol use disorder risk in firefighters using a multimodal deep learning model: a cross-sectional study.

INTRODUCTION: Firefighters constitute a high-risk occupational cohort for alcohol use disorder (AUD) due to chronic trauma exposure, yet traditional screening methodologies relying on self-report instruments remain compromised by systematic underreporting attributable to occupational stigma and career preservation concerns. This cross-sectional investigation developed and validated a multimodal deep learning framework integrating T1-weighted structural magnetic resonance imaging with standardized neuropsychological assessments to enable objective AUD risk stratification without necessitating c

Sensitivity and specificity of the Bamberg Dementia Screening Test's (BDST) full and short versions: brief screening instruments for geriatric patients that are suitable for infectious environments.

BACKGROUND: Currently, many patients suffering from dementia do not have a diagnosis when admitted to geriatric hospitals. This is the case despite an increased risk of complications affecting the length of stay and outcome. Unfortunately, many dementia screening tests cannot be used on geriatric inpatients, who are often bedridden. Therefore, we aimed at evaluating the diagnostic accuracy of a small battery of bedside tasks that require minimal vision and fine motor skills in patients with suspected dementia. METHODS: In this prospective study, the Bamberg Dementia Screening Test (BDST) was

Health risk appraisal in older people 1: are older people living alone an "at-risk" group?

BACKGROUND: In the UK, population screening for unmet need has failed to improve the health of older people. Attention is turning to interventions targeted at 'at-risk' groups. Living alone in later life is seen as a potential health risk, and older people living alone are thought to be an at-risk group worthy of further intervention. AIM: To explore the clinical significance of living alone and the epidemiology of lone status as an at-risk category, by investigating associations between lone status and health behaviours, health status, and service use, in non-disabled older people.
