Sacral neuromodulation (SNM) offers a therapeutic approach to urological patients suffering from idiopathic overactive bladder (OAB) syndrome, with or without incontinence and non-obstructive urinary retention (NOR), who are not responding to or are not compliant with conservative or medical therapies. The exact mechanism of action of SNM is not fully understood but modulation of the spinal cord reflexes and brain networks by peripheral afferents is regarded as the main pathway. Over the years, surgical techniques improved, leading to the development of the modern two-stage implantation techni

Advances in sequencing technologies have allowed for a more complete analysis of the microbiota composition and identification of differences among individuals and/or physiologies. Changes in microbiota composition and associated inflammation have been linked to both metabolic and behavioral disorders, and abnormality in microbiota composition, or dysbiosis, may play a causal role in the etiology and maintenance of these pathologies. There is accumulating evidence showing that the gut microbiota can communicate to the central nervous system to alter host behavior. Supplementation with L. rhamn

Sensory organ damage induces a host of cellular and physiological changes in the periphery and the brain. Here, we show that some aspects of auditory processing recover after profound cochlear denervation due to a progressive, compensatory plasticity at higher stages of the central auditory pathway. Lesioning >95% of cochlear nerve afferent synapses, while sparing hair cells, in adult mice virtually eliminated the auditory brainstem response and acoustic startle reflex, yet tone detection behavior was nearly normal. As sound-evoked responses from the auditory nerve grew progressively weaker fo

CONCLUSIONS: This study suggests that the asynchronization of the cochlear afferent pathway may be one mechanism of the etiopathogenesis of auditory neuropathy.
OBJECTIVE: To investigate the characteristics of the SP-AP compound wave and its generating mechanism in patients with auditory neuropathy.
SUBJECTS AND METHODS: The electrocochleogram (ECochG) was recorded with external ear canal electrode in patients with auditory neuropathy and normal subjects. The peak-latency, amplitude, and time course of the SP-AP compound wave were measured by using an 'assessor-blind' study. The discrepancy

It is widely believed that postprandial somnolence is caused by redistribution of blood flow from cerebral to mesenteric vessels after a meal. This belief persists despite its apparent contradiction with a well-known neurophysiologic principle that cerebral perfusion is preferentially maintained under a wide range of physiologic states. For instance, during exercise when a large amount of perfusion is diverted to muscles, blood flow to the brain is maintained. Furthermore, recent evidence suggests that there is no measurable change of blood flow in the common carotid artery during postprandial

The purpose of the present study was to investigate the pathophysiological role of reciprocal facilitation between antagonistic motoneuron pools in spasticity. The soleus H-reflex was conditioned by prior stimulation of the peroneal nerve in 15 healthy subjects, six hemiplegic patients and 11 spinal cord injured (SCI) patients. The hemiplegic patients were tested from soon after the onset of hemiplegia and up to 2 years later. Whereas stimulation of the peroneal nerve produced short-latency inhibition of the soleus H-reflex in healthy subjects, it produced facilitation in spastic SCI and hemip

Two experiments are reported examining the effects of electroconvulsive shock (ECS) on the electroretinogram (ERG) and the retinal oscillatory potentials (OPs) in the albino rat. Immediately after the induction of generalised seizure activity, both the ERG and the OPs were always preserved basically intact, despite minor alterations to their waveforms. In Experiment 1, it was found that small changes in amplitude of the ERG were recorded following ECS, but these were most likely artifactual. The only other notable finding was a temporary decrease in latency of the b-wave of the ERG. In Experim

BACKGROUND: The innervation of skeletal tissues by sensory nerves is poorly understood - especially of nerve fibres which reach into the bony and cartilaginous tissue.
METHODS: Samples of rat cartilaginous tissues from different locations (knee joint, vertebral column, temporomandibular joint) were fixed by perfusion and decalcified. The distribution of protein gene product (PGP) 9.5-, calcitonin gene-related peptide (CGRP)- and tachykinin (TK)-immunoreactive axons was analysed using fluorescence immunohistochemistry.
RESULTS: Nerve fibres were detected in the outer regions of the hyaline ca

In man there is now strong evidence for the existence of a system of cervical propriospinal premotoneurons interposed in the corticospinal pathway to upper limb motoneurons. The relevant neurons are located rostral to upper limb motoneurons. They receive both weak excitation and strong inhibition from low threshold afferents in the upper limb. Ongoing EMG activity recorded in wrist and elbow extensors during voluntary movements of different types (tonic contraction, reaching) is suppressed by a cutaneous volley evoked by stimulation of the superficial radial nerve. There is evidence that this

The effect of capsaicin pretreatment on frequency of sneezing, decrease of nasal patency, and increase of vascular dye leakage induced by antigen or histamine challenge on the guinea pig nasal mucosa was investigated. The animals were sensitized intraperitoneally with ovalbumin. Capsaicin pretreatment significantly inhibited sneezing induced by nasal challenge with histamine and antigen, indicating that capsaicin-sensitive sensory nerves constitute an afferent pathway of the sneezing reflex in nasal allergy. Although capsaicin pretreatment tended to inhibit the decrease of nasal patency and th

Oesophageal wall distension was performed on standing conscious sheep by inflating a rubber balloon at four different levels inside the cervical and thoracic oesophagus. Although no retrograde oesophageal peristalsis was ever recorded, these stimulations were always followed by a conspicuous increase of parotid saliva secretion. They frequently induced the onset of chewing movements, whatever the level of the oesophageal distension. The cyclical reticular activity does not bear any relationship to the occurrence of the chewing movements. The masticatory response could be either of the "ON" or

TNF-α-converting enzyme, a member of the ADAM (A disintegrin and metalloproteinase) protease family and also known as ADAM17, regulates inflammation and regeneration in health and disease. ADAM17 targets are involved in pain development and hypersensitivity in animal models of inflammatory and neuropathic pain. However, the role of ADAM17 in the pain pathway is largely unknown. Therefore, we used the hypomorphic ADAM17 (ADAM17ex/ex) mouse model to investigate the importance of ADAM17 in nociceptive behavior, morphology, and function of primary afferent nociceptors. ADAM17ex/ex mice were hypose

The opioid analgesics, commonly exemplified by morphine, represent the best option for the treatment of severe pain and for the management of chronic pain states, of both malignant and nonmalignant origin. It is well recognized that the prolonged use of opioids is associated with a requirement for ever-increasing doses in order to maintain pain relief at an acceptable and consistent level. This phenomenon is termed analgesic tolerance. While the concept that tolerance can develop as a result of cellular adaptations to the presence of the opioid has been proposed, it is now becoming abundantly

Basic mechanisms and pathways of itch signaling are reviewed, with an emphasis on the progress to date as well as remaining challenges in translating current knowledge to the clinical treatment of chronic itch. Recent studies reveal 3 subsets of pruriceptive sensory neurons highly expressing itch-related genes. Their fibers project into the spinal cord to activate neurons expressing gastrin releasing peptide (GRP) and its receptor (GRPR), which connect to neurons that express the substance P (NK-1) receptor and project to the parabrachial nucleus and thalamus. Spinal inhibitory interneurons re

Deep brain stimulation (DBS) in the ventral intermediate nucleus of thalamus (Vim) is known to exert a therapeutic effect on postural and kinetic tremor in patients with essential tremor (ET). For DBS leads implanted near the caudal border of Vim, however, there is an increased likelihood that one will also induce paresthesia side-effects by stimulating neurons within the sensory pathway of the ventral caudal (Vc) nucleus of thalamus. The aim of this computational study was to (1) investigate the neuronal pathways modulated by therapeutic, sub-therapeutic and paresthesia-inducing DBS settings

Sleep is a fundamental physiological process closely linked to autonomic homeostasis. Rather than being governed by a localized neural switch, sleep-wake regulation depends on coordination between central circuits and peripheral physiology. The gastrointestinal tract is a major interface where this integration is evident. Ascending visceral signals can influence brain circuits involved in arousal and sleep, while sleep disruption can feed back onto the gut through descending autonomic pathways, altering motility, secretion, immune function, barrier integrity, and the intestinal environment. BACKGROUND: The medullary nucleus of solitary tract (NTS) and its afferents of vagus nerve have long been investigated in regulation of cortical activity and sleep promotion. However, the underlying neural circuit by which the NTS regulates electroencephalogram (EEG) and sleep remain unclear. As the NTS has a strong projection to the pontine arousal site, the parabrachial nucleus (PB), we proposed the NTS via the pontine parabrachial nucleus (PB) regulates cortical activity and sleep.
METHODS: We bilaterally and directly stimulated the NTS neurons by chemogenetic approach and NTS terminals in

Visceral hypersensitivity is an important mechanism underlying increased abdominal pain perception in functional gastrointestinal disorders including functional dyspepsia, irritable bowel syndrome, and inflammatory bowel disease in remission. Although the exact pathophysiological mechanisms are poorly understood, recent studies described upregulation and altered functions of nociceptors and their signaling pathways in aberrant visceral nociception, in particular the transient receptor potential (TRP) channel family. A variety of TRP channels are present in the gastrointestinal tract (TRPV1, TR

Carotid bodies (CBs) sense arterial blood O2 levels. Hypoxemia activates the carotid sinus nerve (CSN) afferent activity to elicit cardiorespiratory reflexes to maintain tissue oxygenation. This brief review presents emerging evidence suggesting that CBs employ distinct O2-sensing mechanisms depending on the severity of hypoxia. During physiologically relevant moderate hypoxia (pO2 ~40 mmHg), glomus cells use heme oxygenase-2 (HO-2)/carbon monoxide (CO)/cystathionine-γ-lyase (CSE)/hydrogen sulfide (H2S) signaling pathway to stimulate CSN activity and breathing response to hypoxia.
