Panic disorder and cigarette smoking behavior.

Smoking has been discussed both as a risk factor for panic disorder and as a contributing factor to elevated cardiovascular risk in panic disorder patients. Smoking habits and their association with panic disorder were studied in a sample of 102 panic disorder patients. Both for female and for male patients, rates of smokers and of exsmokers were substantially higher than in the general population. However, a surprisingly high number of patients had succeeded in reducing or quitting cigarette smoking because of their panic disorder, although they experienced little benefit in regard to panic s

Plasma levels of tumor necrosis factor-alpha in patients with panic disorder: effect of alprazolam therapy.

Plasma concentrations of tumor necrosis factor-alpha (TNF-alpha) were measured in 10 outpatients with panic disorder, twice (at a 48-h interval) before and twice on days 30-32 of treatment with alprazolam (2-2.5 mg/day), and twice in 10 age- and sex-matched healthy controls. TNF-alpha concentrations did not differ in patients and control subjects, either before therapy or at days 30-32 of therapy. In five patients before therapy, and in three of them after therapy, TNF-alpha values were higher than the maximal concentrations of the cytokine in controls.

Panic disorder and vestibular disturbance: an overview of empirical findings and clinical implications.

The purpose of this article is to provide a general overview of the growing literature on the relationship between panic disorder and vestibular disturbance. This relationship has been examined from two distinct perspectives, including: (a) the assessment of vestibular dysfunction in patients with panic disorder; and (b) the evaluation of panic symptomatology in patients with vestibular disturbance. Consequently, this review focuses primarily on the literature pertaining to vestibular symptoms in patients with panic disorder and panic symptomatology in patients with vestibular complaints. Subs

Benzodiazepine treatment of panic and agoraphobic symptoms: use, dependence, toxicity, abuse.

Benzodiazepines are widely prescribed drugs. Most patients are prescribed benzodiazepines for short periods of time for anxiolytic or hypnotic purposes. However, some benzodiazepines are also used for extended periods at high doses to treat panic and agoraphobic disorders. When chronically prescribed benzodiazepines are discontinued, a predictable pattern of discontinuance symptoms may develop, indicating physiological dependence. Benzodiazepines also produce a variety of side effects including sedation, reduced coordination, and impaired cognition, which are primarily related to dose and dura

Use of mobile phones in the behavioral treatment of driving phobias.

Results of two case studies are presented to illustrate the use of mobile phones with in vivo exposure treatment of refractory driving phobias. Number of miles driven and subjective ratings of anxiety were recorded during a baseline phase and 8 weeks of treatment involving a total of 24 driving practices. One subject's use of a mobile phone increased the number of miles driven alone, but the second subject made little progress and regressed following removal of the phone. These two cases and our experience with other patients suggest that mobile phones can benefit many individuals whose therap

Heart rate variability in patients with major depression.

We have previously reported decreased heart rate variability upon standing in panic disorder patients compared with controls. In this study, we extend our report to include patients with major depression (n = 19). Compared to normal controls (n = 20) and panic disorder patients (n = 30), there was no significant difference in the immediate changes in heart rate upon standing in the depressed group. The standing heart rate variability (R-R variability) was significantly lower in panic disorder patients compared to both normal controls and depressed patients as indicated by the corrected standar

Exposure therapy for post-traumatic stress disorder. Four case studies.

Four cases of PTSD, and their treatment, are described. Symptoms responded differently to two behavioural approaches. In-vivo exposure was effective for phobic anxiety while imaginal exposure improved dysphoria and some phobic symptoms. Audio-taped imaginal exposure may be important as part of treatment.

A psychotherapist engages in behaviour therapy.

The article reviews clinical findings that led an experienced psychotherapist to favour behaviour therapy over psychotherapy for patients who have anxiety disorders involving avoidance of specific situations or avoidance of specific activities. It is maintained that anxiety disorders are best treated through psychotherapy when they are demonstrably connected to disturbances in interpersonal relations. Three basic procedures in behaviour therapy are described: desensitization, direct exposure, and cognitive restructuring. Transference patterns in each of these procedures are delineated. A fourt

Denial of separation anxiety as measured by a serial tachistoscopic technique.

A mother-child separation stimulus was repeatedly presented by standard Defense Mechanism Test procedure, at increasing durations of tachistoscopic exposure to 60 clinical and nonclinical subjects. Subjective verbal reports were coded blindly for evidence of perceptual distortions presumably indicative of denial of separation anxiety. Low scores on two items measuring sensitivity to separation anxiety and intolerance of aloneness were, as predicted, significantly more often characterized by codings of denial on the tachistoscopic exposures than subjects with high sensitivity to separation anxi

Perceived parental rearing styles of agoraphobic and socially phobic in-patients.

The perceived parental rearing practices and attitudes of agoraphobics, social phobics and non-patient normal controls were investigated, employing the EMBU, an inventory for assessing memories of upbringing. Findings obtained previously with out-patients were replicated with in-patients as subjects. Compared with the controls, agoraphobics rated both their parents as having been less emotionally warm but only their mothers as having been rejective. Socially phobic in-patients rated both their parents as having been rejective, as having lacked emotional warmth, and as having been over-protecti

Synchrony and desynchrony in high and low arousal subjects undergoing therapeutic exposure.

A comparison of synchrony and desynchrony was made among phobic patients exposed to imaginal flooding under conditions of high and low arousal, respectively. Subjective anxiety (SUDs) and skin conductance (SC) were recorded at five minute intervals during three separate sessions. Results showed some degree of synchrony (small positive relationship) between the two measures for the high arousal group and desynchrony (no relationship) between these measures for the low arousal group. However, with the exception of two cases, one from each group, the findings were by no means clear-cut, suggestin

Test-retest reliability of a standardized psychiatric interview (DIS/CIDI).

The reliability of DSM-III diagnoses using an expanded version of the Diagnostic Interview Schedule (DIS), called the Composite International Diagnostic Interview (CIDI), was evaluated by examining 60 psychiatric inpatients on a test-retest basis. Acceptable agreement coefficients of (kappa) 0.5 or above were found for all but two disorders: dysthymic disorder and generalized anxiety disorder. The subclassification of DSM-III affective disorders also revealed some discrepancies between the test and the retest interviews. When compared with results from earlier versions of the DIS, diagnostic r

Schedule for Affective Disorders and Schizophrenia--Lifetime Version modified for the study of anxiety disorders (SADS-LA): rationale and conceptual development.

The SADS-LA, a modification of the Lifetime Version of the Schedule for Affective Disorders and Schizophrenia, was designed specifically for studies requiring detailed lifetime information on anxiety disorders, symptoms and traits. This article focuses on current difficulties in assessing and conceptualizing anxiety disorders, as addressed in the SADS-LA. The following topics are discussed: conceptual differentiation of certain anxiety disorders; sub-threshold symptoms and syndromes; the relationship between affective and anxiety syndromes; the residual category, Generalized Anxiety Disorder.

[New proposals for the development of the concepts of neurotic anxiety, major depression and panic attack].

An accumulation of research results, laboratory and clinic, leads to revise the classification of disorders of anxious nature (anxiety disorders according to the DSM-III). An empirical confirmation allows to separate anxious states from panic attack and phobic behavior constituting a component separated from generalized anxiety, exaggerated emotional reactions to stress agents and obsessive-compulsive disorders. The response to pharmacotherapeutic agents and the important number of anxiety depression and mixed depression tends to make permeable the separating line between the nosological entit

Prognostic factors with four treatment methods for phobic disorders.

The associations between prognostic factors and outcome with four treatment methods for phobic disorders were studied in 103 women. The prognostic factors were sociodemographic, intelligence, personality, early interpersonal relations, aspects of previous and current mental disorder, and intensity of symptoms at start of treatment. All patients received basal therapy, which implied information, advice on exposure in vivo, and anxiolytics. One group received this treatment only (B), while others received supplementary treatment, with therapist-directed prolonged exposure in vivo (PE), supportiv
