The scholarly and practice profile of respiratory therapists in Canada: A cross-sectional survey.

INTRODUCTION: Respiratory therapists (RTs) are expected to provide high-quality care for patients with chronic and acute cardiopulmonary conditions across the lifespan by staying abreast of emerging scientific evidence and effectively integrating it into clinical practice. This integration of evidence is encompassed within the competency of scholarly practice. However, there is currently a limited understanding of RTs' scholarly practice. Furthermore, despite RTs' widespread presence in the Canadian healthcare system, comprehensive studies describing the profiles of RTs are lacking. This study

The relationship between Chiari 1.5 malformation and sleep-related breathing disorders on polysomnography.

OBJECTIVE: Chiari 1.5 malformation is a subgroup of the Chiari malformation in which tonsillar descent into the foramen magnum is accompanied by brainstem descent. No data exist on whether operative decompression in patients with Chiari 1.5 improves sleep-related breathing disorders (SRBDs) and whether there are radiological parameters predicting improvement. METHODS: The authors performed a retrospective cohort study of consecutive pediatric patients with Chiari 1.5 malformation and SRBDs at the Alberta Children's Hospital. An SRBD was characterized using nocturnal polysomnography (PSG), spe

Hospital organizational context and delivery of evidence-based stroke care: a cross-sectional study.

BACKGROUND: Organizational context is one factor influencing the translation of evidence into practice, but data pertaining to patients with acute stroke are limited. We aimed to determine the associations of organizational context in relation to four important evidence-based stroke care processes. METHODS: This was a mixed methods cross-sectional study. Among 19 hospitals in Queensland, Australia, a survey was conducted of the perceptions of stroke clinicians about their work using the Alberta Context Tool (ACT), a validated measure covering 10 concepts of organizational context, and with ad

How much are we spending? The estimation of research expenditures on cardiovascular disease in Canada.

BACKGROUND: Cardiovascular disease (CVD) is a leading cause of death in Canada and is a priority area for medical research. The research funding landscape in Canada has changed quite a bit over the last few decades, as have funding levels. Our objective was to estimate the magnitude of expenditures on CVD research for the public and charitable (not-for profit) sectors in Canada between 1975 and 2005. METHODS: To estimate research expenditures for the public and charitable sectors, we compiled a complete list of granting agencies in Canada, contacted each agency and the Canadian Institutes of

Pediatric temporal lobectomy for epilepsy.

BACKGROUND: Temporal lobectomy in adults is an accepted form of treatment for patients with intractable complex partial seizures. There have been few long-term studies of children undergoing temporal lobectomy for epilepsy. METHODS: We reviewed the pediatric cases of temporal lobectomy for intractable epilepsy performed by the Comprehensive Epilepsy Program at the University of Alberta Hospitals between 1988 and 2000. All patients had preoperative and postoperative clinical evaluations, seizure charts, drug levels, EEG, CT/MRI, long-term video EEG monitoring and neuropsychological testing. Th

Clinicopathologic Characterization of Hidradenoma on Acral Sites: A Diagnostic Pitfall With Digital Papillary Adenocarcinoma.

Hidradenomas are benign sweat gland tumors that typically present as small nodules in adulthood. Their anatomic distribution is wide and rarely includes acral sites. In this setting, reliable separation from digital papillary adenocarcinoma is important, but notoriously difficult. Hematoxylin and eosin-stained sections of 25 hidradenomas on acral skin were retrieved. The clinical presenting features and morphologic findings were recorded, and follow-up was obtained. Immunohistochemistry was performed for AE1/3, CK5/6, EMA, CEA, SMA, S100, p40, and p63. The tumors presented as solitary nodules

Validation of a novel method for localization of parathyroid adenomas using SPECT/CT.

BACKGROUND: Accurate localization of parathyroid adenomas is of critical importance in surgical planning for minimally invasive parathyroidectomy. SPECT/CT is considered the investigation of choice but has limitations regarding localization of superior versus inferior adenomas. We proposed a novel method for localization using SPECT/CT by determining the anterior-posterior relationship of the adenoma to a horizontal line in the coronal plane through the tracheoesophageal groove. Our objective was to determine the accuracy, validity, and inter-rater reliability of this method. METHOD: This was

Patient Perception of Speech Outcomes: The Relationship Between Clinical Measures and Self-Perception of Speech Function Following Surgical Treatment for Oral Cancer.

PURPOSE: Treatment for oral cancer can result in speech impairments that can have varying impacts on patient quality of life. This study explored the relationship between clinical measures of speech impairment and the perception that patients had of this change in the early stage of recovery. METHOD: This was a quasi-experimental 1-group pre-post study design carried out on 10 patients with surgical intervention for oral cancer. Two clinical measures (word intelligibility and consonant phoneme error) and 2 patient-perception measures (Speech Handicap Index total score and Speech Handicap Inde

A Double-Blinded, Randomized Comparison of Medetomidine-Tiletamine-Zolazepam and Dexmedetomidine-Tiletamine-Zolazepam Anesthesia in Free-Ranging Brown Bears (Ursus Arctos).

We compared anesthetic features, blood parameters, and physiological responses to either medetomidine-tiletamine-zolazepam or dexmedetomidine-tiletamine-zolazepam using a double-blinded, randomized experimental design during 40 anesthetic events of free-ranging brown bears (Ursus arctos) either captured by helicopter in Sweden or by culvert trap in Canada. Induction was smooth and predictable with both anesthetic protocols. Induction time, the need for supplemental drugs to sustain anesthesia, and capture-related stress were analyzed using generalized linear models, but anesthetic protocol did

Long-term outcome of primary Papillary Urothelial Neoplasm of Low Malignant Potential (PUNLMP) including PUNLMP with inverted growth.

BACKGROUND: Few larger studies have evaluated the long-term outcome after a diagnosis of papillary urothelial neoplasm of low malignant potential (PUNLMP), demonstrating a broad range of recurrence and progression rates. Additionally, no study has addressed the outcome of PUNLMP exhibiting inverted growth. We evaluated the long term clinical outcome of primary papillary urothelial neoplasm of low malignant potential (PUNLMP), including PUNLMP with inverted growth in a large single center study. METHODS: We evaluated 189 primary PUNLMP (177 exophytic, 12 inverted), diagnosed from January 1, 20

Innovative low-risk maternity clinic. Family physicians provide care in Calgary.

PROBLEM BEING ADDRESSED: Because some family physicians choose to stop providing maternity care, many women and their physicians have to seek this care from other providers. These providers must respect family physicians' relationships with patients and the necessity of returning the family to the referring physician for primary care after appropriate maternity and newborn care has been provided. OBJECTIVE OF PROGRAM: To design a clinic to address this problem and to deal with the practical issues, such as preservation of lifestyle, adequate remuneration, and avoiding litigation, family physi

Predictors of functional outcome after endovascular thrombectomy in patients with large ischemic core based on DWI-ASPECTS.

BACKGROUND: Several clinical trials have shown the benefit of endovascular thrombectomy (EVT) in patients with large ischemic core infarction. However, the imaging selection modalities used for patient selection have differed across studies. This study aimed to assess the efficacy, safety, and prognostic factors of EVT in patients with large ischemic core selected only on the basis of Diffusion-Weighted Imaging Alberta Stroke Program Early CT Score (DWI-ASPECTS). METHOD: This single-center study, conducted from 2019 to 2024, included patients with anterior circulation acute large vessel occlu

Functional outcome, five-year survival and burden of disease after size- and location-matched hemorrhagic versus ischemic stroke.

BACKGROUND: Worse functional outcome has been reported after hemorrhagic compared to ischemic stroke, yet it remains unclear whether the lesion size, location, or stroke etiology itself results in different degrees of disability. This study compares outcomes in ischemic vs. hemorrhagic stroke patients with matched lesion size and location. METHODS: Data from the longitudinal cohort study on intracerebral hemorrhage (ICH) care (2006–2015) and a retrospective registry on acute ischemic stroke (AIS) (2011–2015), both conducted at the university hospital Erlangen, were used to investigate patients

Senescence Gene Expression Profiles and 90-Day Outcome in Stroke Patients.

BACKGROUND AND OBJECTIVES: Understanding the biological determinants of stroke outcome is essential to improve treatment strategies. Cellular senescence may contribute to vascular dysfunction with age and has been linked to poor outcomes across a range of diseases. However, the role of senescence on stroke outcome in patients is not well defined. We sought to investigate the relationship between enriched senescence-associated gene signature expression and functional outcome after stroke. METHODS: Acute ischemic stroke patients and control participants were recruited from University of Alberta
