Cognitive processing speed in multiple sclerosis clinical practice: association with patient-reported outcomes, employment and magnetic resonance imaging metrics.

BACKGROUND AND PURPOSE: To analyze the relationship between cognitive processing speed, patient-reported outcome measures (PROMs), employment and magnetic resonance imaging (MRI) metrics in a large multiple sclerosis cohort. METHODS: Cross-sectional clinical data, PROMs, employment and MRI studies within 90 days of completion of the Processing Speed Test (PST), a technology-enabled adaptation of the Symbol Digit Modalities Test, were collected. MRI was analyzed using semi-automated methods. Correlations of PST score with PROMs and MRI metrics were examined using Spearman's rho. Wilcoxon rank

Oxidative stress as a driver of organelle cascade damage in neurological diseases.

As a core driver in the pathological progression of neurological diseases, oxidative stress contributes to the onset and development of multiple disorders, including traumatic brain injury (TBI), Alzheimer's disease (AD), Parkinson's disease (PD), Huntington's disease (HD), and amyotrophic lateral sclerosis (ALS), by inducing interconnected and bidirectional damage among mitochondria, endoplasmic reticulum, lysosomes, and the nucleus. This review systematically summarizes the oxidative stress-mediated inter-organelle crosstalk network: Mitochondria act as one of the earliest and central hubs,

Effects of Motion-Style Scalp Acupuncture on External Capsule Microstructure and Spinal Reflex Excitability in a Rat Model of Post-Stroke Spasticity.

Post-stroke spasticity (PSS) severely impairs motor function and quality of life after stroke. Disruption of the cortico-reticulospinal pathway and impaired supraspinal descending inhibition are considered important pathological mechanisms; however, the neural and microstructural mechanisms underlying the therapeutic effects of motion-style scalp acupuncture (MSSA) remain incompletely understood. This study aimed to investigate the effects of MSSA on PSS using a multimodal assessment integrating neuroimaging, electrophysiology, and histology in a rat model of middle cerebral artery occlusion.

Microstructural white matter injury contributes to cognitive decline: Besides amyloid and tau.

BACKGROUND: Cognitive decline and the progression to Alzheimer's disease (AD) are traditionally associated with amyloid-beta (Aβ) and tau pathologies. This study aims to evaluate the relationships between microstructural white matter injury, cognitive decline and AD core biomarkers. METHODS: We conducted a longitudinal study of 566 participants using peak width of skeletonized mean diffusivity (PSMD) to quantify microstructural white matter injury. The associations of PSMD with changes in cognitive functions, AD pathologies (Aβ, tau, and neurodegeneration), and volumes of AD-signature regions

Pulmonary and systemic involvement in COVID-19 patients assessed with ultrasound-guided minimally invasive autopsy.

AIMS: Brazil ranks high in the number of coronavirus disease 19 (COVID-19) cases and the COVID-19 mortality rate. In this context, autopsies are important to confirm the disease, determine associated conditions, and study the pathophysiology of this novel disease. The aim of this study was to assess the systemic involvement of COVID-19. In order to follow biosafety recommendations, we used ultrasound-guided minimally invasive autopsy (MIA-US), and we present the results of 10 initial autopsies. METHODS AND RESULTS: We used MIA-US for tissue sampling of the lungs, liver, heart, kidneys, spleen

Periventricular low intensities on fluid attenuated inversion recovery imaging in the newborn infant: relationships to the clinical data and long-term outcome.

BACKGROUND: The author's previous study showed that it is possible to predict the severity of white matter injury in early infancy by using fluid attenuated inversion recovery (FLAIR) imaging. A follow-up study was performed in order to assess the correlation between the incidence of periventricular low intensities on FLAIR imaging (F-PVLI) and the clinical data including the physiological variables at birth and the long-term outcome of the infant. METHODS: The authors reviewed MRI of 328 newborn infants, which were obtained before 2 months corrected age. Abnormal findings in the periventricu

Language disintegration in dementia: effects of etiology and severity.

The speech characteristics on a standardized picture description task of 26 subjects with presumed senile dementia of the Alzheimer type (SDAT) and 13 subjects with stroke-related dementia (SRD) were compared to 15 normal subjects over age 59 years. Compared to the normal subjects, the dementia subjects used fewer total words, fewer unique words, fewer prepositional phrases, fewer subordinate clauses, and more incomplete sentence fragments. Lexical deficits tended to be more severe than syntactic ones, confirming prior suggestions that lexicon is more vulnerable to disruption in dementia than

Diffusion tensor imaging in extremely low birth weight infants managed with hypercapnic vs. normocapnic ventilation.

BACKGROUND: Permissive hypercapnia is a ventilatory strategy used to prevent lung injury in ventilated extremely low birth weight (ELBW, birth weight ≤1,000 g) infants. However, there is retrospective evidence showing that high CO2 is associated with brain injury. OBJECTIVE: The objective of this study was to compare brain white matter development at term-equivalent age in ELBW infants randomized to hypercapnic vs. normocapnic ventilation during the first week of life and in healthy non-ventilated term newborns. MATERIALS AND METHODS: Twenty-two ELBW infants from a randomized controlled tria

A male rat model integrating hypertension, chronic cerebral hypoperfusion, and aging recapitulates multimodal cerebral small vessel disease phenotypes.

BACKGROUND: Cerebral small vessel disease (CSVD) usually develops under the combined influence of vascular risk factors, chronic perfusion insufficiency, and age-related brain vulnerability; however, animal models driven by a single insult do not fully reproduce this multifactorial clinical context. METHODS: We established a composite CSVD rat model by combining spontaneous hypertension, bilateral common carotid artery stenosis (BCAS)-induced chronic cerebral hypoperfusion, and D-galactose (D-gal)-induced aging-related stress. Fourteen-week-old male Wistar-Kyoto (WKY) rats and spontaneously h

Vestibular agnosia in traumatic brain injury and its link to imbalance.

Vestibular dysfunction, causing dizziness and imbalance, is a common yet poorly understood feature in patients with TBI. Damage to the inner ear, nerve, brainstem, cerebellum and cerebral hemispheres may all affect vestibular functioning, hence, a multi-level assessment-from reflex to perception-is required. In a previous report, postural instability was the commonest neurological feature in ambulating acute patients with TBI. During ward assessment, we also frequently observe a loss of vertigo sensation in patients with acute TBI, common inner ear conditions and a related vigorous vestibular-

Traumatic Facial and Vestibulocochlear Nerve Injury in The Internal Acoustic Canal in The Absence of A Temporal Bone Fracture.

We present a rare case of traumatic facial and vestibulocochlear nerve injury in the internal acoustic canal in the absence of a temporal bone fracture. A 2.5-year-old female presented with sudden-onset left-sided facial paralysis and ipsilateral total hearing loss after being hit by a falling television. High-resolution computed tomography revealed an occipital fracture line that spared the temporal bone and otic capsule. Diagnostic auditory brainstem response testing showed that wave V at 90-db normal hearing level was absent in the left ear. Needle electromyography revealed severe axonal in

Association Between Intraindividual Variability in Cognitive Performance and White Matter Organisation in Chronic Mild Traumatic Brain Injury.

Mild traumatic brain injury (mTBI) can result in persistent cognitive deficits (particularly in attention, processing speed, and working memory), even years after the injury. The majority of behavioural studies have focussed on averaged cognitive performance scores, such as average reaction time or accuracy scores after mTBI. However, less is understood about how mTBI affects intraindividual variability (IIV) in cognitive performance across repeated sessions or measurement occasions over time. In this study, we investigate IIV in cognitive performance in chronic mTBI patients (n = 11) relative

Comparison of trophic factors' expression between paralyzed and recovering muscles after facial nerve injury. A quantitative analysis in time course.

After peripheral nerve injury, recovery of motor performance negatively correlates with the poly-innervation of neuromuscular junctions (NMJ) due to excessive sprouting of the terminal Schwann cells. Denervated muscles produce short-range diffusible sprouting stimuli, of which some are neurotrophic factors. Based on recent data that vibrissal whisking is restored perfectly during facial nerve regeneration in blind rats from the Sprague Dawley (SD)/RCS strain, we compared the expression of brain derived neurotrophic factor (BDNF), fibroblast growth factor-2 (FGF2), insulin growth factors 1 and

Selectively diminished corpus callosum fibers in congenital central hypoventilation syndrome.

Congenital central hypoventilation syndrome (CCHS), a condition associated with mutations in the PHOX2B gene, is characterized by loss of breathing drive during sleep, insensitivity to CO2 and O2, and multiple somatomotor, autonomic, neuropsychological, and ophthalmologic deficits, including impaired intrinsic and extrinsic eye muscle control. Brain structural studies show injury in peri-callosal regions and the corpus callosum (CC), which has the potential to affect functions disturbed in the syndrome; however, the extent of CC injury in CCHS is unclear. Diffusion tensor imaging (DTI)-based f

Postoperative rupture of an artery dissected from a cerebral aneurysm dome following clipping: A rare and fatal complication.

The adhesion of arteries to aneurysm domes can pose significant technical challenges during surgical clipping. Dissection of these vessels carries a risk of iatrogenic wall damage and subsequent complications. We present the case of a 67-year-old woman with three unruptured intracranial aneurysms. Following successful coil embolization of a right posterior communicating artery aneurysm, surgical clipping was planned for the remaining left middle cerebral artery and anterior choroidal artery aneurysms. Intraoperatively, the M2 inferior division was found to be densely adherent to an aneurysm lo

Inhaled gases as novel neuroprotective therapies in the postcardiac arrest period.

PURPOSE OF REVIEW: The purpose of this review is to summarize recent advances about inhaled gases as novel neuroprotective agents in the postcardiac arrest period. RECENT FINDINGS: Inhaled gases, as nitric oxide (NO) and molecular hydrogen (H2), and noble gases as xenon (Xe) and argon (Ar) have shown neuroprotective properties after resuscitation. In experimental settings, the protective effect of these gases has been demonstrated in both in-vitro studies and animal models of cardiac arrest. They attenuate neuronal degeneration and improve neurological function after resuscitation acting on d

Severe Decompression Illness: Case Report, Prehospital Recognition, and Regional Transport Considerations.

A 46-year-old male presented to our tertiary care emergency department (ED) with shortness of breath and chest pain following an uneventful four-hour SCUBA dive at 100 feet. His prehospital emergency medical services (EMS) assessment revealed transient hypotension and hypoxia. He later developed progressive skin mottling. Serology was significant for acute kidney injury, transaminitis, hemoconcentration, and hypoxia on an arterial blood gas. Computed tomography (CT) angiography demonstrated intravascular gas throughout the mesenteric and pulmonary arteries as well as the portal venous system.
