[Latest results of treatment with pacemaker and implantable cardioverter defibrillators].

Device-based anti-arrhythmic therapy is one of the most dynamically evolving branches of the medicine. Brady- and tachyarrhythmias can be treated efficiently and cost-effective with current pacemakers and ICDs. Beside conventional indications new indications have appeared in the last years: resynchronization treatment of severe congestive heart failure with biventricular pacing, primary prevention of sudden cardiac death with ICD, reduction of intraventricular gradient in hypertrophic obstructive cardiomyopathy. Several large clinical studies have investigated the efficacy of treatment in conv

Safety and efficiency of low-field magnetic resonance imaging in patients with cardiac rhythm management devices.

PURPOSE: Low-field magnetic resonance imaging (MRI), i.e. MRI with a static magnetic field strength <0.5 T, has been reported to be safe in patients with pacemakers, however there are no data about the safety of low-field MRI in patients with implantable cardioverter defibrillators (ICD) and/or cardiac resynchronization therapy (CRT). We aimed to investigate the safety and diagnostic efficiency of routine low-field MRI in patients with different devices for cardiac rhythm management (i.e. pacemakers and ICD, including devices with CRT). METHOD: MRI scans of 446 regions of interest were evalua…

Changes in causes of death and influence of therapeutic improvement over time in patients with heart failure and reduced ejection fraction.

INTRODUCTION AND OBJECTIVES: In patients with heart failure and reduced ejection fraction (HFrEF), several therapies have been proven to reduce mortality in clinical trials. However, there are few data on the effect of the use of evidence-based therapies on causes of death in clinical practice. METHODS: This study included 2351 outpatients with HFrEF (< 40%) from 2 multicenter prospective registries: MUSIC (n=641, period: 2003-2004) and REDINSCOR I (n=1710, period: 2007-2011). Variables were recorded at inclusion and all patients were followed-up for 4 years. Causes of death were validated by

His bundle pacing - a curative method: A Case Report.

INTRODUCTION: Pacing of the His bundle and conduction system seems an attractive site for pacing. Lead placement in His-pacing might be technically challenging due to surrounding structures and particular anatomic location. PATIENT CONCERNS: A 62-years old male patient was admitted for recurrent syncope. Electrocardiographic monitoring revealed periods of complete atrioventricular block with left branch block morphology and a QRS duration of 160 ms. DIAGNOSIS: A diagnosis of intermittent complete atrioventricular block was made with a Class I indication of permanent dual-chamber cardiac paci

Complex phenotype linked to a mutation in exon 11 of the lamin A/C gene: Hypertrophic cardiomyopathy, atrioventricular block, severe dyslipidemia and diabetes.

The lamin A/C (LMNA) gene encodes lamins A and C, which have an important role in nuclear cohesion and chromatin organization. Mutations in … ricular block with no apparent cause, and mildly depressed systolic function, prompted referral for cardiac resynchronization therapy. In conclusion, the association between LMNA mutations and different phenotypes is complex and not fully understood, and can present with a broad spectrum of severity.

Successful destination therapy after stepwise mechanical circulatory support in a patient with cardiogenic shock following mechanical aortic valve replacement.

Patients with mechanical aortic valve replacement and end-stage heart failure present major challenges when mechanical circulatory support is required, as transvalvular devices such as the Impella 5.5 are contraindicated. We report a 57-year-old man with idiopathic dilated cardiomyopathy who developed cardiogenic shock after prior mechanical aortic valve replacement. Despite cardiac resynchronization therapy defibrillator support and intra-aortic balloon pumping, he became dialysis dependent. The patient's condition progressively deteriorated, necessitating more robust mechanical circulatory s…

Pathophysiology, Diagnosis, and New Therapeutic Approaches for Ischemic Mitral Regurgitation.

Ischemic mitral regurgitation (MR) is a valvular complication frequently seen in patients with coronary artery disease and is associated with increased mortality and morbidity. Ischemic mitral regurgitation has a complex, heterogeneous, and still incompletely understood pathophysiology involving both the mitral valve and the left ventricle. The occurrence of valve regurgitation in patients with ischemic cardiomyopathy in return accelerates left ventricular remodelling and dysfunction, ultimately leading to irreversible heart failure. Diagnostic evaluation of ischemic MR is unique and different
