BACKGROUND: Pseudoaneurysm after mechanical thrombectomy (MT) is rare but is one of the potential complications associated with endovascular procedures. There is limited information regarding its mechanism of formation and the potential risk of delayed bleeding.
CASE DESCRIPTION: An 84-year-old woman was admitted to our institution with right hemiplegia and global aphasia. Magnetic resonance imaging and angiography revealed occlusion of the M2 segment of the left middle cerebral artery with subtle acute ischemic change in this territory. After initiating intravenous thrombolysis, MT was perfo

BACKGROUND: It remains controversial which bypass methods are optimal for treating adult moyamoya angiopathy patients. This study aimed to analyze the literature about whether different bypass methods affect differently outcome results of adult moyamoya patients with symptoms or hemodynamic instability.
METHODS: A systematic search of the PubMed, Embase, and Cochrane Central databases was performed for articles published between 1990 and 2015. Comparative studies about the effect of direct or combined bypass (direct bypass group) and indirect bypass (indirect bypass group) in patients with mo

OBJECTIVE Moyamoya disease (MMD) is a chronic cerebrovascular disease that can lead to devastating neurological outcomes. Surgical intervention is the definitive treatment, with direct, indirect, and combined revascularization procedures currently employed by surgeons. The optimal surgical approach, however, remains unclear. In this decision analysis, the authors compared the effectiveness of revascularization procedures in both adult and pediatric patients with MMD. METHODS A comprehensive literature search was performed for studies of MMD. Using complication and success rates from the litera

BACKGROUND: Moyamoya angiopathy most often manifests in patients in the second and third decades of life. Although uncommon, it can also manifest later in life. We present our results in patients >50 years old with moyamoya angiopathy who were treated with surgical revascularization via either direct bypass or indirect bypass (encephaloduroarteriosynangiosis).
METHODS: A retrospective review was conducted to identify patients with moyamoya disease who were treated with surgical revascularization at our institution between 2002 and 2015. Outcomes and complications were analyzed.
RESULTS: We i

BACKGROUND AND PURPOSE: In a swine stroke model we have previously demonstrated a high-intensity fluid attenuated inversion recovery (FLAIR) signal representing the erythrocyte component of the clot. We hypothesized that the intensity of the FLAIR clot signal in patients with acute stroke may predict the efficacy of recanalization by thrombectomy devices. In this study we compared the pretreatment FLAIR signal intensity of the clots in the distal internal carotid artery (ICA) and the degree of angiographic recanalization rate after mechanical thrombectomy.
METHODS: 25 consecutive acute stroke

Large and giant anterior communicating artery (ACoA) aneurysms usually show partial thrombosis and incorporate both the A(1) and A(2) segments and crucial perforating vessels. Therefore, direct clip placement or endovascular strategies often fail, leaving cerebral bypass surgery as a relevant therapeutic option. The authors present 3 cases in which a giant or large ACoA aneurysm was successfully occluded using a new technique that applies a double-barrel radial artery bypass. A radial artery graft is modified into a Y-shaped double-barrel conduit. After both pterional and parasagittal cranioto

Giant basilar tip aneurysms are a challenge to treat because of the complex anatomy and critical vessels that arise in this region. For large aneurysms, with multiple recurrences after coiling, when microsurgical clipping is not possible, a bypass to the posterior cerebral artery (PCA) can be helpful in providing definitive treatment. Two patients with giant basilar tip aneurysms were treated with bypass to the PCA. One patient had a microsurgical clipping and the other had a proximal occlusion of the basilar artery along with the bypass. Long-term angiographic and clinical follow-up was obtai

OBJECT: The radial artery is a common conduit used for high-flow bypasses. Until now the radial artery has been harvested using a long incision in the forearm that follows the course of the artery. The authors present an endoscopic technique that has been used during coronary bypass surgery but is not yet widespread in the neurosurgical arena.
METHODS: From October 2006 to October 2007, the authors used the radial artery as a graft in 6 patients during the treatment of complex cerebral aneurysms. The artery was harvested via an endoscopic technique. The radial artery was exposed distally at t

PURPOSE: Our goals were to assess image quality of time-resolved contrast-enhanced MR angiography (CE MRA), by using 3D data acquisition along with a parallel imaging technique that can improve temporal resolution and to compare this technique with 3D-time-of-flight (TOF) MRA in the postoperative assessment of extracranial (EC)-intracranial (IC) bypass surgery.
METHODS: On a 1.5T imaging system, we performed CE MRA by using a 3D fast field-echo sequence in combination with a parallel imaging technique, to obtain images in the coronal plane centered at the postoperative site. Our patient group

Patients with a ruptured intracranial aneurysm should be treated as soon as possible after the haemorrhage to prevent rebleeding and to allow vigorous treatment of ischemic events in case of vasospasm. The choice of treatment, endovascular or surgical, should be based on the angio-architectural aspects of the aneurysm. 3D rotational subtraction angiography will more clearly show the aneurysm morphology and will therefore help in the decision-making process. If an aneurysm is suitable for endovascular treatment ('coiling'), this should be the treatment of first choice, as has been clarified in

An 8-year-old boy was found to have primary moyamoya disease associated with a brain stem glioma. For over 3 years the child had experienced transient ischemic attacks induced by hyperventilation. One month before referral to our hospital he had presented with progressive left facial nerve palsy. Magnetic resonance imaging showed a cystic mass in the lower pons. Angiography revealed severe bilateral stenosis of the internal carotid arteries and prominent moyamoya vessels in the basal ganglia. Partial resection of the tumor yielded a histological diagnosis of pilocytic astrocytoma. Local radiat

The jugular bulb oxygen saturation (SjO2) and end-tidal carbon dioxide (ETCO2) were monitored continuously during surgery in six cases of Moyamoya disease who had demonstrated multiple episodes of transient ischemic attacks (TIAs) and/or fluctuating neurological deficits preoperatively. The arterial carbon dioxide tension (PaCO2) levels were also measured repeatedly at predetermined interval. In two cases (group H), the ETCO2 was controlled at hypercapnic levels during surgery (45.5 +/- 1.5 mmHg) and the remaining four (group N) were operated on in a normocapnic state (39.0 +/- 2.0 mmHg). To elucidate the relationship between cerebral blood flow (CBF) after hyperventilation (HV) and the rebuild-up phenomenon on the encephaloelectrogram (EEG) in children with moyamoya disease, comparative study of CBF immediately after HV and CBF after administration of acetazolamide (Diamox) was examined. CBF was measured by means of single photon emission CT (SPECT) using the 133Xe inhalation method. The subjects were 11 children (21 hemispheres) with moyamoya disease who were divided into two groups as follows, 1) Bypass group; who underwent STA-MCA anastomosis with other synangiosis. 2) Non-

Carotid artery reconstructive surgery for atherosclerotic lesions of the extracranial cerebral circulation has become the most common operation in peripheral vascular surgery. A better understanding of the indications for operative intervention, enhanced monitoring during surgery, and more precise management of intraoperative anesthesia have all helped decrease the risks associated with internal carotid endarterectomy. To evaluate the safety and efficacy of extracranial carotid reconstructive surgery, we reviewed 2,857 operations done on 2,087 patients from 1976 to 1985. Operation was recommen
