Medial fracture line significance in calcaneus fracture.

In Sanders' classification of calcaneus fractures, the medial fracture line (subtype C) is close to the tarsal canal, which contains an artery for the talus and calcaneus. We hypothesized that because of this brittle vascular localization, patients with C line fracture patterns might describe radiologic subtalar arthritis more often and have more complaints. The purpose of the present study was to compare the results of C line fracture patterns with other types of calcaneus fractures. A total of 25 surgically treated feet were involved. Regarding Sanders' classification, group 1 included fract

Treatment of fractures of the ulnar coronoid process.

OBJECTIVE: To discuss the classification, management and outcome of fractures of the ulnar coronoid process. METHODS: Retrospective analysis was carried out in 31 patients (19 men and 12 women of average age 29.8 years [range, 18-52 years]) with fractures of the ulnar coronoid process. The fractures were classified into four major groups based on the extent of injury to the ulnar coronoid process, the state of the anterior bundle of the ulnar collateral ligaments (UCL) and elbow stability. A fracture of the coronoid process less than halfway up was defined as type I (eleven cases); of the mid

Managing Low-Velocity Mandibular Gunshot Wounds: An Institutional Review and the "FLOSS" Severity Scoring System.

OBJECTIVE: Management of mandibular ballistic trauma is poorly delineated, given the variable injury complexity. This study examines surgical outcomes and presents a novel scoring system to define and guide the management of low-velocity ballistic mandibular fractures. METHODS: A retrospective chart review was performed from 2015 to 2022 to collect data on patients who suffered ballistic mandibular fractures. Computerized tomography facial bone scans were analyzed to determine a mandible severity scoring based on the following "F.L.OS.S." variables: fracture description (F), location of fract

The value of ultrasonography in predicting the outcomes of simple long bone fractures treated by closed intramedullary nail fixation.

PURPOSE: Ultrasonography has been used increasingly in orthopaedic practice credited to its low cost, easy accessibility, non-invasiveness, reproducibility, and safety from radiation. The purpose of this study was to test the validity and efficacy of ultrasonography as an adjunct in the assessment of fracture healing in long bones treated with intramedullary interlocking devices and its predictive value in determining the need for a secondary surgical procedure. METHODS: This was a descriptive longitudinal study of 40 skeletally mature patients from November 2016 to February 2019, who sustain

Fracture of the acetabulum: a retrospective review of ninety-one patients treated at a single institution.

Acetabular fracture result in fairly good outcome after the anatomic reduction in the displaced fracture fragments and damaged joint structure, but some patients will inevitably suffer from hip joint problems during their courses after the insult. We retrospectively reviewed 91 patients with acetabular fractures to investigate the causes of clinical failure and relationship among the fracture types, selected treatment options and their courses. Ninety-one patients (73 men and 18 women) with an average age of 49 years (range 18-80) at the time of injury were followed up for an average of 8.6 ye

Associated both-column acetabular fracture managed with anterior-only approach: a case report.

Associated both-column (ABC) acetabular fractures are complex injuries often requiring combined surgical approaches, however this case evaluates the efficacy of an anterior-only technique. A 58-year-old male sustained an ABC fracture after a 3-meter fall, managed via a Pfannenstiel and ilioinguinal first-window approach. Fixation targeted the iliac crest (4.5 mm cannulated screws), comminuted iliac wing (calcaneal plate), and anterior column (reconstruction plate), achieving satisfactory reduction. Posterior fixation was not performed due to satisfactory intraoperative alignment. Pain, functio

Osteosarcoma of the proximal tibia in a dog 6 years after tibial tuberosity advancement.

A 9-year-old spayed female Cane Corso dog was presented with a 1-week history of an acute right hind limb non-weight-bearing lameness. Six years previously, a right tibial tuberosity advancement (TTA) procedure had been performed. Orthogonal radiographs of the right pelvic limb taken before presentation revealed a comminuted displaced mid-diaphyseal tibial fracture. An aggressive bone lesion with osteolysis and periosteal reaction of the proximal tibia were evident that were most consistent with a primary bone tumor. The owner elected to have the dog euthanized. After euthanasia a fine-needle

Crossing Suture Technique for the Osteochondral Fractures Repair of Patella.

Osteochondral fracture of the patella is a common concomitant injury of the knee, especially in lateral patellar instability, and the importance of early stable fixation with minimal complication and early mobilization should be emphasized. Screws and Kirschner wires both absorbable and nonabsorbable have been the common mode of fixation of these fractures. Nevertheless, these fixation techniques require larger osteochondral fragments and are associated with cartilage abrasion, hardware prominence, synovitis, and foreign body reaction. In contrast, suture fixation can adequately stabilize smal

Frontal sinus fractures: anterior skull base.

The frontal sinus is fractured in 30% of patients sustaining maxillofacial injury. The CT scan has simplified the treatment plan allowing the surgeon to be more selective in the treatment choice, surgical approach, and follow-up. Fractures may involve the anterior, posterior, or both tables of the sinus. The frontal sinus repair or surgery takes place when there is minimal displacement and the frontal ducts are in tact. Obliteration of the sinus is suggested when the ducts are injured, but there is no significant bone loss or comminution of the posterior table. More extensive injuries of the p

Clavicular Distraction Osteoplasty With Sternoclavicular Reconstruction for a Medial Clavicle Fracture: A Case Report.

CASE: An 80-year-old man with osteoporosis sustained a complex, comminuted medial clavicle (MC) fracture that failed nonoperatively. A novel reconstruction involved MC resection, midclavicle distraction osteoplasty (DO) with iliac crest bone graft, and sternoclavicular (SC) joint reconstruction using a semitendinosus tendon allograft. By 1 year, the patient achieved successful healing and functional recovery. CONCLUSION: Patients with excessive MC bone loss may require a clavicular lengthening to approximate the MC to the manubrium. Restoring clavicular length through a DO in conjunction with

Role for Wrist Hemiarthroplasty in Acute Irreparable Distal Radius Fracture in the Elderly.

Volar locking plates for distal radius fracture (DRF) in the elderly may show complications in the most comminuted osteoporotic cases. The authors provide criteria for DRF in elderly that may not be amenable to volar plating ("irreparable DRF") and review the current results of a preliminary series of wrist hemiarthroplasty for these injuries. Between 2011 and 2019, 28 wrists with acute irreparable intra-articular DRF were treated with wrist hemiarthroplasty (96% female, mean age 79 years). A total of 17 wrists with a mean follow-up of 32 months were reviewed. At follow-up, mean visual analog

Efficacy of composite allograft and demineralized bone matrix graft in treating tibial plateau fractures with bone loss.

Tibial plateau fractures with bone loss or significant comminution require grafting and stable fixation. We hypothesized a standardized protocol of internal fixation augmented with a mixture of demineralized bone matrix and corticocancellous allograft chips would result in high healing rates with minimal subsidence. Union was achieved in all 36 patients available for follow-up by a mean of 4.4 months. Mean range of motion was 2 degrees to 120 degrees. One patient developed osteomyelitis. Subsidence ranging from 2.5 to 5.7 mm occurred in 4 patients (11%). This treatment method provides sufficie

[Surgical stabilization of multiple rib fractures can shorten inpatient stay].

Flail chest is a common complication in patients with blunt chest wall traumas resulting in high mortality rates. In this case report a 43-year-old woman was stepped on by a horse, thereby receiving multiple rib fractures and pneumohaemothorax. She was not able to wean from epidural analgesia after ten days and maintained a "thoracic floating feeling". In opposition to the non-operative management previously preferred, a surgical stabilization was then performed in the patient, who was discharged only five days later. Thus, surgical fixation of multiple rib fractures may reduce inpatient stay.

[The problem of the sacrum fracture. Clinical analysis of 377 cases].

Sacral fractures are rare injuries that are often neglected because of the general severity of the patient's injuries. They are typical injuries in patients with polytraumata. A "hidden" injury, they are often diagnosed late or are even missed. In a well-documented consecutive series of 1,350 patients with pelvic fractures treated in the trauma department of the Hannover Medical School between 1972 and 1991, a total of 377 sacrum fractures were evaluated in a retrospective study. The cause of the accident, mechanism of injury, concomitant injuries, diagnostic procedures, classification of the

Patellectomy: an overview with reconstructive procedures.

Patellectomy is a relatively old procedure and its effect on knee function has been a matter of controversy. Although indications for a patellectomy have been narrowed, patellectomy still is indicated as the last choice of treatment in certain situations including comminuted fractures, advanced chondromalacia or osteoarthritis, dislocations, infections, and tumoral conditions. Excision of the patella results in a decrease in the moment arm of the extensor mechanism, alteration of the forces acting on the tibiofemoral joint and instant center of motion, limitation of range of motion, anterior i

Injuries to the hand inflicted by rotary snowcutters.

In the period January 1, 1975, through December 31, 1986, 28 patients were treated for hand injuries caused by small home rotary snowcutters. These injuries were characterized by extensive lacerations and contusions, particularly on the dorsal side of the hand and the fingers, with concomitant extensor tendon ruptures, comminuted and complicated fractures, and traumatic amputations of fingers of the dominant hand. The index and middle finger were nearly always injured; the thumb was usually totally spared. The frequency of postoperative complications was low after delayed primary surgical trea

A novel nail-plate construct for the treatment of AO/OTA 31-A3.3 intertrochanteric fractures: a finite element analysis.

BACKGROUND: The AO/OTA 31-A3.3 is the most unstable type with a lesser trochanteric fragment and a broken lateral femoral wall (LFW), which constitute a four-part unstable intertrochanteric fracture. Implant failure remains one of the catastrophic consequences after surgical treatment. A novel nail-plate construct, called proximal femoral universal nail system (PFUN), is proposed by our team to fix comminuted LFW fracture fragment and lesser trochanteric fragment. The aim of this study is to evaluate the biomechanical properties of PFUN compared with proximal femoral nails anti-rotation (PFNA)

Preoperative HIFU ablation combined with femoral bone marrow nailing for the treatment of pathological fracture of femur: a case report.

INTRODUCTION AND IMPORTANCE: Bone is one of the common sites of metastasis in lung cancer. Pathological fractures of the femur significantly reduce patients' quality of life and increase the risk of death. However, there is still no consensus on the optimal treatment of pathological femoral fractures. The authors' report provides a treatment method for a patient with pathological fracture of lung cancer with preoperative HIFU lesion ablation followed by combined intramedullary nail fixation. CASE PRESENTATION: A 61-year-old Chinese woman was hospitalized with severe pain in her right thigh. X
