Total hip arthroplasty (THA) with conventional polyethylene bearings is traditionally the standard operative treatment for endstage arthritis of the hip. This design has excellent survivorship in most populations, with a low occurrence of infection and other associated complications. Mechanical failure of the long gamma nail was encountered in two elderly patients with proximal femur fractures over a 6-month period. One of the patients had a known history of lymphoma. The other patient had a history of rheumatoid arthritis but no history of cancer or other metabolic bone disease. Both nails failed at the junction of the compression screw and the nail. The angle of failure of the nail was the same in both cases. The index of suspicion for imminent mechanical failure of the gamma nail should be higher in pathologic fractures and fractures which are malreduced during nailing.

In 2005, a nationwide clubfoot treatment program focused on the Ponseti method -an effective, affordable and minimally-invasive method- was initiated in China. The purpose of this study was to evaluate and identify barriers to the program. Olecranon epiphyseal stress fractures and epiphyseal non-unions have been described in throwing athletes, weight lifters and gymnasts. We present a case in which bilateral olecranon epiphyseal fractures were diagnosed in a competitive NCAA Division One wrestler who presented with chronic elbow pain. Given the rigors and physical demands of collegiate wrestling, we present a novel technique for open reduction internal fixation, grafting and supplementation with BMP for accelerated healing and return to competition.

Twenty-eight knees in 26 patients underwent revision TKA requiring surgical management of major osteolytic defects. Three groups of osteolytic defects were identified based upon the degree of implant stability and the magnitude of bone loss. Outcome measures included the Knee Society Clinical Rating Score (KSCRS), visual analog pain score, and radiographs. SUMMARY OF BACKGROUND DATA: Spinal injections with steroids play an important role in non-operative care of lumbar radiculopathy. The trans-foraminal epidural steroid injection (TESI) theoretically has a higher success rate based on targeted delivery to the symptomatic nerve root. We describe a case of primary articular synovial chondromatosis in a metacarpophalangeal (MCP) joint of a 48-year-old male. The patient initially presented with MCP joint swelling and pain with normal plain radiographs. Following a fourteen month course of conservative management, surgical debridement of the joint revealed multiple loose bodies consistent with primary articular synovial chondromatosis. Though synovial chondromatosis rarely occurs in hand joints, the diagnosis should be considered in cases of metacarpophalangeal pain when common etiologies have been excluded.
