[Space flight/bedrest immobilization and bone. Mechanical stress : a double-edged sword to osteoarthritis].

Osteoarthritis (OA) is a disease caused by the degeneration and destruction of joint cartilage followed by peri-articular or subchondral bone formation and concomitant degeneration of other components of joints including synovium. Several animal OA models that adopt surgically induced joint instability have been developed. Analysis of those OA models indicates that increased mechanical stress exacerbates OA. In contrast, the effectiveness of physical therapy to alleviate OA symptoms suggests that optimal mechanical stimulation can impede the progression of OA. We propose that there are two fac

Duration of postoperative antibiotic therapy following revision for infected knee and hip arthroplasties.

In the period from 1990 to 1998 28 patients (14 males and 14 females) were treated for infected hip (19 cases) and knee (9 cases) arthroplasties. The median age at index operation was 72 y (range 34-82 y) and at revision surgery 74 y (range 36-83 y). The primary diagnosis was osteoarthritis (16 cases), rheumatoid arthritis (5 cases), failed femoral neck fracture (6 cases) and arthritis secondary to congenital hip dislocation (1 case). Nine cases were infected early, i.e. within 3 months postoperatively, and 19 had late infections. The median interval from index operation to diagnosis of the in

[Injuries in the First German Inline Skater Hockey League].

Inline skater hockey is a sport originating from ice hockey, which is practised in Germany by about 6000 athletes. There are some differences to ice hockey, which lead to a special risk profile of the athletes. All 315 active players of the First German Inline Skater Hockey League in the years 2018 and 2019 were contacted to participate in an online survey. The subjects of the study completed an anonymised 83-item multiple-choice questionnaire about injuries, training frequency, training content, and sports equipment. 178 athletes responded and 116 questionnaires could be analysed (100 men, 8 w

Cartilage restoration procedures in patellofemoral stabilization.

Patellar instability and dislocation events are often associated with chondral injury to the patellofemoral joint. Patients may present with swelling, anterior knee pain, and mechanical symptoms. Clinical evaluation should include a detailed history, assessment of standing and rotational alignment, and radiographic and advanced imaging. Operative intervention for treating instability includes soft tissue reconstruction, trochleoplasty, and osteotomies, as both soft tissue and bony pathoanatomic factors contribute to patellar maltracking. Cartilage injury in the setting of patellofemoral instab

Complications of periprosthetic fracture revision vs aseptic revision of total knee arthroplasty.

BACKGROUND: Periprosthetic fractures after total knee arthroplasty (TKA) are a challenging problem due to complex fracture patterns, poor bone quality, and a high-risk patient population. Treatment of both periprosthetic fractures and aseptic complications can include revision TKA. In this study, we compared systemic and orthopaedic complications following periprosthetic fracture associated revision TKA to aseptic revision TKA. METHODS: This is a retrospective cohort study using data from the years 2010-2020 from a national administrative claims database. Billing codes were used to identify r

Cementing a Monoblock Dual-Mobility Implant into a Fully Porous Cup in Revision Total Hip Arthroplasty to Address Hip Instability: Surgical Technique.

BACKGROUND: The use of a cemented monoblock dual-mobility implant into a fully porous cup is indicated for patients with acetabular bone loss who have a high risk of postoperative hip instability. Patients undergoing lumbar fusion for sagittal spinal deformities have an increased risk of hip dislocation (7.1%) and should be assessed on sitting and standing radiographs1. Gabor et al. conducted a multicenter, retrospective study assessing the use of a cemented monoblock dual-mobility bearing in a porous acetabular shell in patients with acetabular bone loss and a high risk of hip instability2. Does preoperative patellofemoral joint state affect medial unicompartmental arthroplasty survival?

One contested contraindication to medial unicompartmental knee arthroplasty (UKA) has been status of the patellofemoral joint. Surgeons have avoided UKA when the patellofemoral joint has radiographic evidence of arthritic changes. However, recent studies advocate ignoring patellofemoral joint status when considering UKA. The purpose of this study was to compare the failure rate of mobile-bearing, medial UKA in patients with and without preoperative radiographic evidence of patellofemoral joint degeneration. Preoperative radiographs from a random selection of 503 patients (638 knees) treated wi

A comparison of four models of total knee-replacement prostheses.

Twenty-nine knees with unicondylar, sixty-four with duocondylar, fifty with Guepar, and fifty with geometric prostheses were studied. The follow-up ranged from two to three and one-half years. The unicondylar prosthesis was used in the mildest cases and gave the least complications, but the quality of results was not superior to that achieved with the other prostheses. The duocondylar model was best suited for knees with rheumatoid arthritis and mild deformity. The geometric prosthesis was the best condylar prosthesis for osteoarthritis with moderate to severe deformity, but gave the worst res
