Quantitative analysis of the visual vestibulo-ocular reflex using sinusoidal rotation in patients with peripheral vestibular disorders.

In order to investigate interaction of the vestibular and optokinetic systems in patients with peripheral vestibular disorders, the visual vestibulo-ocular reflex (V-VOR) gain in 49 patients with peripheral vestibular disorders was quantitatively measured by using pendular harmonic sinusoidal rotation with optokinetic stationary stimuli. The patients were rotated sinusoidally both with eyes closed and open at an amplitude of 240 degrees and at a frequency of 0.1 Hz (peak velocity 75.4%s). Comparing the vestibulo-ocular reflex (VOR) and V-VOR gains in the patients with definite Meniere's diseas

Hearing preservation in vestibular neurectomy.

The goal of vestibular neurectomy is to control disabling vertigo while preserving hearing in patients with nonhydropic intractable peripheral vertigo or in patients with Meniere's disease in which an endolymphatic sac procedure has failed. Labyrinthectomy continues to be used to treat patients with intractable vertigo and serviceable hearing. We feel that a labyrinthectomy is contraindicated when any useful hearing remains. Vestibular neurectomy affords the surgeon a means to eliminate the abnormal vestibular input without sacrificing hearing. Two approaches have been used to section the vest

Middle-ear cholesteatoma co-existing with labyrinthine fistula and vestibular schwannoma.

BACKGROUND: Many conditions, among them vestibular schwannoma and middle ear cholesteatoma with lateral semicircular canal destruction, may be associated with asymmetrical sensorineural hearing loss (SNHL) and vertigo. However, the probability that these two distinct disease entities causing the same symptoms occur in a single patient is very low, approximately 1 per 28 billion per 1 year. METHODS: We present the case of a 40-year-old male admitted to our clinic because of chronic middle ear inflammation with concomitant tinnitus vertigo, and deafness in the right ear. The patient was diagnos

Vestibular neurons with direct projections to the solitary nucleus in the rat.

Anterograde and retrograde tract tracing were combined with neurotransmitter and modulator immunolabeling to identify the chemical anatomy of vestibular nuclear neurons with direct projections to the solitary nucleus in rats. Direct, sparsely branched but highly varicose axonal projections from neurons in the caudal vestibular nuclei to the solitary nucleus were observed. The vestibular neurons giving rise to these projections were predominantly located in ipsilateral medial vestibular nucleus. The cell bodies were intensely glutamate immunofluorescent, and their axonal processes contained ves

Hypothalamic hamartomas and inner ear diverticula with X-linked stapes gusher syndrome - new associations?

X-linked stapes gusher syndrome is a genetic form of deafness with distinct radiographic features on temporal bone CT. Hypothalamic hamartoma is a congenital glioneuronal anomaly of the hypothalamus. We report a potential association between these two rare anomalies that, to our knowledge, has not been reported. Two brothers presented with sensorineural hearing loss and almost identical inner ear and hypothalamic abnormalities, consistent with a diagnosis of X-linked stapes gusher syndrome and hypothalamic hamartoma. Genetic testing revealed identical mutations in the POU3F4 gene associated wi

Activating Fgfr3 Y367C mutation causes hearing loss and inner ear defect in a mouse model of chondrodysplasia.

Fibroblast growth factor receptor 3 (FGFR3) is a key regulator of skeletal development and activating mutations in FGFR3 cause skeletal dysplasias, including hypochondroplasia, achondroplasia and thanatophoric dysplasia. The introduction of the Y367C mutation corresponding to the human Y373C thanatophoric dysplasia type I (TDI) mutation into the mouse genome, resulted in dwarfism with a skeletal phenotype remarkably similar to that of human chondrodysplasia. To investigate the role of the activating Fgfr3 Y367C mutation in auditory function, the middle and inner ear of the heterozygous mutant

Monitoring of the glycerol test with sinusoidal harmonic acceleration in Menière's disease patients.

Changes in auditory pure tone threshold and speech discrimination and the phase and gain in the sinusoidal harmonic acceleration test were measured in subjects with endolymphatic hydrops (Meniere's disease) following the administration of glycerol. Although all subjects showed a marked improvement in auditory function, a comparable change in vestibular function, as measured by the sinusoidal harmonic acceleration test, was not observed.

The acoustic reflex test in cochlear and eighth nerve pathology ears.

The clinical interpretation of acoustic reflex test results in ears with sensorineural impairment should be based on a consideration of three characteristics of the response: the reflex threshold hearing level, the reflex threshold sensation level, and the decay of the reflex. Examination of results in 152 ears with cochlear pathology and in 152 ears with acoustic tumor indicated that in some cases the combination of characteristics may be contradictory as to site of lesion. Regarding a contradictory response pattern as questionable -- neither confirming nor denying a specific site of lesion -

Clinical examination and management of the dizzy patient.

Dizziness is the commonest reason for a GP appointment over the age of 75 years and many dizzy patients will end up in hospital. This article introduces the range of vestibular disorders commonly seen in hospital practice with a symptom-based approach and discusses the range of treatments that might be considered.
