The lumbar vessels in the retroperitoneum must be addressed during many urological and vascular procedures. Few studies have assessed the exact pattern of the lumbar vasculature, and most anatomical descriptions suggest the presence of a generally regular pattern. The number and location of the infrarenal lumbar arteries and veins were documented in 102 consecutive retroperitoneal lymph node dissections. Anomalies of the renal vessels were also noted. Wegener's granulomatosis appeared as localized or infiltrative tumefactions in six patients. The tissue swellings affected the retroperitoneum, mediastinum (two), breast, retroorbital tissue (two), and gingiva. This is a case report on an endometriosis in retroperitoneally situated lymphatic nodes in a 29-year-old patient. The patient had a cervical carcinoma FIGO Ib and was treated surgically by radical hysterectomy after Wertheim-Meigs-Okabayashi with pelvic lymphonodectomy. Endometriosis was found in two lymphatic nodes of the retroperitoneum. The patient had no anamnesis or clinical symptoms of endometriosis. Endometriosis could also not be found in any other parts of the abdomen.

Approximately 30% of patients with disseminated testis cancer who receive platinum-based chemotherapy will experience normalization of tumor markers but have persistent, radiographically evident disease in the retroperitoneum. From 1984 to 1991 20 multiple trauma patients with pelvic fractures and retroperitoneal bleeding from pelvic vessels underwent angiographic localization and embolization of massively bleeding arterial vessels. A case is reported of classic granulosa cell tumor of the testis with metastases to the retroperitoneal lymph nodes occurring in a 26-year-old man. The patient had left-sided testicular enlargement and bilateral gynecomastia. He was treated by radical orchiectomy, retroperitoneal lymph node dissection, and radiation therapy and is well without evidence of disease 14 years after diagnosis. This is the first documented case of granulosa cell tumor of the testis with metastases and long remission after successful therapy to the authors' knowledge.

Retroperitoneal hematoma resulting in femoral nerve injury is a serious potential complication of systemic heparin anticoagulation. Review of the literature reveals lack of agreement with respect to conservative versus surgical management. The authors report the first case in which return of function was established by percutaneous decompression of a retroperitoneal hematoma in a patient who was not a candidate for immediate surgery. The favorable result suggests that percutaneous drainage may represent a reasonable alternative or first step in surgical treatment of this compression.

A case of retroperitoneal perforation of the rectum complicating a single contrast barium enema is presented. The appearances on computed tomography (CT) allowed accurate depiction of the extent of barium extravasation not appreciated on plain films. Although this is an unusual complication of the barium enema, CT may prove useful when the suspicion of perforation arises.

Sixty patients had a radical abdominal hysterectomy with or without a bilateral salpingo-oophorectomy. During the period 1963--76, sixty-nine patients underwent aorto-iliac of aorto-femoral by-pass operation at the department. Fifty-two of the operations were performed through a retroperitoneal approach. The operative procedure is described. The approach is found gratifying and the statement of earlier authors concerning fewer cases of paralytic ileus and smoother convalescence is confirmed.
