Myofascial face pain. Clinical characteristics of those with regional vs. widespread pain.

BACKGROUND: The authors conducted a study to determine whether there are differences in salient clinical characteristics between patients who have both myofascial face pain, or MFP, and comorbid fibromyalgia, or FM, and patients who have MFP but not FM. METHODS: The authors enrolled in the study 162 female subjects who had histories of MFP. In physical examinations at the time of initial consultation, they recorded facial pain signs and symptoms. At the research interview follow-up (seven years post-consultation), participants were screened for a lifetime history of FM and other health proble

Genetic and sociodemographic factors associated with trajectories of physical and mental health multimorbidity in a South Asian cohort in the UK: A multistate modelling analysis.

BACKGROUND: UK South Asian populations are at high risk of physical and mental health multimorbidity, which means they live with multiple long-term conditions. The life course emergence of multimorbidity, its underlying aetiology, and consequences for future health and mortality have yet to be studied in this population. METHODS AND FINDINGS: We studied Internalising (depression, anxiety, somatoform disorders) and Cardiometabolic (hypertension, obesity, type 2 diabetes, chronic kidney disease, dyslipidaemia) MultiMorbidity (ICM-MM): the lifetime occurrence of ≥1 internalising mental health co…

[Functional urination disorders in patients recovering in an intensive care unit for more than 30 days].

Patients in rest-homes, intensive care units, long hospitalization and dialysis and whoever stays in a hospital continuously or occasionally for long periods of time may run into the well-known and typical complications of prolonged hospitalization. We have studied the urologic aspect in all the cases of the Medical- Surgical Emergency Institute in the Hospital of Ancona (seated in Torrette), over the period 1990-96, in patients hospitalized for at least 30 days. All the patients were followed and studied with the same method, though taking into account their different pathological conditions.

Voxel-based global-brain functional connectivity alterations in first-episode drug-naive patients with somatization disorder.

BACKGROUND: Altered functional connectivity (FC) is associated with the pathophysiology of patients with somatization disorder (SD). However, inconsistent results were obtained due to different selections of regions of interest (ROIs) in previous researches. This study aims to examine voxel-wise brain-wide FC alterations in patients with first-episode, drug-naive SD in an unbiased way. METHODS: A total of 25 patients with SD and 28 age-, sex-, and education-matched healthy controls underwent resting-state functional magnetic resonance imaging. Global-brain FC (GFC) was applied to analyze the

The Prevalence and Impact of Overlapping Rome IV-Diagnosed Functional Gastrointestinal Disorders on Somatization, Quality of Life, and Healthcare Utilization: A Cross-Sectional General Population Study in Three Countries.

OBJECTIVES: The population prevalence of Rome IV-diagnosed functional gastrointestinal disorders (FGIDs) and their cumulative effect on health impairment is unknown. METHODS: An internet-based cross-sectional health survey was completed by 5,931 of 6,300 general population adults from three English-speaking countries (2100 each from USA, Canada, and UK). Quota-based sampling was used to generate demographically balanced and population representative samples with regards to age, sex, and education level. The survey enquired for demographics, medication, surgical history, somatization, quality

Enhancing Diagnostic Performance of Symptom-Based Criteria for Irritable Bowel Syndrome by Additional History and Limited Diagnostic Evaluation.

OBJECTIVES: Symptom-based criteria to diagnose irritable bowel syndrome (IBS) positively perform only modestly. Our aim was to assess whether including other items from the clinical history and limited diagnostic evaluation improves their performance. METHODS: We collected complete symptom, colonoscopy, and histology data from 318 consecutive, unselected adult patients with lower gastrointestinal (GI) symptoms in secondary care. All participants underwent colonoscopy, with relevant organic findings recorded. The reference standard used to define the presence of true IBS was patient-reported l

Does psychological distress vary between younger and older adults in health and disease?

OBJECTIVE: The effect of age on psychological distress remains controversial and it is unclear how a chronic medical illness influences this association. We aimed to compare the level of psychological distress between younger and older patients with chronic medical conditions attending hospital specialty clinics and to test whether a different pattern emerges when comparisons with individuals without long-term conditions are made. METHODS: In 519 individuals without chronic medical conditions and 949 patients with established severe chronic medical illnesses, we compared psychological distres

Symptoms, personality traits, and stress in people with mobile phone-related symptoms and electromagnetic hypersensitivity.

OBJECTIVE: Some people report symptoms that they associate with electromagnetic field (EMF) exposure. These symptoms may be related to specific EMF sources or to electrical equipment in general (perceived electromagnetic hypersensitivity, EHS). Research and clinical observations suggest a difference between mobile phone (MP)-related symptoms and EHS with respect to symptom prevalence, psychological factors, and health prognosis. This study assessed prevalence of EMF-related and EMF-nonrelated symptoms, anxiety, depression, somatization, exhaustion, and stress in people with MP-related symptoms

Facets of job effort in bus driver health: deconstructing "effort" in the effort-reward imbalance model.

This research aimed to test the relative value of developing and using job-specific facets of effort and testing them using J. Siegrist's (1996) effort-reward imbalance (ERI) theory to extend understanding of how one might determine job strain in urban bus driving. In addition, the interactive effects of the ERI model are further investigated to address the lack of research into the relationships of the model's constructs. Using focus groups and published papers, a measure of bus driver effort was created, which was subsequently completed by 186 male U.K. bus drivers as part of a questionnaire

Management of functional somatic syndromes.

Although functional somatic syndromes (FSS) show substantial overlap, treatment research is mostly confined to single syndromes, with a lack of valid and generally accepted diagnostic criteria across medical specialties. Here, we review management for the full variety of FSS, drawn from systematic reviews and meta-analyses since 2001, and give recommendations for a stepped care approach that differentiates between uncomplicated and complicated FSS. Non-pharmacological treatments involving active participation of patients, such as exercise and psychotherapy, seem to be more effective than those

Unexplained swelling symptoms in women (idiopathic oedema) comprise one component of a common polysymptomatic syndrome.

BACKGROUND: Non-menstrually-related swelling symptoms (idiopathic oedema) are common in women. The community prevalence of such symptoms, their association with other symptoms, and their underlying aetiology, are uncertain. AIM: To determine the community prevalence of swelling symptoms and the independent contributions of major risk factors. DESIGN: Comparison of major risk factors in women with and without swelling symptoms. METHODS: We assessed 196 women attending a menopause clinic, 201 women attending a fracture clinic and 201 women attending their general practitioner. Each documented

The reliability and validity of a sexual and physical abuse history questionnaire in female patients with gastrointestinal disorders.

Although the literature on the psychological and medical sequelae of sexual and physical abuse has been growing, researchers have made few attempts to standardize the measurement of abuse history. The authors of this article report the first standardization of a screening instrument to identify sexual and physical abuse in a medical population. Reliability was assessed using test-retest methodology, and validity was supported by comparison with an interview (criterion validity). The sample included 139 female patients in a gastroenterology clinic. Data indicate acceptable test-retest reliabili

Tell me what to expect: how instructions affect the pain response of patients with chronic myofascial pain with referral.

The aims of the study are to analyze the influence of pain and no pain expectations on the physiological (electromyography (EMG) and pupillometry) and cognitive (Numerical Rating Scale (NRS)) response to pain. Pain expectation and no pain expectation situations were induced by employing instructional videos. The induction of pain was performed by palpating the masseter with an algometer in a sample of 2 groups: 30 healthy participants (control group) and 30 patients (Temporomandibular disorders (TMD) group) with chronic myofascial pain with referral in the masseter muscle (Diagnostic Criteria

Psychological traumatization and adverse life events in patients with organic and functional vestibular symptoms.

OBJECTIVE: . A relationship has frequently been found between a history of traumatization and the existence of somatoform symptoms. The objective of this study was to examine whether this relationship is also observed for functional, i.e. medically not sufficiently explained, vestibular symptoms (FVS). We tested whether patients with FVS and organically explained vestibular symptoms (OVS) differ with regard to frequencies of previous traumatic experiences and posttraumatic stress symptoms. We also explored whether the impact of previous trauma was associated with characteristics of vestibular

What characterizes individuals developing chronic whiplash?: The Nord-Trøndelag Health Study (HUNT).

OBJECTIVE: Most individuals experiencing whiplash accidents recover rapidly. A considerable proportion, however, develop chronic symptoms. Psychological factors may slow recovery, possibly by increasing the likelihood of other symptoms being misattributed to, and amplified by the whiplash injury. We aimed to investigate how pre-injury mental and somatic symptoms, self-rated health, use of health-services and medications, health-behavior and socio-demographics predict the development of chronic whiplash. METHODS: Data from two waves of a large, population based study (HUNT2 (baseline) and HUNT

Evaluation of a focused short-term preventive counselling project for families with a parent with cancer.

Twenty-four families participated in counselling for families with a parent with cancer (24 mothers, 17 fathers, and 34 children). Parents who received counselling were significantly more depressed before the counselling than a nonrandomized control group who did not receive counselling, but participated in another part of the project. For the parents, there was a significant decrease in depression and increase in family functioning scores from before to after the intervention. For the children, a significant pre- to post-decrease in depression scores was found. Changes in depression and famil
