Acquired adult flat foot due to isolated plantar calcaneonavicular (spring) ligament insufficiency with a normal tibialis posterior tendon.

BACKGROUND: We present nine cases of acquired flat foot deformity (AFFD) in adults caused by isolated spring ligament insufficiency. METHODS: We present the clinical sign that allows differentiation of this diagnosis from posterior tibialis tendon (PTT) dysfunction namely the ability to single leg tiptoe, but with persistent forefoot abduction and heel valgus. In addition we illustrate the unique radiological features which allow confirmation of the diagnosis. Only a solitary previous case report has documented this alternative aetiology of AFFD; in that case, diagnosis was made intra-operati

Leptospirosis mimicking sepsis after orthopedic surgery: a case report.

We report a case of leptospirosis that occurred after elective surgery involving tendon transfer and shoulder arthroscopy. The disease mimicked hospital infection after orthopedic surgery and was at first misdiagnosed as post-operative sepsis. The patient was 60 year old female that developed sepsis with hypotension, shock, bleeding, jaundice and renal insufficiency 4 hours after surgery. Shock treatment procedures were performed and broad spectrum antibiotic therapy was used with coverage for bacteria acquired in hospitals. A careful investigation was carried out by the Hospital Infection Con…

Moving car-tire injuries of the foot: reconstruction with microvascular free flaps.

Moving car-tire injury of the foot is a condition that appears to be a new clinical entity in Istanbul, in which the non-weight-bearing surface of the foot is prone to be injured. Between 1992 and 1995, eight moving car-tire injuries of the foot were managed. Patient age averaged 10.25 years (range: 7 to 21 years). All patients presented with soft-tissue loss combined with extensor tendon and bone lesions. In 6 patients, immediate flap coverage after initial debridement and, in 2 patients, delayed coverage after multiple debridements, were performed. All the flaps survived. Although in all pat

Distal median to ulnar nerve transfers to restore ulnar motor and sensory function within the hand: technical nuances.

ULNAR NERVE INJURIES can be severely debilitating and result in weakness of wrist flexion, loss of hand intrinsic function, and ulnar-sided hand anesthesia. When these injuries produce a Sunderland fourth- or fifth-degree injury, surgical intervention is necessary for functional recovery. Traditional methods for restoring hand intrinsic function after ulnar nerve palsy include interposition nerve grafting for timely presentations or tendon transfers for either complex injuries or late presentations. Distal median to ulnar nerve transfer to restore ulnar intrinsic nerve muscle function was firs…

Leptin receptor signaling mediates the distinct tendon-bone interface reconstruction in rotator cuff tears and osteoporosis-comorbid rotator cuff tears.

BACKGROUND: The intact tendon-bone interface (TBI) consists of four histological layers-tendon, fibrocartilage, calcified fibrocartilage, and bone-that gradually merge into each other, making complete structural restoration after injury challenging. Osteoporosis poses a significant risk for rotator cuff tears (RCTs) and re-tears after arthroscopic rotator cuff repair (ARCR). Activating Leptin receptor (Lepr) mediated Stat3 signaling transduction facilitates the transcription of Runx2 and Sox9, respectively, and promotes osteogenesis and chondrogenesis. MATERIALS AND METHODS: Sixty-five female

Single-Row Repair Versus Double-Row Repair in the Surgical Management of Achilles Insertional Tendinopathy: A Systematic Review.

BACKGROUND: Approximately 6% of people will report Achilles tendon pain during their lifetime, and one-third of these individuals will have Achilles insertional tendinopathy (AIT). For patients who have failed conservative treatment, surgical repair is performed. Achilles tendon repair can occur through various techniques, including a single-row or double-row repair. PURPOSE: To determine if there are significant advantages to double-row repair over single-row repair with respect to biomechanical and clinical outcomes. STUDY DESIGN: Systematic review; Level of evidence, 3. METHODS: A system

Transposition of the Semimembranosus as an Augmentation Technique for Anteromedial Rotatory Instability of the Knee: A Retrospective Case Series Study.

Anteromedial rotatory instability (AMRI) resulting from medial collateral ligament (MCL) injuries, often combined with anterior cruciate ligament (ACL) tears, poses a significant challenge in knee surgery. This study evaluates the use of a novel technique-semimembranosus (SM) tendon transposition-as an augmentation to ACL and superficial MCL (sMCL) reconstruction, reducing medial knee opening and AMRI. A case series of 15 patients with AMRI underwent ACL and sMCL reconstruction with SM tendon transposition between January 2017 and July 2021, with a minimum follow-up of 24 months. Inclusion cri

Rupture of the Extensor Hallucis Longus Muscle Secondary to Repetitive Overuse in a Taekwondo AthleteA Case Report.

Closed rupture of the extensor hallucis longus (EHL) tendon is rare, with most cases caused by either direct penetrating trauma or predisposing factors such as corticosteroid injection and iatrogenic trauma incidences. Almost all of the previous case reports have reported on rupture of the EHL tendon rather than the EHL muscle. In this report, we highlight an unusual clinical presentation of a rupture of the EHL muscle and discuss its predisposing factors. This patient was a taekwondo athlete with EHL muscle rupture secondary to repetitive overuse without any underlying systemic or local predi

Less invasive Achilles tendon reconstruction.

BACKGROUND: The optimal management of chronic ruptures of the Achilles tendon is surgical reconstruction. Reconstruction of the Achilles tendon using peroneus brevis has been widely reported. Classically, these procedures involve relatively long surgical wounds in a relatively hypovascular area which is susceptible to wound breakdown. RESULTS: We describe our current method of peroneus brevis reconstruction for the Achilles tendon using two para-midline incisions. CONCLUSION: This technique allows reconstruction of the Achilles tendon using peroneus brevis preserving skin integrity over the

Comparative evaluation of effectiveness of different motor muscles in modified lasso procedure for correction of finger clawing.

A retrospective study was carried out to compare the effectiveness of different muscles as motors in modified lasso procedures for correction of finger clawing in leprosy. It was observed that palmaris longus and extensor carpi radialis longus were more suitable than the flexor digitorum superficialis. In some patients, removal of superficialis is associated with complications which could not be predicted before surgery. Extensor carpi radialis longus has advantages over palmaris longus in selected cases.

Operative treatment of chronic distal biceps tendon ruptures.

Chronic biceps tendon ruptures typically involve tendon retraction, scarring, and even compromised tissue. Indirect repair, such as tenodesis to the brachialis, does not provide optimal functional recovery. Chronic biceps tendon ruptures can be reconstructed with autogenous grafts (semitendinosis, tensor fascia lata) or allografts (typically Achilles tendon). The complications associated with these grafts include harvest site morbidity and graft incorporation. Using a vascularized local soft tissue source could minimize complications of graft reconstructions. The authors provide a novel recons

Analysis of Tsuge's procedure for the treatment of radial nerve paralysis.

Radial deviation and limited flexion of the wrist joint and a lack of abduction of the thumb have been noticed after the Riordan's procedure. Therefore, Tsuge et al. modified the Riordan's procedure, and their procedure includes transfer of the pronator teres to the extensor carpi radialis brevis, the flexor carpi radialis (FCR) to the extensor digitorum communis (EDC), and the palmaris longus to the extensor pollicis longus, along with tenodesis of the abductor pollicis longus. We reviewed the charts of 21 patients with isolated radial nerve paralysis who were treated with the Tsuge's procedu

Soft Tissue Surgical Technique for Obligate Dislocation of the Patella.

BACKGROUND: Obligate dislocation of the patella (ODP) is a subtype of congenital disorders and dislocations of the patella. Severity of the condition varies; symptoms range from minor disabilities to impaired ambulation. Definitive treatment for ODP is surgery, with the choice of surgical procedure determined by skeletal maturity. CASE SERIES: This report documents a soft tissue surgical correction for 2 patients with ODP using (1) lateral release of the retinaculum, (2) Z-lengthening of the quadriceps tendon, (3) patellar tendon split transfer (medial side only), and (4) vastus medialis obli
