PURPOSE: Transgender and nonbinary young adults (TNB YA) in the United States (U.S.) have higher risk of adverse mental health outcomes than cisgender YA, due in part by exposure to social stressors such as online harassment.
METHODS: We analyzed cross-sectional data from an online survey of U.S. TNB YA ages 18-25 years (n = 152). We assessed lifetime experiences of online harassment in six domains (e.g., physical threat); participants reported whether the harassment was based on a stigmatized social status (e.g., gender, weight). Logistic models assessed associations between online harassmen

Transgender and gender-diverse (TGD) youth may present to the emergency department with a range of medical problems and health concerns. Some of these may be directly related to their gender identity, but the vast majority are not. While gender diversity is not considered a mental illness, TGD youth are at increased risk for suicide, anxiety, depression, and other psychological conditions, as well as family rejection, homelessness, food insecurity, and poverty. Lack of knowledge and cultural competency among emergency clinicians can create a barrier to effective care. This issue will review re

Insecure attachment is a well-established risk factor for the emergence of borderline personality features (BPF), encompassing identity disturbance, affective instability, problematic interpersonal relationships and self-harming behaviours. From a mentalizing-based perspective, BPF may develop through the interplay of hypomentalizing, defined as a reduced capacity to understand and reflect on one's own and others' mental states, and epistemic mistrust (EM), the diminished capacity to trust communicated information. Both processes are shaped by attachment anxiety and avoidance. However, researc

Purpose: Among black transgender women (transwomen) at high risk of HIV acquisition or already living with HIV/AIDS, the study examined whether medical or socially based gender affirming factors may contribute differentially to selected measures of resiliency, perceived stress, and a scale measure of mental health outcomes. This question has implications for clinical care and counseling of this population. Methods: Seventy-seven black transwomen were recruited to participate in a private, face-to-face structured interview. Two index measures of gender affirmation (GA) were constructed from the

Across the United States, Black women are likely to have unmet mental health needs, with greater personal, social, and structural barriers to mental health treatment compared to other groups. The Theory of Planned Behavior (TPB) may help to conceptualize young Black women’s mental health beliefs and help-seeking behaviors. The current study aimed to use the TPB framework and a concurrent mixed methods approach to explore behavioral, normative, and control beliefs toward seeking mental health care among a sample of young Black women from community settings. The sample consisted of 33 Black wome

A transgender (trans) man is a trans person who was assigned female sex at birth. Some trans men undergo gender-affirming surgical procedures, particularly mastectomy. We present a case of keloid formation after gender-affirming mastectomy that highlights the importance of choosing the most appropriate mastectomy technique in patients at risk of keloid and the most effective therapeutic strategies for anterior-chest keloids. A 40-year-old Japanese trans man developed keloids along inverted-T mastectomy-induced anterior-chest scars. The keloids were completely excised, and the defects were clos

PURPOSE: Lesbian, gay, bisexual, and transgender (LGBT) people are a health disparate population as identified in Healthy People 2020. Yet, there has been limited attention to how LGBT older adults maintain successful aging despite the adversity they face. Utilizing a Resilience Framework, this study investigates the relationship between physical and mental health-related quality of life (QOL) and covariates by age group.
DESIGN AND METHODS: A cross-sectional survey of LGBT adults aged 50 and older (N = 2,560) was conducted by Caring and Aging with Pride: The National Health, Aging, and Sexua

BACKGROUND: Accurate measurement of binge drinking is essential for understanding population-level health disparities. However, national surveys commonly rely on binary sex-based thresholds, which may introduce classification challenges when applied across diverse sex and gender classifications.
METHODS: Data from the 2019-2023 Behavioral Risk Factor Surveillance System were used to examine binge drinking patterns among adults reporting transgender identity. Binge drinking was defined using standard sex-based thresholds (≥5 drinks for men, ≥4 drinks for women on a single occasion). Multivaria

BACKGROUND: Gender-affirming care (GAC) improves the mental health, quality of life, and social functioning of gender-diverse patients. In spite of this, GAC in the United States (as elsewhere) has become a major focus of debate, with several federal policy changes enacted through Executive Orders and legislation creating major restrictions on transgender (trans) Americans' ability to obtain the care they need.
OBJECTIVE: We aimed to understand how trans Americans are responding to the anticipated changes in their access to GAC and to provide healthcare professionals with strategies to suppor

Black transgender women in the United States (US) report a significant burden of minority stress contributing to high post-traumatic stress disorder (PTSD) and depressive symptom severity and suicidality. Literature suggests Black Americans are less likely than White peers to report suicidal ideation (SI) but more likely to attempt suicide in the last year. Limited research exists to identify specific symptoms that may indicate increased risk of suicidality among Black transgender women who do not disclose SI-limiting clinicians' ability to identify those at risk of suicidality. Our study aime

IMPORTANCE: Acne commonly affects transgender individuals prescribed gender-affirming hormone therapy, yet population-level incidence data remain limited.
OBJECTIVE: To compare the incidence of acne and moderate to severe acne in transgender individuals, including those initiating gender-affirming hormone therapy, with matched cisgender individuals.
DESIGN, SETTING, AND PARTICIPANTS: A retrospective matched cohort study of electronic health record data was carried out across 4 Kaiser Permanente health plan regions. Index dates were the earliest documentation of transgender status, ranging fr

BACKGROUND: The HIV Prevention Trials Network (HPTN) 083 Trial demonstrated the superiority of long-acting, injectable cabotegravir (CAB) over daily oral tenofovir disoproxil fumarate/emtricitabine (TDF/FTC) for HIV pre-exposure prophylaxis among cisgender men (MSM) and transgender women (TGW) who have sex with men. Plasma CAB concentrations associated with HIV protection in humans are unknown.
METHODS: We conducted a nested case-control study to investigate the association between plasma CAB concentrations and HIV risk. Plasma CAB concentrations were estimated for participants with confirmed

BACKGROUND: Oral HIV pre-exposure prophylaxis (PrEP) uptake in the Asia-Pacific region is sub-optimal with only 2% of the 2025 goal of PrEP use achieved in 2023. There is no agreed-on measure of unmet PrEP need, and no assessment of unmet need across the Asia-Pacific.
METHODS: We surveyed men who have sex with men (MSM) and transgender women (TGW) in 15 Asian countries and Australia (16 total). We defined unmet PrEP need as those who had: (1) heard of and wanted to take PrEP, but never taken it; (2) never heard of PrEP but had HIV risk factors; (3) taken oral PrEP but preferred a different Pr

Transgender and gender-expansive young people, ages 13-24 years, experience disproportionate HIV risk yet are among those with the lowest US PrEP uptake rates (< 10%). Factors influencing PrEP outcomes for this population are poorly understood. This study examines the effects of gender minority stressors, gender affirmation, and heavy substance use on their PrEP outcomes using data from the CDC's 2018 START study (N = 972). A conceptual model integrating the gender minority stress and gender affirmation models was developed, mapping relevant START items onto it. Structural equation modeling (M

INTRODUCTION: This study examined an intersectional perspective on alcohol use disparities within transgender and nonbinary (TGNB) adults.
METHODS: We examined the data from the 2015 U.S. Transgender Survey (N = 27,715), a cross-sectional, nationwide survey of TGNB adults. The number of drinking days and the number of binge-drinking days were primary outcomes. Analyses followed a multilevel analysis of individual heterogeneity and discriminatory accuracy approach to examine alcohol disparities across gender identities (transgender, nonbinary and crossdresser) and intersections with race/ethni

INTRODUCTION: Little is known about differences in HIV risk for trans women by partner gender, particularly with respect to social determinants of health and partner-level factors that affect behavior. We examined differences in demographic, social determinants, and HIV-related risk behaviors for trans women with cisgender men and trans women sexual partners.
MATERIALS AND METHODS: Data are from a cross-sectional survey of trans women and their sexual partners conducted between April 2020 and January 2021. Interviews were held remotely via videoconference during shelter-in-place ordinances du

INTRODUCTION: Internal medicine (IM) residents often lack the knowledge, comfort, and competency to care for gender and sexual diverse (GSD) patients, contributing to health disparities. We developed a practical, evidence-based framework providing residents with a systematic, stepwise approach to addressing GSD health needs.
METHODS: The curriculum was implemented at an urban safety-net hospital for 46 IM residents. Two 2-hour sessions utilized a case-based, small-group, interactive scenario of a transgender woman requiring screening and treatment of sexually transmitted illnesses (STIs), pre

Transgender and gender non-conforming (TGNC) individuals encounter distinct challenges that can significantly impact their psychological well-being. Among these challenges, internalized transphobia -where individuals internalize societal stigma related to their gender identity-emerges as a critical stressor potentially exacerbating psychological distress. This study aims to explore these mechanisms by examining how social support and gender identity disclosure indirectly affect the relationship between internalized transphobia and psychological distress among TGNC emerging adults. Data for thi

OBJECTIVES: The present study examines the quality of life (QoL) of transgender and gender-diverse individuals receiving versus not receiving gender-affirming hormone therapy (GAHT) in those assigned male at birth (AMAB) and assigned female at birth (AFAB). It also explores the relationship between QoL and concentrations of oestradiol and testosterone.
METHODS: This cross-sectional study used the WHOQOL-BREF questionnaire to assess QoL. Participants were categorised into four groups based on assigned sex at birth (AMAB or AFAB) and GAHT status, with non-GAHT participants serving as controls.

BACKGROUND: Gender-affirming hormone therapy is medically necessary for many transgender, non-binary, and gender-diverse (trans) individuals and might improve HIV clinical outcomes for trans people. This study evaluated gender-affirming hormone therapy delivered in primary care as an intervention to improve HIV outcomes for trans adults in the USA.
METHODS: LEGACY is a longitudinal cohort of trans adult participants receiving primary care at two US federally qualified health centres: Fenway Health (Boston, MA, USA) and Callen-Lorde Community Health Center (New York, NY, USA). Eligibility crit
