Argon laser peripheral iridoplasty for topiramate-induced bilateral acute angle closure.

PURPOSE: To report a case of a topiramate-induced bilateral acute angle closure that was treated successfully by argon laser peripheral iridoplasty (ALPI). METHODS: A 59-year-old healthy woman taking oral topiramate developed bilateral acute angle closure. B-scan ultrasonography, ultrasound biomicroscopy, and slit-lamp adapted ocular coherence tomography were performed. RESULTS: The patient had shallow anterior chambers and total angle closure for 360 degrees OU. Ocular imaging showed circumferential ciliochoroidal effusions with anterior displacement of the iris-lens diaphragm. ALPI relieve

Clinical features distinguishing angle closure from pseudoplateau versus plateau iris.

PURPOSE: To evaluate clinical aspects of patients with the diagnosis of plateau iris (PI) or pseudoplateau iris (PPI) made by ultrasound biomicroscopy (UBM) in order to determine if there are any clinical factors that can help differentiate between these two entities. METHOD: A retrospective cohort of consecutive UBM patients with the diagnosis of PI or PPI. The diagnosis of PI was based on an anteriorly positioned ciliary body that abutted the peripheral iris, a narrow (<10 degrees) or closed angle for at least 180 degrees, and the anterior portion of the iris positioned anterior to scleral

Hypotonous malignant glaucoma: aqueous misdirection with low intraocular pressure.

Two patients who underwent trabeculectomy and postoperatively manifested axially shallow anterior chamber associated with hypotony but without choroidal effusion are described. The first patient was treated conservatively with topical cycloplegics. The second patient was treated with pars plana anterior vitrectomy. The interventions resulted in deepening of the anterior chamber and posterior rotation of the ciliary body to a natural position as observed by ultrasound biomicroscopy. Intraocular pressure remained low in both cases. These cases demonstrate that hypotony and axial shallowing of th

Long ciliary processes with no ciliary sulcus and appositional angle closure assessed by ultrasound biomicroscopy.

PURPOSE: Nonpupil block mechanisms and appositional angle closure after laser iridotomy (LI) have been reported as common findings in Asians. We evaluated the presence of these findings in a cohort of Brazilian patients using ultrasound biomicroscopy (UBM). METHODS: This observational case-control study included 22 open angle eyes and 31 eyes with occludable angles on gonioscopy (defined by 2 examiners). UBM radial scans through a typical ciliary process were obtained in both light and dark conditions, at 6 and 12-o'clock positions. Long ciliary processes with no ciliary sulcus were determine

Effects of Schlemm's Canal Suture Implantation Surgery and Pilocarpine Eye Drops on Trabecular Meshwork Pulsatile Motion.

The trabecular meshwork is an important structure in the outflow pathway of aqueous humor, and its movement ability directly affects the resistance of aqueous humor outflow, thereby affecting the steady state of intraocular pressure (IOP). (1) Objective: The purpose of this study was to preliminarily estimate the effects of pilocarpine eye drops and trabeculotomy tunneling trabeculoplasty (3T) on trabecular meshwork (TM) pulsatile motion via phase-sensitive optical coherence tomography (Phs-OCT). (2) Method: In a prospective single-arm study, we mainly recruited patients with primary open-angl

[Anterior eye segment damage in coronavirus infection (COVID-19)].

Conjunctivitis may appear as the first symptom of the coronavirus infection (COVID-19). In isolated cases, the lesion of the conjunctiva evokes a systemic infectious process. Currently, the conjunctiva is not considered as an area of long-term reproduction of coronavirus, and its damage is caused by hyperproduction of pro-inflammatory cytokines (especially IL-6); development of iridocyclitis and keratoconjunctivitis is also possible. Most often, local corticosteroids are used to treat these processes, although their use requires caution due to the risk of activating secondary infection (herpet

Blindness from misuse of over-the-counter eye medications.

Blindness from misuse of over-the-counter (OTC) eye drop preparations is reported. This is a previously unreported category of visual loss. Four eyes were blinded in three patients who used OTC preparations when they suffered angle-closure glaucoma. Another eye suffered a reduction in vision from 20/20 to 20/200 when the patient treated a subluxed lens implant with OTC preparations for three months. The motivating factors for self treatment by individuals are discussed. These include advertising, lack of education, and a desire to reduce the cost of health care. Since the common OTC eye drops

Evaluation of the Painful Eye.

Eye pain is a common presentation in outpatient, urgent care, and emergency settings. Causes range from benign to vision-threatening. Family physicians should promptly identify red-flag features that require urgent or emergent ophthalmology referral. A painful eye with vision loss, severe photophobia, proptosis, hyphema, or corneal ulceration is a medical emergency. Patient history should include onset, associated vision changes, contact lens use, trauma, chemical exposure, systemic symptoms, and rheumatologic disease. Physical examination should include evaluation of visual acuity (eg, using

Venom Ophthalmia and Ocular Complications Caused by Snake Venom.

Little is known about the detailed clinical description, pathophysiology, and efficacy of treatments for ocular envenoming (venom ophthalmia) caused by venom of the spitting elapid and other snakes, as well as ocular complications caused by snake venom injection. In this paper, we review clinical information of case reports regarding venom ophthalmia and snake venom injection with associated ocular injuries in Asia, Africa, and the United States. We also review the literature of snake venom such as their compositions, properties, and toxic effects. Based on the available clinical information a

Systemic effects of medications used to treat glaucoma.

Medications used to treat glaucoma can have clinically important systemic effects in some patients; these effects may not be recognized in elderly patients who have chronic medical problems and who are taking several systemic medications. Beta-blocking ophthalmic agents are generally safe, but can be absorbed systemically to induce bronchospasm, worsen heart block, decompensate congestive heart failure, or create central nervous system effects in some patients. Reports of adverse systemic effects from miotics, such as pilocarpine, are rare, although cardiovascular decompensation has been seen

Insertion of a foldable hydrophobic IOL through the trabeculectomy fistula in cases with microincision cataract surgery combined with trabeculectomy.

BACKGROUND: The use of conventional foldable hydrophobic intraocular lenses (IOLs) in microincision cataract surgery (MICS) currently requires wound enlargement. We describe a combined surgical technique of MICS and trabeculectomy with insertion of a foldable IOL through the trabeculectomy fistula. METHODS: After completion of MICS through two side port incisions, a 3.2 mm keratome is used to enter the anterior chamber under the previously outlined scleral flap. An Acrysof multi piece IOL (Alcon labs, Fort Worth, Tx) is inserted into the capsular bag through this incision. The scleral flap is

Accommodative Demand and Near Addition in Early-Onset Primary Angle-Closure Disease: A Comparative Study.

PURPOSE: Early-onset primary angle-closure disease (EOPACD), defined by onset before 40 years of age, is associated with a thicker and more anteriorly positioned crystalline lens and a crowded anterior segment, which may alter accommodative performance. This study aimed to compare accommodative function and near-addition requirements in EOPACD with those in age-matched healthy controls, and to assess the relationship between anterior segment biometry and accommodative outcomes. METHODS: This prospective case-control study compared unrelated EOPACD patients with healthy controls. Multiple clin

Determination of iris thickness development in children using swept-source anterior-segment optical coherence tomography.

PURPOSE: The uvea comprises the iris, ciliary body, and choroid. However, the development of the anterior part (iris and ciliary body) in children is not yet fully elucidated. We investigated the iris thickness (IT) in children using swept-source anterior-segment optical coherence tomography (ASOCT). METHODS: In this retrospective, clinic-based study, we enrolled 41 children (mean ± standard deviation: 6.8 ± 3.3 years; range: 3-16; 17 males) with normal or mild refractive error. Horizontal scanning images of swept-source ASOCT were analyzed in temporal and nasal angle areas. The ITs at 1 and

Unilateral Acute Closed-Angle Glaucoma After Elective Lumbar Surgery Reveals Multiple Intracranial Aneurysms. A Case Report and Discussion on Workup of Differential Diagnoses.

The purpose of our paper is to present a case of a rare complication of posterior lumbar surgery. Our patient presented for elective lumbar decompression, which was complicated by durotomy. She then developed sudden headache and right eye pain once upright on postoperative day 2. Then on postoperative day 3, she developed a dilated nonreactive pupil with extraocular movements intact. A computed tomography scan of the head was negative for subarachnoid hemorrhage. Magnetic resonance angiography showed a possible right posterior communicating artery aneurysm. She was transferred to a tertiary ce
