Fluid and electrolyte intake during the first week of life in preterm infants receiving parenteral nutrition according current guidelines.

There is growing evidence that early water and electrolyte homeostasis may be influenced by energy balance in preterm infants. Some fluid and electrolyte disturbances of postnatal life could be in part promoted by the catabolism due to withheld amino acid and energy supply after birth. According to current guidelines parenteral nutrition with relatively high protein and lipid needs is commenced on day one. By turning the nitrogen balance from negative to zero or even positive, amino acid administration could also minimize the occurrence of water and ions disturbances after birth especially in

Which is the ideal target for preterm growth?

In the last two decades the improved survival of very low birth weight (VLBW) and extremely low birth weight (ELBW) infants has underscored the problem of postnatal growth failure of these subjects. Notwithstanding the evident improvements in the general management of these infants during the hospital stay, most of them acquire a significant extrauterine growth restriction (EUGR). Frequent illnesses, medical and surgical therapies, feeding intolerance and, most of all, inadequacy of nutrient deliveries are responsible for the great part of this growth failure. However other factors, such as ge

Interesting neuroimaging findings in a child with hyperlipidemia: a case report.

INTRODUCTION: Neurological manifestations are rare in hyperlipidemia; however, extreme hypertriglyceridemia may cause “milky” blood and cerebrospinal fluid (CSF) and may compromise the central nervous system. We report an infant with familial hyperlipidemia who presented with seizures and unusual neuroimaging findings, including fat deposition in the cerebral vasculature and intracranial xanthomas. CASE DESCRIPTION: A 2-month-old boy, previously healthy, presented with fever, followed by a focal seizure. His parents are consanguineous. His perinatal history and early development are unremarkab

Oral iron-induced stunting is associated with increased Christensenella abundance in a piglet model for human infants.

PURPOSE: Iron deficiency anaemia (IDA) affects around 40% of < 5-year-olds globally. Unfortunately, IDA treatment by oral iron intervention often causes diarrhoea, thought to be linked to iron-induced changes in the gut microbiota. Piglets are valuable preclinical models for human infants since they share many physiological characteristics, have similar gut microbial populations and, if left untreated with iron, reliably develop IDA. We hypothesised that IDA and different forms of iron supplementation (oral and injection) would have differential effects on the gut microbiota that would consequ

Infant Botulism and Carbohydrate Blood Group Antigens: Preliminary Results from a Discordant Twin Study.

OBJECTIVE: We investigated if specific carbohydrate blood group antigens were associated with infant botulism (IB) among discordant twins and triplets. STUDY DESIGN: Infants in the US and Canada from 2015 to 2022 who had laboratory-confirmed IB; were members of a multiple gestation birth; and had unaffected sibling(s), were eligible to participate. Blood specimens were tested for ABO, Lewis, H, and P1 carbohydrate blood antigens. FUT2 and FUT3 genes were sequenced to determine secretor and Lewis antigen status, respectively. In addition, characteristics of IB cases in the study were compared

Higher total ultrafiltration volume during cardiopulmonary bypass-assisted infant cardiac surgery is associated with acute kidney injury and fluid overload.

BACKGROUND: Ultrafiltration (UF) is used for fluid removal during and after infant cardiopulmonary bypass (CPB) surgery to reduce fluid overload. Excessive UF may have the opposite of its intended effect, resulting in acute kidney injury (AKI), oliganuria, and fluid retention. METHODS: This is a single-center, retrospective review of infants treated with conventional and/or modified UF during CPB surgery. UF volume was indexed to weight. AKI was defined using serum creatinine "Kidney Disease Improving Global Outcome (KDIGO)" criteria. Fluid balance was defined according to: [Formula: see text

Dietary Considerations in Tyrosinemia Type I.

Since the introduction of 2-(2 nitro-4-3 trifluoro-methylbenzoyl)-1, 3-cyclohexanedione (NTBC), life expectancy of HT1 patients greatly improved. However, due to treatment with NTBC, tyrosine concentrations greatly increase. As a consequence to possible neurocognitive problems, the main objective of dietary therapy in HT1 is to provide adequate nutrition allowing normal growth and development while strictly controlling tyrosine levels in blood (and tissues). Although no well-defined target levels exist, tyrosine concentrations below 400 μmol/L are considered to be safe. To achieve this aim a d

The role of preoperative biometry in selecting initial contact lens power in the Infant Aphakia Treatment Study.

PURPOSE: To investigate the role of preoperative biometry for selecting initial contact lens power. METHODS: Patients randomized to receive contact lenses in the Infant Aphakia Treatment Study (IATS) were retrospectively analyzed. Inclusion criteria were availability of both a preoperative immersion axial length measurement and a 1-month postoperative refractive value. The target contact lens power for distance was determined using 1-month postoperative spherical equivalent refraction (after adjusting for a vertex distance) over the known contact lens power. We compared targeted contact lens

Dietary Bovine Lactoferrin Reduces Staphylococcus aureus in the Tissues and Modulates the Immune Response in Piglets Systemically Infected with S. aureus.

BACKGROUND: Bovine lactoferrin (bLf) reduces Staphylococcus aureus infection in premature infants and promotes the growth of Bifidobacterium infantis, a predominant infant gut species. We hypothesized that bLf in combination with B. infantis would reduce the severity of systemic S. aureus infection. OBJECTIVE: The aim was to determine the effects of oral administration of bLf and B. infantis on the course of systemic S. aureus infection. METHODS: Colostrum-deprived piglets were fed formulas containing 4 g whey/L (CON group) or bLf (LF group). One-half of the piglets in each group were gavage

Inpatient and outpatient treatment for acute malnutrition in infants under 6 months; a qualitative study from Senegal.

BACKGROUND: Treatment of acute malnutrition in infants under 6 months is a relevant topic regarding the global problem of maternal and child malnutrition. While treatment for older age groups has shifted more towards an outpatient, community based approach, young infants are mostly treated in hospital. This study aims to describe barriers and facilitators for outpatient and inpatient treatment of malnourished infants under 6 months in Senegal. METHODS: This qualitative descriptive study uses in-depth interviews with health workers and focus group discussions with mothers of malnourished infan

Avoidance of bottles during the establishment of breastfeeds in preterm infants.

BACKGROUND: Preterm infants often start milk feeds by gavage tube. As they mature, sucking feeds are gradually introduced. Women with preterm infants may not always be in hospital to breastfeed their baby and need an alternative approach to feeding. Most commonly, milk (expressed breast milk or formula) is given by bottle. Whether using bottles during establishment of breastfeeds is detrimental to breastfeeding success is a topic of ongoing debate. OBJECTIVES: To identify the effects of avoidance of bottle feeds during establishment of breastfeeding on the likelihood of successful breastfeedi

Feeding the Larger Low-Birthweight Infant in a Resource-Poor Environment.

The high low birthweight prevalence in resource-poor countries (16.5%) places a burden on overstretched resources. Labor ward must have written guidelines to triage these infants for optimal nutritional support to the special care nursery (SCN; 1,500-1,800 g and <34 weeks) and postnatal ward (PW; >1,800 g and ≥ 34 weeks). Separation of mother and infant should be prevented. Initiating breastfeeding and kangaroo mother care (KMC) in labor ward by skilled nurses in the latter group is a priority and continues in the PW. SCN infants receive an intravenous 10% glucose-electrolyte solution and, if

A case of fetal midgut volvulus and jejunal atresia: nutritional support and maintenance of mucosal function and integrity.

Fetal midgut volvulus is a quite rare disease. Here, we report a case of a preterm newborn with fetal peritonitis, jejunal atresia with volvulus of post-atresic small bowel since about 30 cm from ileo-cecal valve (ICV) followed by a not-used microileus and microcolon. The surgical intervention consisted in the resection of volvulated necrotic small bowel followed by primary anastomosis. After surgery, total parenteral nutrition (TPN) has been started since 16(th) of post-operative days when enteral feeding (EN) was administered with a lactose-free formula containing hydrolyzed protein and medi
