Detecting developmental dysplasia of the hip.

Recent guidelines from the American Academy of Pediatrics mandate the universal screening of all newborns for developmental hip dysplasia, in an effort to promote prompt recognition and treatment of this condition. The effectiveness of universal screening hinges on the appropriate education, training, and experience of health care providers. This article reviews the normal embryologic, natal, and postnatal development of the hip. Historical risk factors that may have an impact on this developmental sequence are outlined. A step-by-step guide to the examination of the hip is offered, along with

Assessment of infants with peripherally inserted central catheters: Part 1. Detecting the most frequently occurring complications.

Inserting, maintaining, and monitoring vascular access are integral components of neonatal care. Advances in vascular access technology have led to the insertion of peripherally inserted central catheters (PICC) to provide stable venous access for early and aggressive parenteral nutrition. Medications that are irritating or damaging, or those with a high osmolality or a nonphysiologic pH, can also be safely administered into the central venous system. The need for repeated peripheral intravenous attempts, as well as the associated pain and physiologic instability, are virtually eliminated once

Examination of the newborn foot: positional and structural abnormalities.

Newborn feet come in a variety of shapes and sizes. The foot is malleable, making it susceptible to compression and deformation from intrauterine positioning. Clinicians frequently question whether variations represent deformations, that is, an alteration in the shape and contour of a normally formed foot, as opposed to a true structural malformation. Distinguishing between a temporary positional deformity and a more serious structural foot malformation is challenging and requires a clear understanding of the anatomy of the foot, its complex embryologic development, and the impact of environme

Slow advancement of enteral feed volumes to prevent necrotising enterocolitis in very low birth weight infants.

BACKGROUND: Early enteral feeding practices are potentially modifiable risk factors for necrotising enterocolitis (NEC) in very preterm or very low birth weight (VLBW) infants. Observational studies suggest that conservative feeding regimens, including slowly advancing enteral feed volumes, reduce the risk of NEC. However, slow feed advancement may delay establishment of full enteral feeding and may be associated with metabolic and infectious morbidities secondary to prolonged exposure to parenteral nutrition. OBJECTIVES: To determine effects of slow rates of enteral feed advancement on the i

Upper body central venous catheters in pediatric cardiac surgery.

BACKGROUND: A central venous catheter located in the jugular or subclavian vein provides rapid, reliable vascular access for pediatric heart surgery. However, intravascular catheters are associated with vessel injury. Stenosis or thrombosis of central veins in the upper body can lead to 'superior vena cava syndrome' with markedly elevated venous pressures in the head and neck, causing facial swelling and headaches. This complication may be especially serious for patients with superior cavopulmonary (Glenn) or total cavopulmonary (Fontan) circulation. The authors hypothesized that upper body ce

[Interaction involving the thymus and the hypothalamus-pituitary axis, immunomodulation by hormones].

Perfectly projected and impeccably created, the endocrine system precisely regulates the most delicate immune processes. The immune and neuroendocrine systems are two essential physiological components of mammalian organisms important for protection from the infection and disease on one hand, and on the other, for regulation of metabolism and other physiological activities; namely, the evidence has been found indicating that there is active and dynamic collaboration of these systems in the execution of their designated functions. These interactions occur at many stages of embryonic and neonata

The Effect of Kangaroo Care Provided by Fathers in the Neonatal Intensive Care Unit on Paternal Stress and Neonatal Comfort: A Pilot Randomized Controlled Trial.

BACKGROUND: Fathers of infants hospitalized in neonatal intensive care units (NICUs) frequently experience high levels of stress and limited involvement in direct infant care. While kangaroo care is well-established for mothers, evidence regarding father-provided kangaroo care and its effects on both paternal stress and neonatal comfort remains limited. PURPOSE: To examine the effects of kangaroo care provided by fathers on paternal stress levels and neonatal comfort in an NICU setting. METHODS: This pilot randomized controlled trial was conducted from March to December 2024 in a city hospit

Factors Influencing Breastfeeding Knowledge and Attitudes Among Parents of Moderate-To-Late Preterm Infants: A Cross-Sectional Study.

BACKGROUND: Prematurity constitutes a major threat to perinatal health, with moderate-to-late preterm infants accounting for over 80% of all preterm births in China. This places significant challenges on breastfeeding for the affected families. As the optimal enteral nutrition for preterm infants, human milk reduces the incidence of necrotizing enterocolitis and fulfills their growth needs. PURPOSE: The aim of this study was to explore factors associated with breastfeeding knowledge and attitudes among parents of moderate-to-late preterm infants in the context of mother-infant separation. Empowering Parents Through Targeted Journaling to Promote Engagement and Reduce Stress.

BACKGROUND: Parents of preterm infants admitted to the neonatal intensive care unit (NICU) experience significant and lasting psychological stress, which can negatively impact both their own well-being and their child's development. To address the need for accessible, low-cost interventions that promote engagement and emotional support, we developed a structured parental journal aimed at enhancing bonding, tracking medical progress, and reducing stress during the NICU stay. PURPOSE: We hypothesized that implementing a structured journal will enhance parental mental well-being and engagement.

Interprofessional Perspectives on Communication Quality in the Neonatal Intensive Care Unit: A Qualitative Study.

BACKGROUND: Communication with parents in the Neonatal Intensive Care Unit (NICU) setting is both important and uniquely challenging. Prior research has established that communication quality directly effects patient and parent outcomes in the NICU. A range of provider types comprise the interprofessional NICU team, each bringing a unique perspective. Yet, there is limited literature reflecting these diverse points of view. Although NICU staff often endorse the sentiment that high-quality communication is essential for excellent patient care, it is not clear that providers in different roles d

Thermal Stability of Extremely Preterm Infants During Early Kangaroo Mother Care in the First 5 Days of Life.

BACKGROUND: Extremely preterm infants are highly vulnerable to hypothermia because of immature thermoregulation and increased heat loss. Kangaroo mother care (KMC) is an evidence-based intervention associated with improved thermal stability, but evidence in extremely preterm infants during the first days of life remains limited. PURPOSE: To evaluate axillary temperature 60 minutes after initiating KMC in infants <28 weeks' gestation during the first 5 days of life, analyze temperature trends across sessions, and identify factors associated with thermal stability. METHODS: This prospective ob
