Clinical performance of lateralized versus non-lateralized reverse shoulder arthroplasty: a prospective randomized study.

BACKGROUND: Reverse shoulder arthroplasty (RSA) is an established therapeutic option in the treatment of cuff tear arthropathy (CTA). Recent studies have described a positive effect of lateralization of the center of rotation, especially on postoperative rotational function. The goal of this study is to compare the outcome of non-lateralized versus lateralized RSA with particular regard to rotational function in patients with CTA. METHODS: Thirty-four patients underwent implantation of RSA either with lateralization (n = 17) or without lateralization (n = 17) by use of a 1-cm autologous bone

Rotator cuff tears: should abduction and external rotation (ABER) positioning be performed before image acquisition? A CT arthrography study.

OBJECTIVE: To evaluate the impact of abduction and external rotation (ABER) positioning performed before image acquisition on the assessment of rotator cuff tears. METHODS: Twenty-seven consecutive patients with clinically suspected rotator cuff tears underwent an initial CT arthrogram of the shoulder in neutral position, immediately followed by temporary ABER positioning, before a second CT acquisition in neutral position. Two observers blinded to potential pre-procedure ABER positioning independently analysed the randomly distributed images. Lesions were classified into partial-thickness (P

Return to sports after arthroscopic anterior stabilization in patients aged younger than 25 years.

PURPOSE: To prospectively evaluate the return-to-sports rates after arthroscopic anterior stabilization in patients aged younger than 25 years. METHODS: Fifty-eight patients underwent arthroscopic capsulolabral repair for isolated anterior instability. The mean age at the time of surgery was 19.5 years (range, 12 to 24 years). At a mean follow-up of 27 months (range, 20 to 32 months), 53 patients (42 male and 11 female patients) were assessed with American Shoulder and Elbow Surgeons, L'Insalata, and visual analog scale scores, as well as physical examination. The rate of return to sports and

The effect of rotator cuff tears on reaction forces at the glenohumeral joint.

The rotator cuff muscles maintain glenohumeral stability by compressing the humeral head into the glenoid. Disruption of the rotator cuff compromises concavity compression and can directly affect the loads on the glenohumeral joint. The purpose of this study was to quantify the effect of rotator cuff tears on the magnitude and direction of glenohumeral joint reaction forces during active shoulder abduction in the scapular plane using nine cadaveric upper extremities. Motion of the full upper extremity was simulated using a dynamic shoulder testing apparatus. Glenohumeral joint reaction forces

Special shoulder problems in the throwing athlete: pathology, diagnosis, and nonoperative management.

Shoulder problems in the overhead and throwing athlete can result in rotator cuff tendinitis. Biomechanically, there is a delicate balance between mobility and stability of the shoulder complex. Repetitive overhead stressful motion can lead to overuse tendinitis, subtle instability, labral changes, and eventual fiber failure of the rotator cuff. Secondary impingement symptoms are commonly present. An accurate diagnosis along with an early and aggressive rehabilitation program is essential in the treatment of these athletes.

Instability Severity Index Score Does Not Predict the Risk of Shoulder Dislocation after a First Episode Treated Conservatively.

The first purpose of this study was to verify the association between Instability Severity Index Score (ISIS) and Recurrent Shoulder Dislocation (RSD) after a first episode treated conservatively. The second aim is to identify the risk factors associated with RSD after a primary acute shoulder anterior dislocation treated conservatively. A total of 111 patients with first traumatic anterior shoulder dislocation treated at a single trauma centre between January 2014 and March 2016 were enrolled. The main predictive variables of risk factors and the ISIS score were calculated. Among the 85 patie

Chronic unreduced dislocations of the glenohumeral joint: imaging strategy and pathologic correlation.

Although the evaluation of acute or recurrent dislocations of the glenohumeral joint (GHJ) using routine radiography, specialized views, arthrography, and computed tomography (CT) has been extensively studied, methods of evaluation and osseous pathology of chronic unreduced GHJ dislocations are not well documented. In order to define such methods and pathology, we studied nine patients with chronic (greater than 6 months) unreduced anterior, posterior, superior, or central dislocations as well as one patient with a chronic rotator cuff injury; and seven scapular specimens, six with matching hu

A less invasive surgery for rotator cuff tear: mini-open repair.

As a less invasive surgery of the shoulder, the mini-open repair was carried out from March 1997 through December 1998 in 22 patients (mini-open repair group) for all types of rotator cuff tears, except massive tears. A transacromial skin incision about 3 cm long was made. The anterior deltoid was split along its muscle fibers, and an anterior acromioplasty was accomplished by using a specially designed bone rasp. The rotator cuff was repaired with use of the McLaughlin technique. The results were compared with those obtained in 36 patients subjected to conventional open surgery from April 199

Repair of massive rotator cuff tears in patients older than 70 years.

Major ruptures of the rotator cuff are common in elderly patients, and their management has been a challenge for orthopaedic surgeons. This study was undertaken to evaluate the results obtained in patients older than 70 years who were treated with open surgical repair for massive ruptures of the rotator cuff tendons and to correlate the functional outcomes with the quality of the repairs. Between January 1991 and November 1994, 69 patients older than 70 years of age underwent rotator cuff repairs of massive tears with a minimum follow-up of 2 years (mean of 3 years). These patients were evalua

Tenodesis of the long head of biceps brachii in the painful shoulder: improving results in the long term.

Fifteen shoulders of 14 patients with a keyhole tenodesis of the long head of the biceps were reviewed at an average follow-up of 7 years (3 years, 1 month to 13 years, 2 months). In 13 cases additional shoulder disease was noted during the operation. Eight patients had undergone rotator cuff decompression before the reference biceps tenodesis was performed. Eight (53%) cases achieved an excellent result; one was rated as good, four were rated as fair, and two had failures. Seven shoulders had an improved result from short to long term, and only two deteriorated. An upward migration of the hum

Magnetic resonance imaging for supraspinatus muscle atrophy after cuff repair.

Between 1998 to 2000, the shoulders of 17 patients with rotator cuff tears were examined using magnetic resonance imaging preoperatively, and 6 and 12 months after open surgical repair. To assess the course of the supraspinatus muscle atrophy, the easily reproducible magnetic resonance imaging parameters occupation ratio and tangent sign in the Y-shaped view of the supraspinous fossa were evaluated and compared with the clinical examination. Occupation ratio is a quantitative parameter that reflects the area ratio, expressed as a percentage of the supraspinatus muscle belly to the supraspinous

Arthroscopic findings in the overhand throwing athlete: evidence for posterior internal impingement of the rotator cuff.

The purpose of this article is to describe the outcome of an arthroscopic examination and the pathology in symptomatic shoulders of 41 professional overhand throwing athletes. With the arm in the position of the relocation test, 100% of the subjects had either contact between the rotator cuff undersurface and the posterosuperior glenoid rim or osteochondral lesions. Other key findings included undersurface cuff fraying in 93%, posterosuperior labral fraying in 88%, and anterior labral fraying in 36% of the subjects. This study supports the concept of impingement of the posterior cuff undersurf

Incongruity of the scapulohumeral arch: an indirect sign of posterior subluxation of the humeral head in primary glenohumeral osteoarthritis.

BACKGROUND: In symptomatic glenohumeral osteoarthritis, arthroplasty is the gold standard to restore a mobile and painless shoulder. The kind of arthroplasty chosen is mainly based on the evaluation of the rotator cuff and the type of glenoid. The aim of this study was to analyze the scapulohumeral arch in primary glenohumeral osteoarthritis (PGHOA) with an uninjured cuff and to determine whether posterior humeral subluxation influences the Moloney line, which corresponds to an intact scapulohumeral arch. MATERIALS AND METHODS: Between 2017 and 2020, 58 anatomic total shoulder arthroplasties
