Oral PrEP continuation rates among key populations in Nigeria: a retrospective cohort study of a large-scale HIV prevention programme.

INTRODUCTION: Pre-exposure prophylaxis (PrEP) is an effective HIV prevention strategy, but its impact is limited by low uptake and poor continuation rates. This study investigates oral PrEP continuation among key populations (KPs) enrolled in an HIV prevention programme across seven states in Nigeria, representing one of the largest longitudinal PrEP continuation studies in Sub-Saharan Africa to date. METHODS: We analysed data from 43 788 clients who initiated daily oral PrEP between January 2020 and March 2023. The sample comprised female sex workers (20 574, 47.0%), men who have sex with me

Qualitative client perspectives on PrEP well: a community-led PrEP implementation project with structural supports at a transgender and nonbinary community center.

Although pre-exposure prophylaxis (PrEP) is an effective way to prevent HIV infection, PrEP uptake and adherence remain low in transgender and nonbinary (TGNB) communities. This qualitative study explored perspectives on the community-developed PrEP Well program that implemented PrEP services in a TGNB community center. Clients completed an interview approximately 90 days after program enrollment about factors that influenced their participation and suggestions for program improvement. Interviews were audio-recorded, transcribed, translated to English (if conducted in Spanish), de-identified,

Doxycycline post-exposure prophylaxis as prevention of STIs - the golden bullet?

INTRODUCTION: Doxycycline post-exposure prophylaxis ('doxyPEP') is an emerging strategy to prevent bacterial sexually transmitted infections (STIs). Users take 200 mg of doxycycline up to 72 hours after condomless sex, and data from randomized controlled trials and real-world implementation have shown doxyPEP to be effective in preventing syphilis, chlamydia, and to a lesser extent gonorrhea, in gay, bisexual, and other men-who-have-sex-with-men (GBMSM) and transgender women. AREAS COVERED: We discuss the potential benefits, risks, and important considerations for doxyPEP implementation, draw

Uterine changes in transgender men receiving testosterone therapy.

OBJECTIVES: Despite regular gender-affirming hormone therapy (GAHT), the presence of uterine bleeding can occur occasionally and cause profound discomfort. This study aimed to evaluate the histologic features and immunohistochemical expression of estrogen (ER), progesterone (PR), and androgen receptors (AR) in the endometrium and myometrium of transgender men receiving testosterone therapy and relate them to clinical and hormonal characteristics. DESIGN: Retrospective cross-sectional study. METHODS: Thirty-four transgender men undergoing gender-affirming surgery were included. Clinical, soci

A compassion microintervention targeting stress reactivity among sexual minority women and transgender/nonbinary people: Study protocol for a randomized controlled trial.

OBJECTIVE: Sexual minority women (SMW) and transgender and/or nonbinary (TNB) people report more adverse health outcomes (e.g., depression, anxiety, posttraumatic stress, substance use) relative to heterosexual, cisgender people, often due to the additional stress burden from experiencing stigma. Physiological and emotional stress reactivity are mechanisms through which high cumulative stress contributes to adverse health outcomes. The randomized controlled trial (RCT) described in this study protocol examines whether a single-session compassion microintervention may attenuate physiological an

Physical, Sexual, and Intimate Partner Violence Among Transgender and Gender-Diverse Individuals.

IMPORTANCE: Transgender and gender-diverse (TGD) communities experience disproportionate levels of violence, yet due to limitations in measuring TGD identity, few state-representative estimates are available. OBJECTIVE: To assess gender identity differences in experiences of violence among adults. DESIGN, SETTING, AND PARTICIPANTS: Cross-sectional data from the 2023 California Violence Experiences (CalVEX) survey, weighted to provide state-representative estimates, was used to assess associations between gender identity and past-year experiences of violence among adults 18 years and older. Advancing Programme Science approaches to understand gaps in HIV prevention programme coverage for key populations in 12 Nigerian states: findings from the 2020 Integrated Biological and Behavioural Surveillance Survey.

INTRODUCTION: Effective HIV prevention programme coverage is necessary to achieve Nigeria's goal of ending the epidemic by 2030. Recent evidence highlights gaps in service coverage and utilization across the country. The Effective Programme Coverage framework is a Programme Science tool to optimize a programme's population-level impact by examining gaps in programme coverage using data generated through programme-embedded research and learning. We apply the framework using Integrated Biological and Behavioural Surveillance Survey (IBBSS) data from Nigeria to examine coverage of four prevention

Histological ovarian features and hormonal determinations in assigned females at birth transgender individuals according to different testosterone preparations.

INTRODUCTION: Distinct androgen formulations have been used as gender-affirming hormone treatment, but little is known about the specific changes that may occur in the ovary depending on the testosterone preparation used. The study aims to evaluate the histological modifications of the ovarian tissue and the hormonal changes after gender-affirming surgery based on the testosterone preparation employed, such as testosterone cypionate or undecanoate. DESIGN: Unicenter transversal cohort study. MATERIALS AND METHODS: Sixty transmasculine persons before and after gender-affirming surgery. A hist

Patterns of Polysubstance Use in Young Black and Latinx Sexual Minority Men and Transgender Women and Its Association with Sexual Partnership Factors: The PUSH Study.

Background: Adult studies have demonstrated that polysubstance use increases HIV acquisition risk through increased sexual behaviors, however, few studies have examined polysubstance in young Black and Latinx sexual minority men (SMM) and transgender women (TW). Methods: We used cross-sectional data from 466 young Black and Latinx SMM and TW living in four high HIV-burden US cities enrolled in the PUSH Study, a status-neutral randomized control trial to increase HIV prevention and treatment adherence. We examined data for patterns of polysubstance use comparing age differences of use and explo

Hormones, Stress, and Heart Disease in Transgender Women with HIV in LITE Plus.

INTRODUCTION: Cardiovascular disease (CVD) is a leading cause of death among transgender women and people with HIV. Exogenous estrogen and psychosocial stressors are known risk factors for CVD. Yet, few studies have used biomarkers to examine the role of stress in CVD risk among transgender women with HIV (TWHIV). This analysis examined whether stress moderates relationships between gender-affirming hormone therapy (GAHT) duration and CVD risk among TWHIV. METHODS: This cross-sectional analysis of baseline data from an observational cohort of 108 Black and Latina TWHIV in Boston, New York, an

How can HIV self-testing facilitate increased access to HIV testing among multiply marginalised populations? Perspectives from GBMSM and trans women in England and Wales.

BACKGROUND: HIV self-testing (HIVST) may facilitate marginalised populations' uptake of HIV testing, but whether the extent of marginalisation challenges individual uptake of HIVST remains under-researched. We aim to explore the perspectives of multiply marginalised cis-gender gay, bisexual and other men who have sex with men (GBMSM) and trans women on whether HIVST might increase their uptake of HIV testing. METHODS: We reanalysed qualitative interview data from SELPHI (the UK's largest HIVST randomised trial) collected between 2017 and 2020 from marginalised populations, defined as people s

TransOdara study: the challenge of integrating methods, settings and procedures during the COVID-19 pandemic in Brazil.

OBJECTIVE: Sexually transmitted infections (STIs) disproportionately affect transgender women and travestis (TGW), who often lack access to healthcare due to stigma and discrimination. We describe the approach and methodology of a study investigating the prevalence of syphilis, HIV, hepatitis A, B, and C, Neisseria gonorrhoeae (NG), Chlamydia trachomatis (CT), and human papillomavirus (HPV) among TGW, as well as their knowledge and perceptions regarding syphilis, to better inform policies to curb STIs among this vulnerable population. METHODS: TransOdara was a multicentric, cross-sectional st

Descriptive Data of Transgender and Nonbinary People's Experiences of Sexual Assault: Context, Perpetrator Characteristics, and Reporting Behaviors.

Purpose: Nearly half of transgender and nonbinary (trans/NB) people will experience sexual assault in their lifetime. Beyond prevalence, little else is known about the general context in which sexual assault occurs in this community. In addition, whether and to whom trans/NB people report these experiences is also not well understood. As such, we examined these contextual factors regarding trans/NB people's sexual assault experiences. Methods: Using a cross-sectional online survey, we assessed sexual assault prevalence rates, contextual details, and reporting behaviors in a sample of 230 trans

Delays in Accessing Healthcare Across the Gender Spectrum in the All of Us Research Program.

BACKGROUND: Gender minorities and cisgender women face barriers to healthcare access. Prior work suggests cost may represent a particular barrier to accessing care for transgender and gender diverse (TGD) individuals. OBJECTIVE: To examine odds of delaying care for any reason and, secondarily, for 7 specific reasons among TGD individuals and cisgender women compared with cisgender men in the All of Us Research Program. DESIGN: We calculated the odds of delayed care by gender identity relative to cisgender men using multivariable-adjusted logistic regression, with adjustment for age, race, in

[Advances and challenges in the transsexualizing process in the Brazilian Unified Health System].

The implementation of the Transsexualizing Process (TP) / Gender-affirming Surgeries (GAS) in the Unified Health System (SUS) was the result of social struggles by the LGBT community for sexual rights, the construction of gender identity, and bodily autonomy. The scope of this article is to analyze the advances and challenges of TP/GAS in the SUS, through a qualitative narrative literature review. In June 2022, searches were conducted in the Google Scholar, SciELO, and VHL databases to select scientific articles in Portuguese published in the last 10 years, excluding articles in foreign langua

Apocrine ductal carcinoma in situ associated with testosterone therapy in a transgender individual.

We report the first case of apocrine ductal carcinoma in situ (DCIS) in a female-to-male transgender individual on testosterone therapy (TT). The gender confirmation total mastectomy revealed 2 cm DCIS with apocrine cytology, high nuclear grade with associated calcification, and necrosis. Immunohistochemistry revealed the DCIS was negative for ER, positive for AR with HER2/neu overexpression (3+). This patient with negative screening mammography developed apocrine DCIS on TT, suggesting that gender-affirming hormone therapy may have advanced malignant transformation of atypical apocrine cells.
