PURPOSE: Calculating the power of a secondary intraocular lens (IOL) requires axial lengths and keratometry measurements. In children, these measurements often need to be done under either sedation at the time of surgery or additional sedation. We devised a method of calculating IOL power using only the aphakic refraction and a standard keratometry value.

PURPOSE: To present a case of successful laser in situ keratomileusis (LASIK) performed on a patient with residual hyperopic astigmatism and symptomatic anisometropia after previous epikeratoplasty (epikeratophakia).
METHODS: LASIK was used successfully to treat hyperopic astigmatism after regression of epikeratophakia performed 16 years earlier.
RESULTS: Visual acuity improved to 20/30 without correction, and best-corrected visual acuity was 20/25 at the 3-month postoperative visit.
CONCLUSIONS: LASIK can be used successfully as an alternative treatment to correct residual refractive error

We used the Ahmed valve for the treatment of 142 eyes with high-risk glaucoma. The glaucoma was neovascular in 74 cases, aphakic or pseudophakic in 19 eyes and occurred after previous faliure of filtering surgery in 49 eyes. Preoperative mean IOP was 31.6+/-10.4 mmHg under topical and/or systemic treatment. The success rate (IOP<22 mmHg with only topical treatment) was 56.3% with a mean follow-up of 32.4 months. Complications were fewer and milder compared with those reported for other types of drainage implants and for other types of antiglaucoma procedures.

Sensitivities at 5.5 deg off-fovea from the aphakic observer superimposed on orange, purple and blue fields were obtained to estimate short-, middle- and long-wavelength cone spectra respectively. The middle-wavelength mechanism had a visible wavelength maximum resembling a nomogram plus an UV sensitivity fitting a cis-peak. We performed homoplastic keratomileusis on 6 aphakic eyes with central corneal leucoma. The results showed that the post-operative corrected visual acuities were markedly increased to 0.3-0.5 in 3 cases and to 0.1-0.2 in another 3 cases. Since such patients were neither indicated to undergo intraocular lens implantation or penetrating keratoplasty for various reasons nor indicated for epikeratoplasty, homoplastic keratomileusis can be chosen for them.

The management of three patients with aphakic bullous keratopathy by means of pars plana vitrectomy is described. The patients had previously undergone uneventful intracapsular cataract extraction with intact vitreous face. To evaluate the efficacy of multifocal contact lenses in the correction of presbyopic symptoms we fit six young monocular aphakic patients with four different bifocal contact lenses (ACC, Tangent Streak, VFL, and Constavu). Each patient had 20/20 uncorrected vision and normal accommodation in the other eye. We evaluated both subjective and objective parameters of comfort and vision for all four lenses. We found no significant overall performance advantage among the four lenses based on comfort, visual satisfaction, visual acuity, contrast sensitivity, or wear-related corneal complications.
