INTRODUCTION: Ampullary carcinoma is typically treated with pancreaticoduodenectomy (PD). Nevertheless, performing curative resection remains remarkably challenging when the clinical course is complicated by necrotizing pancreatitis and extensive walled-off necrosis (WON). Although resection is occasionally performed after mild pancreatitis, few reports describe curative surgery following severe necrosis requiring necrosectomy. We report a rare case managed with necrosectomy for extensive WON, followed by PD after regression of the necrotic area.

BACKGROUND: Few studies have compared the oncological benefit of laparoscopic (LPD) and open pancreatoduodenectomy (OPD) for ampullary carcinoma. The aim of this study was to compare the oncological results of these two approaches.
METHODS: Between 2011 and 2020, 103 patients who underwent PD for ampullary carcinoma, including 31 LPD and 72 OPD, were retrospectively analyzed. Patients were matched on a 1:2 basis for age, sex, body mass index, American Society of Anaesthesiologists score, and preoperative biliary drainage. Fourteen cases of Mirizzi's syndrome are presented here. Clinical presentation was pain (14), jaundice (14), fever (10) and peritonitis (1). A clinical diagnosis of choledocholithiasis was considered in all the patients. Pre-operative diagnosis of Mirizzi's syndrome was made in five patients on the basis of cholangiogram and the remaining cases were diagnosed at surgery.
