Endophthalmitis associated with extrusion of a ganciclovir implant.

PURPOSE: To report a case of endophthalmitis with associated scleral defect after intravitreal positioning of a ganciclovir implant. METHOD: Interventional case report. A 39-year-old woman developed acute endophthalmitis 1 month after insertion of a ganciclovir implant. The eye was enucleated. RESULTS: Pathologic examination of the enucleated globe revealed an intrascleral location of the implant suture tab and a vitreous abscess. CONCLUSION: Intraocular infection resulted from a scleral gap containing a foreign body (surgical implant). Appropriate surgical techniques should minimize the ri

[Two rare complications of percutaneous endoscopic gastrostomy: obstruction of the pylorus and gastrocolic fistula occurring in one patient].

UNLABELLED: We report an 10 month old male infant, who developed an obstruction of the pylorus by dislocation of the retention disk 5 months after percutaneous endoscopic gastrostomy (PEG). This could be corrected under endoscopic control. Two years after PEG insertion diarrhoe occurred immediately after feeding caused by a gastrocolic fistula with dislocation of the retention disk in the colon transversum. An excision of the fistula and a resection of the colon segment were performed successfully. CONCLUSION: In patients with PEG and unclear abdominal symptoms a tube dislocation has to be ke

Late traumatic intraocular lens extrusion after penetrating keratoplasty.

BACKGROUND: Penetrating keratoplasty places a patient at risk for wound rupture from blunt trauma because the graft-host interface remains weakened for years after the surgery. Violent environments, contact sports, and strenuous activity put patients with compromised corneal structural integrity at high risk of traumatic injury. CASE REPORT: This case report presents a 42-year-old penetrating keratoplasty patient with a history of homelessness, polysubstance abuse, and domestic violence. This patient experienced a ruptured globe at the graft-host junction secondary to a direct blow by a fist,

Randomized trial of stent placed above and across the sphincter of Oddi in malignant bile duct obstruction.

BACKGROUND: Placement of stents above an intact sphincter of Oddi might prevent migration of bacteria and deposition of organic material into the stent. In patients with malignant obstructive jaundice prolongation of function time of the stent would be expected if it is placed above the sphincter of Oddi. METHODS: Thirty-four patients were randomized to stent placement either above (n = 17) or across (n = 17) the sphincter of Oddi. Straight 10F gauge Teflon stents were used. The patients were evaluated clinically and biochemically at monthly intervals during follow-up. RESULTS: The median st

Total integration of an ocular implant/prosthesis: preliminary in vivo study of a new design.

An ocular implant designed to simplify surgical implantation, minimize infection and extrusion, and improve motility when used as a prosthetic replacement for eyes removed because of damage or disease was tested in rabbit eyes. The implant consisted of a silicone sphere with patches of porous silicone elastomer at the natural insertion sites of the extraocular muscles. Nine implants also had patch material around the base of an integral post designed to distribute the weight of and transfer motility to an overlying prosthetic conformer. Six implants without posts had a circular porous patch on

[A complication of oleothorax--after 38 years].

We present a case-history of a 50 year-old woman who 38 years earlier had been operated for pulmonary tuberculosis by plugging of the pleural cavity with oil (oleothorax). She developed a tumour under the scapula, which it turned out was due to migration of the oil plug to the subscapular area, an unusual complication to oleothorax. Other complications are described.

Cardiac Perforation and Multiple Emboli After Percutaneous Vertebroplasty.

Percutaneous vertebroplasty is a minimally invasive technique for treating vertebral compression fractures and tumors. Although percutaneous vertebroplasty is considered a relatively safe and technically simple procedure, it is also associated with life-threatening complications as a result of cement leakage, including cardiac perforation and pulmonary embolism. A 63-year-old woman underwent percutaneous vertebroplasty for an L3 vertebral fracture and had cement leaks into the inferior vena cava, pulmonary arteries, and right heart chambers, with a free wall perforation. Surgical removal of th

Neck lymphadenitis due to silicone granuloma after mammary implants.

INTRODUCTION: A foreign body reaction due to silicone where it is infiltrated or at the places to which it can migrate is known as siliconoma. The use of silicone in breast augmentation procedures can provoke this reaction at the neck level in cases of leakage from mammary implants. METHODS: We reviewed the cases of patients with increased size neck lymph nodes who had previously undergone plastic surgery of the breast with highly cohesive silicone gel implants. RESULTS: In a 10-year period, we identified 12 cases with silicone-infiltrated neck lymphadenopathies, histologically confirmed by

Nitric Oxide-Releasing Thixotropic Hydrogels as Antibacterial and Hemocompatible Catheter Locks.

Catheters are indispensable medical tools for accessing blood vessels, hollow organs, and body cavities to facilitate medication delivery and fluid drainage. However, they also serve as major entry points for bacterial contamination and trigger foreign body responses, necessitating locking strategies that are both bactericidal and biocompatible. This study introduces the first gel-based catheter lock, in contrast to conventional liquid locks. The gel is a poloxamer-based hydrogel formulated with 2-hydroxypropyl α-cyclodextrin (HP-αCD). HP-αCD forms supramolecular complexes with the poloxamer t

Macrophages and fibroblasts in foreign body reactions: How mechanical cues drive cell functions?

Biomaterials, when implanted in the human body, can induce a series of cell- and cytokine-related reactions termed foreign body reactions (FBRs). In the progression of FBRs, macrophages regulate inflammation and healing by polarizing to either a pro-inflammatory or pro-healing phenotype and recruit fibroblasts by secreting cytokines. Stimulated by the biomaterials, fibrotic capsule is formed eventually. The implant, along with its newly formed capsule, introduces various mechanical cues that influence cellular functions. Mechanosensing proteins, such as integrins or ion channels, transduce ext

Early Buckle Migration and Restrictive Strabismus after Successful Medical Management of Scleral Buckle Infection.

We report a case of restrictive strabismus caused by early scleral buckle (SB) migration within 1 month of surgery after successful medical management of SB infection. A 24 year-old man underwent scleral buckling surgery for left eye inferior retinal detachment (RD). A solid silicone buckle element was placed inferiorly along with an encircling silicone band. Two days after surgery, he presented with SB infection. Methicillin resistant Staphylococcus aureus was cultured from the exudate at conjunctival suture sites. Since the retina was well attached and it was only the second postoperative da

Spinal intradural granuloma as a complication of an infected cerebrospinal fluid drainage tube fragment--case report.

A 69-year-old man presented with subarachnoid hemorrhage due to a ruptured anterior communicating artery aneurysm. The aneurysm neck was clipped and a lumbar drainage tube was inserted for cerebrospinal fluid drainage. However, the tube was accidentally cut during removal and a fragment remained in the spinal canal. A subarachnoid, subcutaneous abscess appeared 7 days later, which was treated with antibiotics. He noted numbness of his left leg after 6 months, and gait disturbance manifested 3 months later. T(1)-weighted magnetic resonance (MR) imaging disclosed a well-enhanced extramedullary m

Misdiagnosed large bladder stone after laparoscopic radical prostatectomy and literature review: a case report.

Large bladder stone is an uncommon disease, and its diagnosis is sometimes challenging. Herein we reported the case of a 75-year-old male patient who was admitted to undergo open cystostomy for a diagnosis of "urinary incontinence, urethral stricture, post-radical prostatectomy state, and urinary tract infection". However, during the operation, we found that he was misdiagnosed by preoperative MRI examination, and he had a large bladder stone measuring approximately 6 × 5 cm and weighing 105 grams. Furthermore, the stone was found to be caused by a migrated Hem-o-lok clip originating from the
