Infection of a Nylon Foil Orbital Implant Due to Fusarium brachygibbosum and Lomentospora prolificans After Intranasal Methamphetamine Use.

The authors describe a case of nylon foil implant infection caused by Fusarium brachygibbosum , and Lomentospora prolificans following medial orbital wall fracture repair in the setting of postoperative nasal methamphetamine use. A 61-year-old male presented with OS pain and swelling after a physical assault on his face. A CT of maxillofacial bones without contrast showed a moderately comminuted fracture of the medial wall of the left orbit with depression of fracture fragments into the left ethmoid air cells. Six days after repair of the medial wall fracture, the patient returned with a new o

Outcomes, risk factors, and conversion rate in acetabular fractures in patients over 65 years of age. Analysis at a mean follow-up of 6 years.

INTRODUCTION: Acetabular fractures are devastating injuries that can affect patients of different ages, as well as being associated with high or low energy trauma. Conversion to total hip arthroplasty (THA) results in increased complication rates, resource use and costs compared to primary THA due to osteoarthritis. The aim of this paper is to describe a retrospective cohort of patients over 65 years of age who presented with an acetabular fracture and were treated with open reduction and internal fixation (ORIF). METHODS: A retrospective cohort study was conducted from January 2002 to Decemb

Treatment of Distal Femur Fractures with a Combined Nail-Plate Construct: Techniques and Outcomes.

A combination treatment using a retrograde intramedullary (IM) nail and a lateral locking plate has scarcely been described in distal femur fracture treatment. In this retrospective inquiry, we review 97 patients who were treated for distal femur fracture by one of five fellowship-trained orthopedic trauma surgeons at an urban level-1 trauma center. Of the 97 patients enrolled in this investigation, eight were treated with a combined nail-plate hybrid construct. The remaining 89 patients were treated with either traditional IM nailing (22 patients) or locking plate fixation (67 patients) alone

Acute fracture of the acetabulum secondary to a convulsive seizure 3 years after total hip arthroplasty.

While aseptic loosening, osteolysis, and infection are the most common causes of failure after total hip arthroplasty (THA), late hip pain can also be the result of acetabular fracture related to trauma and resultant prosthetic failure. However, atraumatic fracture of the acetabulum around a well-fixed acetabular component is unusual. We present a patient with an acetabular fracture resulting from a generalized convulsive attack 3 years after an uncomplicated primary THA. A 33-year-old man presented with acute left hip pain. He had chronic renal insufficiency and had undergone bilateral THA du

[Complete transverse fractures of the talus: value of magnetic resonance imaging for detection of avascular necrosis].

The authors report a series of 32 complete transverse fractures of the neck or body of the talus. The fractures occurred mostly in young males, as a result of motor vehicle accidents. The fracture line was transverse in the neck or body of the talus in 20 cases, sagittal in four and comminuted in eight cases. Using Hawkins' classification, there were 10 type I, 16 type II, and 6 type III fractures. The treatment was conservative in 8 cases and surgical in 24. The patients were evaluated clinically and radiologically with an average follow-up of 7 years. All patients underwent radiological stud

Diagnosis of a C-2 fracture during physiatric consultation: a case report.

Consultation in the acute care setting allows physiatrists to make significant contributions. In our case, a 90-year-old woman presented to the emergency room after falling down five stairs. Six view C-spine films taken at that time revealed no fracture. She was sent home on anti-inflammatory medication and was later admitted for gastritis. A repeat six-view C-spine series again revealed no fracture. Physiatry was then consulted to prescribe modalities and therapies for "cervical strain." On examination, the patient demonstrated severely decreased neck range of motion limited by pain, with pai

Stabilization of acetabular fractures in elderly patients.

Eighteen patients who were sixty years or older and had an acute displaced fracture of the acetabulum were managed with open reduction and internal fixation. The average age of the patients was sixty-seven years (range, sixty to eighty-one years). Nine fractures were a result of a motor-vehicle accident, and nine occurred in a fall. Nine patients had multiple associated injuries, and most (sixteen patients) had other complex acetabular fractures. All of the patients had open reduction and internal fixation with either the ilioinguinal approach (thirteen patients) or the Kocher-Langenbeck appro

Interventions for treating fractures of the patella in adults.

BACKGROUND: Fractures of the patella (kneecap) account for around 1% of all human fractures. The treatment of these fractures can be surgical or conservative (such as immobilisation with a cast or brace). There are many different surgical and conservative interventions for treating fractures of the patella in adults. This is an update of a Cochrane Review first published in 2015. OBJECTIVES: To assess the effects (benefits and harms) of interventions (surgical and conservative) for treating fractures of the patella in adults. SEARCH METHODS: We searched CENTRAL (2020, Issue 1), MEDLINE, Emba

Custom-Made Cement-Linked Distal Femoral Replacement for Interprosthetic Femur Fractures: Five-Year Follow-Up.

Interprosthetic femur fractures between total hip and total knee replacements are challenging injuries with rising incidence. Stability of the either implants, quality, and quantity of remaining bone as well as patients' comorbidities play an important role in operative management. Open reduction and internal fixation, advanced revision arthroplasty techniques, and megaprostheses are available options. In more complex cases, custom-made implants maybe necessary to achieve immediate weight-bearing and restore function.

Unstable fracture-dislocations of the elbow.

Fracture-dislocations of the elbow are devastating injuries. The surgeon must maintain a high index of suspicion when evaluating an elbow dislocation to avoid missing critical associated injuries. Patterns of unstable fracture-dislocations include the "terrible triad" injury of the elbow (elbow dislocation, radial head fracture, and coronoid fracture), transolecranon fracture-dislocations, and the posterior Monteggia lesion. Complex fracture-dislocations of the elbow are treated surgically and are challenging injuries to manage. Elbow stability must be restored by addressing the specific compo

Complications after Pi Plate osteosynthesis.

BACKGROUND: Distal intraarticular radius fractures can be operated on by means of a palmar or dorsal approach. The dorsal approach has been reported as a treatment option that often leads to tendon ruptures. The purpose of this study was to show that a dorsal implant does not lead to a high rate of tendon ruptures. METHODS: A retrospective study was conducted in which the patient population was analyzed for complaints and complications, follow-up time, and further surgical procedures. RESULTS: A series of 389 patients with distal intraarticular fractures of the radius treated with the dorsal

The Association Between Distal Screw and Articular Subsidence in the Open Treatment of Intra-articular Distal Radius Fractures.

PURPOSE: This study investigated the association between volar plate screw position and postoperative radiographic outcomes in the open treatment of comminuted intra-articular distal radius fractures (DRFs). We hypothesized that increased distance from distal screws to subchondral bone is associated with early loss of reduction and articular subsidence. METHODS: We performed a two-center retrospective review over 10 years (2013-2023) for Arbeitsgemeinschaft für Osteosynthesefragen (AO) type C3 DRF treated with open reduction and volar locking plate fixation. We measured the distance from the

Early complications associated with fixation constructs of operatively treated patella fractures: a retrospective study.

PURPOSE: Although multiple fixation constructs exist for patella fractures, postoperative complications are common, and optimal construct choice remains unclear. This study evaluated construct-specific complication rates in operatively treated patella fractures. METHODS: A retrospective review was conducted of adult patients who underwent surgical fixation of patella fractures (AO/OTA 34) at a Level I trauma center between 2012 and 2022. Patients younger than 18 years, treated with isolated cerclage wiring, or with less than 90 days of follow-up were excluded. Patients were stratified by fixat

Techniques for Fixing Anterior Cruciate Ligament Tibial Avulsion Fractures in Multiligament Knee Injuries.

BACKGROUND: In patients with multiligament knee injuries, anterior cruciate ligament (ACL) tears are often reconstructed. Recent studies have shown good results when the ACL tibial avulsions are repaired. The advantages of ACL tibial avulsion repair are the preservation of the native anatomy, reduction in donor site morbidity, and lower risk of tunnel convergence. INDICATIONS & TECHNIQUE DESCRIPTION: We show 2 techniques for repairing ACL tibial avulsion fractures. The first case describes the use of hybrid fixation (screw and suture), with staged repair and reconstruction in a patient with h
