Treatment of idiopathic clubfoot: an historical review.

Idiopathic clubfoot, one of the most common problems in pediatric orthopaedics, is characterized by a complex three-dimensional deformity of the foot. The treatment of clubfoot is controversial and continues to be one of the biggest challenges in pediatric orthopaedics. This controversy is due in part to the difficulty in measuring and evaluating the effectiveness of different treatment methods. We believe the heart of the debate is a lack of understanding of the functional anatomy of the deformity, the biological response of young connective tissue to injury and repair, and their combined eff

Open reduction and cementation for femoral head fracture secondary to avascular necrosis: preliminary report.

Current treatment for femoral head avascular necrosis has shown good results in early stages of disease, but are not as impressive after progression to collapse. We treated 19 patients (20 hips) with Stage III avascular necrosis (AVN) by open reduction augmented by methylmethacrylate cementation. Follow up ranges from 6 months to 2 years (average = 8.7 months). We followed patient progress with pre- and post-operative Harris Hip Scores, Womac Osteoarthritis Index and a Health Status Questionnaire (SF36). All patients realized immediate post-operative pain relief and improvement in function. Ha

Osteoporosis: the increasing role of the orthopaedist.

Osteoporosis is an ever-increasing problem as our population ages. However, it is also to a large extent a preventable problem. The orthopaedist now has the ability to determine bone mass, the rate of turnover, and the fracture risk. Skeletal bone mass can be evaluated with DXA; the rate of bone resorption can be determined by assessment of collagen-degradation urinary products; and the weight status, fracture history, and history of smoking can be used to predict the fracture risk in individual patients. The orthopaedic physician also needs to take an active role in advising their younger pat

Subchondral metastasis: report of five cases.

Subchondral metastasis is a rare occurrence and poses a diagnostic dilemma as initial films may show a lytic lesion in the subchondral region often misinterpreted as being benign. We present five cases of subchondral metastasis as well as a review of the literature. In our cases, we present subchondral metastasis in the elbow, shoulder, and hip joints. All patients had pain over the affected joint and most presented with a lytic lesion in the subchondral bone. Three patients have died since presentation and two are doing well at last follow up visit. Subchondral metastasis is a rare entity, bu

Distal short segment fixation of thoracolumbar and lumbar injuries.

Fifteen patients with thoracolumbar and lumbar spine injuries were treated between November, 1993, and April, 1996, using a posterior approach that involved short segment distal fixation at or one level below the level of injury. Medical records and radiographs were reviewed for complications, maintenance of correction, repeat surgeries and functional status. Indications for this technique included fractures and ligamentous injuries that did not require direct canal decompression and were without severe comminution. The level of injury was T12 in two patients, L1 in seven patients, L2 in two p

Kienbock's disease: treatment by implantation of vascular pedicle and bone grafting.

Eleven patients with stage II and IIIA Kienbock's disease treated with vascular pedicle implantation and bone grafting were reviewed to determine the long term efficacy of the procedure. Eight wrists had a negative ulnar variance and three wrists had a neutral ulnar variance. The procedure involved implanting the second dorsal metacarpal artery and its venae commitantes into a hole made in the lunate through a dorsal approach. Cancellous bone graft from the dorsal aspect of the distal radius was loosely packed in the hole to secure the pedicle. Follow-up was from 37 to 140 months with an avera

The pathology of transient regional osteoporosis.

Biopsy specimens from 19 patients with transient regional osteoporosis were studied. All patients presented with pain. There were nine patients with transient osteoporosis of the hip. Six of these specimens were therapeutic core biopsies, and three were femoral heads removed during total hip replacement. The other patients with osteoporosis in different locations had biopsies to rule out infection or neoplasm. Five of these patients had transient osteoporosis of the knee. Three had ankle involvement and two had involvement of the tibial shaft. Plain radiographs were available for study in all

Fractures of the radius and ulna in adults: an analysis of factors affecting outcome.

Concurrent data were collected by the authors for 104 fractures of the shafts of the radius and ulna in 102 adult patients to determine the relationship of subjective, objective, radiographic and economic outcome parameters to the method of treatment, type of fracture (open or closed), degree of comminution, and the presence of other injuries. Patients treated by open reduction and internal fixation (ORIF) had less pain, lost less forearm rotation, and returned to the same work following injury more frequently than those treated by closed reduction and casting (CR) or pins-in-plaster (PIP). Severe growth retardation following limb lengthening: a case report.

The first limb lengthening was reported by Codivilla in 1905. Common complications reported with this procedure include pain tract infections, angulation deformities, joint contractures, and residual limb length discrepancies. Growth arrest or severe growth retardation in the lengthened bones has rarely been reported. We report a case of a skeletally immature patient with right sided hemihypertrophy who underwent lengthening of the left lower extremity by the method of DeBastiani using Orthofix external fixators. Postoperatively he developed severe growth retardation in the lengthened limb, as

Differences in ligamenta flava among some mammals.

The ligamenta flava of the thoracolumbar spine of mammals with great spinal mobility have a larger content of elastin than in animals with little spinal motion. The elastin content is also higher in the ligamenta flava of mammals with arched backs and prolonged sitting posture than in nonsitting mammals with the vertebral column in the shape of a flat bow. The ligamentum flavum appears to be important; a) in the control of intervertebra movements, b) in providing intrinsic stability to the spine in the sitting and standing postures and c) in maintaining a smooth surface in the posterior wall o

The management of severe avascular necrosis following slipped capital femoral epiphysis by transtrochanteric rotational osteotomy. Results of successful treatment in two cases with longterm follow-up.

We present two cases of unstable slipped capital femoral epiphysis complicated by avascular necrosis that were successfully treated with transtrochanteric anterior rotational osteotomy. These two cases both had severe avascular necrosis and were felt to be unsalvageable. We recommend this surgical technique as a viable alternative to other forms of treatment in this selected patient population.

Anterior labrum reconstruction with mini-capsular shift procedure.

An anterior labrum reconstruction and mini-capsular shift (ARMS) procedure was performed in 64 patients (69 shoulders) with traumatic anterior or anterior-inferior glenohumeral instability between 1984 and 1990. Sixty-three of the sixty-nine stabilizations were performed for recurrent dislocation and six were performed for recurrent subluxation. Eighty-eight percent of the patients (61 shoulders) were available for clinical follow-up at an average of 36 months (range 28-78). There were 44 males and 12 females with an average age of 28 years (range 15-46). Excellent range of motion was recorded

Ulnar subluxation of the extensor digitorum communis tendon: a case report and review of the literature.

Ulnar subluxation of the extensor digitorum communis tendon at the MCP joint occurs infrequently in the nonrheumatoid patient and is secondary to one of four reported etiologies: traumatic, spontaneous, congenital, or epileptic. If symptomatic, patients may present with pain, swelling, a sensation of the tendon "snapping", "catching", "locking", or the inability to fully extend the MCP joint. Conservative and operative interventions have been recommended as treatment options. In the acute traumatic dislocation (less than ten days post injury), satisfactory results may be obtained with simple s

Treatment of difficult fractures and nonunions of the humerus and elbow with a modified Küntscher nail.

A series of fourteen difficult fractures and nonunions of the humerus and elbow have been treated over a period of twenty-eight years with a modified Kuntscher Nail. A total of twenty-six operative attempts had been previously made in this group of fourteen patients. One patient had eight failed surgeries prior to treatment. Slots were placed along the spine of the nail for transfixion with screws. In two instances additional modification of the Kuntscher nail was made by attaching a plate to the end of the nail for fixation to the ulna after retrograde insertion into the humerus. One such dev
