Increased vestibular contribution to posture control in individuals with chronic headache.

OBJECTIVE: Numerous studies have identified an association between headache disorders and vestibular symptoms, such as dizziness, vertigo, and motion sensitivity. Using bipolar, binaural galvanic vestibular stimulation (GVS), our objectives were to (1) determine the degree of vestibular sensitivity in borderline headache sufferers, and (2) characterize the postural response to vestibular perturbation in these individuals. METHODS: Fourteen volunteers participated in this study: 8 individuals with chronic headache (6 with migraine and 2 with tension type headache (TTH)), and 6 healthy control

[Diagnosis of labyrinthine fistula in patients suffering from otitis media purulenta chronica with cholesteatoma: informative value of different methods].

Informative content of different methods of labyrinthine fistulas (LF) diagnosis was assessed in 118 patients suffering from otitis media purulenta chronica (OMPC) with cholesteatoma which ran for 20 years and longer in 82.2% patients, 83.9% had fistula symptom. Unilateral hypoacusis was diagnosed in 68 (57.6%), bilateral--in 50 (42.4%) patients. Severity of vestibular disorders correlated positively with LF size and duration of the disease. Advanced LF was detected in 65 (55.1%) patients, multiple--in 7 (5.9%). Computed tomography of the temporal bone detected LF in all 29 OMPC examinees incl

Mondini-like malformation mimicking otosclerosis and superior semicircular canal dehiscence.

In 2003, it was reported that superior semicircular canal dehiscence can mimic otosclerosis because of low-frequency bone conduction hearing gain and dissipation of air-conducted acoustic energy through the dehiscence. We report the case of a 17-year-old girl with left-sided combined hearing loss thought to be due to otosclerosis. Bone conduction thresholds were -10 dB at 250 and 500 Hz and she had a 40 dB air-bone gap at 250 Hz. When a tuning fork was placed at her ankle she heard it in her left ear. Acoustic reflexes and vestibular evoked myogenic potentials could be elicited bilaterally. Im

[Validation of a clinical scale for the diagnosis of perilymphatic fistula].

INTRODUCTION: The diagnosis of perilymphatic fistula (PLF) is difficult because no single clinical situation gives the diagnosis for sure. The goal of this article is to study the clinical situations where you must suspect a PLF and to support a clinical scale described in a previous work (Bussières et al 2003). METHODS: Retrospective study of 15 patients that had an exploratory tympanotomy with a PLF not confirmed preoperatively. An analysis of the symptoms, signs and complementary exams were done. The surgical technique and findings and the postoperative evolution were noted. RESULTS: Ther

Cochlear microperfusion: experimental evaluation of a potential new therapy for severe hearing loss caused by inflammation.

HYPOTHESIS: Cochlear microperfusion will be a useful treatment of severe sensorineural hearing loss caused by inflammation. BACKGROUND: Viruses, bacteria, and autoimmunity can initiate inflammation in the inner ear. The acute phase is associated with elevations in cytokines, nitrous oxide, and cellular infiltrates and the breakdown of the blood-labyrinthine barrier. The chronic phase leads to irreversible ossification of the labyrinth. METHODS: The authors developed cochlear microperfusion to facilitate removal of inflammatory cells and their byproducts during the acute phase of inflammation

Comparison of tone burst and tapping evocation of myogenic potentials in patients with chronic otitis media.

OBJECTIVE: Vestibular evoked myogenic potential (VEMP) has recently been broadly studied in cochleo-vestibular disorders to elucidate its mechanism. Because it is evoked by loud sound stimulation, impairment of the sound transmission through the middle ear may affect VEMP results. This study aims to compare the response rate of VEMPs using the tone burst method and the tapping method in patients with chronic otitis media (COM). DESIGN: Fourteen patients (22 ears) with conductive hearing loss due to COM were subjected to VEMP tests using both the tone burst method and the tapping method. Each

Scala vestibuli cochlear implantation in patients with partially ossified cochleas.

Partial cochlear obstruction is a relatively common finding in candidates for cochlear implants and frequently involves the inferior segment of the scala tympani in the basal turn of the cochlea. In such patients, the scala vestibuli is often patent and offers an alternative site for implantation. The current report describes two patients with such partial obstruction of the inferior segment of the basal cochlear turn, caused in one case by systemic vasculitis (Takayasu's disease) and in the other by obliterative otosclerosis. A scala vestibuli implantation allowed for complete insertion of th

Conductive hearing loss: investigation of possible inner ear origin in three cases studies.

This is a report of three cases of mixed hearing loss that resulted from inner ear disorders. Two cases were unilateral and the third was bilateral. The diagnosis was based on the findings of normal middle and external ears in association with the absence of round window reflexes. The contralateral stapedial reflex was present in the two unilateral cases. This is the first documentation of conductive deafness due to inner ear abnormality. This diagnosis should be considered in cases of conductive hearing loss if the middle and external ears are normal. More studies are needed to establish the

Vestibular projections in the human cortex.

There is considerable evidence from studies on cats and monkeys that several cortical areas such as area 2v at the tip of the intraparietal sulcus, area 3av in the sulcus centralis, the parietoinsular vestibular cortex adjacent to the posterior insula (PIVC) and area 7 in the inferior parietal lobule are involved in the processing of vestibular information. Microelectrode recordings from these areas have shown that: (1) most of these cortical neurons are connected trisynaptically to the labyrinthine endorgans and (2) they receive converging vestibular, visual and somatosensory inputs. These da

Eye movements during torso rotations in labyrinthine-defective subjects.

The aim of this study was to examine whether the chronic loss of vestibular function modifies perceptual and oculomotor responses during torso rotations in darkness. Subjects (4 patients with complete vestibular loss and 7 healthy volunteers) were seated on a rotating chair. Stimuli consisted of sinusoidal chair rotations (+/-30 degrees, 0.1 Hz and 0.011 Hz). We used 2 conditions: space stationary head (neck stimulation) and space stationary head and shoulders (torso stimulation). Horizontal eye deviations and slow component of eye movements were analysed. The results showed that eye movements

[Repositioning maneuver in benign paroxysmal vertigo of horizontal semicircular canal].

The introduction of the Lempert or Baloh "barbecue" maneuver in clinical practice has resolved many cases of paroxysmal positional vertigo of the horizontal semicircular canal. Unfortunately, this maneuver is rather difficult to perform in elderly or overweight patients and in those with reduced mobility. In such cases the Vannucchi "forced position" is the maneuver of choice. Diagnostic confirmation is, however, still a problem. The authors feel diagnosis is only confirmed when the positional nystagmus has been converted, inverted or disappears after the physical therapy maneuver. For this re

Reduction in obstructive breathing events during body rocking: a controlled polygraphic study in preterm and full-term infants.

OBJECTIVE: To investigate the effect of body rocking on infant respiratory behavior during sleep. METHODS: Eighteen infants with documented obstructive sleep apneas were studied. There were eight premature infants with persistent bradycardias and 10 infants born full-term, admitted after an idiopathic apparent life-threatening event. No cause for the obstructive apneas was found. The infants were recorded with polygraphic techniques during two successive nights. They were randomly assigned to a rocking or a nonrocking mattress. The conditions were reversed the following night, in a crossover

Effects of combined rupture of Reissner's membrane and the round window on hearing in the guinea pig.

Hearing impairment caused by sequential rupture of Reissner's membrane and the round window was investigated in guinea pigs to determine if the double-membrane break can cause acute profound sensorineural hearing loss. Reissner's membrane was lacerated through the stria vascularis at one of the turns of the cochlea, followed by rupture of the round window to create leakage of the perilymph. Action potentials (APs) to tone pip stimuli were recorded from the Fallopian canal electrode before and 90 min after these procedures. The results showed that the ears with the double-membrane break at the

Vestibular apparatus disorders after external radiation therapy for head and neck cancers.

External irradiation of different head and neck cancers may involve parts of the ear. The vestibular function of 25 patients in which the inner ear was comprised in the irradiated volume was investigated by electronystagmography (ENG). Doses administered to the vestibular system ranged between 2800 and 5120 cGy. Five patients suffered subjective vertigo or dizziness. Eleven patients (three out of five with vertigo) showed vestibular abnormalities to ENG (44% of the total). Altered responses to specific tests were as follow: six patients to the bithermal caloric stimulation, two to the pendular

Monothermal differential caloric testing in patients with Menière's disease.

The monothermal differential caloric test allows determination of vestibular recruitment and decruitment, variables which may help discriminate peripheral from central vestibular lesions. Previous reports indicated a strong association between vestibular recruitment and Menière's disease. This study examined patients having unilateral Menière's disease. Nystagmus beat frequency (NBF) and slow-component velocity (SCV) responses were recorded by electronystagmography (ENG). Electronystagmographic findings showing unilateral dysfunction were present in 54% of patients by slow-component velocity a

Cervico-ocular reflex enhancement in labyrinthine-defective and normal subjects.

The cervico-ocular reflex (COR) was elicited in 12 normal and 30 labyrinthine-defective (LD) subjects, lacking a vestibulo-ocular reflex (VOR) in the usual laboratory tests, by sinusoidal horizontal rotation of the body at 30 degrees amplitude and frequencies of 0.1, 0.2 and 0.4 Hz. The head was fixed in space and the eyes were open in total darkness. The gain of the COR was measured by relating the maximum slow phase velocity (MSPV) of the eyes to the maximum input velocity (19, 38 or 75 degrees/s) and by relating the cumulative eye displacement per hemicycle to the peak-to-peak amplitude of

Perilymphatic fistulas in children: rationale for therapy.

We report 26 consecutive patients (32 ears) who were identified in a 2 year period (July 1, 1985-June 30, 1987) with unexplained sudden, fluctuating, or progressive sensorineural hearing loss (SNHL). All patients underwent an exploratory tympanotomy and a perilymphatic fistula was identified in 13 patients (14 ears). The mean change of 14 +/- 27 dB in speech reception threshold before and after surgery was significant at p = 0.08 among children with fistula and ranged from -30 to 80 dB. In children with sudden, progressive or fluctuating SNHL and multiple sensory deficits, including blindness
