Conservative management of Retzius space hematoma following spontaneous vaginal delivery in a woman with an unscarred uterus: A case report.

INTRODUCTION: A retroperitoneal hematoma in obstetrics is very rare following spontaneous vaginal delivery. It requires a high index of suspicion, especially in a hemodynamically unstable patient with no active bleeding or external trauma postpartum, as early intervention is needed to prevent maternal morbidity and mortality. Conservative management may be an appropriate strategy in managing large hematomas even though surgical evacuation is classically recommended. CASE: We report the case of a 39-year-old woman, para three, with a hematoma of the Retzius space after spontaneous uncomplicate

Unnoticed small bowel perforation as a complication of lumbar discectomy.

BACKGROUND CONTEXT: Surgery for disc herniation is one of the most common traumas and neurosurgical procedures. Although discectomy has low morbidity, serious intra-abdominal complications can affect retroperitoneal structures, such as the large vessels, small intestine, and ureters. CASE REPORT: A 36-year-old woman in uncontrollable pain presented with left sciatic neuralgia in the L5 region. Magnetic resonance imaging revealed an extruded left paracentral hernia at L5-S1. With the patient in the decubitus prone position, trauma surgeons specializing in spine surgery performed an L5-S1 flave

Combined retroperitoneal resection with cytoreductive surgery and heated intraperitoneal chemotherapy for metastatic colorectal cancer with retroperitoneal and peritoneal metastases.

INTRODUCTION: Colorectal peritoneal metastases (CRPM) and retroperitoneal lymphatic metastases (RLM) are each considered poor prognostic indicators in colorectal cancer (CRC). Current data suggests that surgery for each including cytoreductive surgery and heated intraperitoneal chemotherapy (CRS/HIPEC) for CRPM and retroperitoneal resection (RR) for RLM may improve survival. Given the poor prognosis of each finding individually, each condition is generally considered a contraindication to the other. We describe the first series of combined RR and CRS/HIPEC. MATERIALS AND METHODS: Retrospectiv

Oncologic outcomes of retroperitoneal lymph node dissection following first-line chemotherapy for metastatic non-seminomatous germ-cell tumors.

BACKGROUND: Post-chemotherapy retroperitoneal lymph node dissection (pcRPLND) is integral to multimodal treatment of patients with metastatic non-seminomatous germ-cell tumors (NSGCT). We review pathologic and long-term outcomes of pcRPLND following first-line chemotherapy with a focus on residual mass size and primary tumor histology. Our goal was to identify new predictive approaches that can refine surgical indications. PATIENTS AND METHODS: Patients who underwent pcRPLND for NSGCT at our institution between 1 January 2000 and 18 January 2023 following first-line chemotherapy were included

Impact of CYP2D6 Polymorphisms on Tamoxifen Treatment in Patients With Retroperitoneal Fibrosis: A First Step Towards Tailored Therapy?

OBJECTIVE: To investigate the influence of CYP2D6 polymorphisms on outcomes and health-related quality of life of patients with retroperitoneal fibrosis (RPF) receiving tamoxifen (TMX). TMX is an effective alternative to corticosteroids for patients with RPF. Conversion of TMX to more potent endoxifen is dependent on enzyme activity of CYP2D6. MATERIALS AND METHODS: CYP2D6 genotyping and phenotype prediction of all patients treated with TMX between 02/2007 and 01/2018 was assessed using multiplex polymerase chain reaction (PCR). Groups were classified by phenotype: extensive (EM) vs poor and

Duodenal perforation and retroperitoneal abscess after coil embolisation of an inferior pancreaticoduodenal artery pseudoaneurysm in a patient previously treated with bevacizumab.

Transcatheter arterial embolisation is an established treatment for visceral pseudoaneurysms and pancreaticoduodenal arcade embolisation is generally well tolerated because of rich collateral flow. We report a man in his 70s with metastatic rectal cancer previously treated with bevacizumab who developed duodenal perforation and retroperitoneal abscess 15 days after embolisation of an inferior pancreaticoduodenal artery pseudoaneurysm using coils and adjunctive N-butyl cyanoacrylate-Lipiodol. He improved with broad-spectrum antibiotics, CT-guided drainage and conservative management, avoiding e

Challenges in diagnosis and management of a retroperitoneal pregnancy.

An ectopic pregnancy in a retroperitoneal location is an extremely rare and life-threatening situation. Here, we present a case report of a nulliparous woman in her 30s who presented with rising β-human chorionic gonadotropin (hCG) titre after embryo transfer but with no pregnancy detected on transvaginal scan. She was referred to us after two negative laparoscopies done outside and finally an MRI revealing a retroperitoneal pregnancy embedded close to the aorta near the origin of the left renal artery. As parenteral methotrexate had no effect, an ultrasound-guided intrasac methotrexate instil

Crossing Vessels in Retroperitoneal Laparoscopic Pyeloplasty: Anatomy and Outcomes.

PURPOSE: Crossing vessels are commonly encountered during pyeloplasty for ureteropelvic junction obstruction (UPJO) and may influence surgical strategy. We aimed to describe the anatomical distribution of crossing vessels and their management options during retroperitoneal laparoscopic pyeloplasty (RLP) and report the long-term outcomes. MATERIALS AND METHODS: We retrospectively reviewed consecutive adult patients who underwent RLP for UPJO between 2006 and 2025 by a single surgeon. Preoperative computed tomography (CT) or magnetic resonance (MR) urography was routinely performed to assess va

A ten-year propensity score-matched comparison of lateral transabdominal and posterior retroperitoneoscopic approaches to adrenalectomy.

BACKGROUND: Minimally invasive adrenalectomy (MIA) is mostly performed as lateral transabdominal adrenalectomy (LTA) or posterior retroperitoneoscopic adrenalectomy (PRA). This study aims to compare LTA and PRA in terms of perioperative outcomes. METHODS: A retrospective cohort study was conducted on 225 patients who underwent MIA from 2013 to 2023. Patients were divided into two groups: LTA (n = 182) and PRA (n = 43). Propensity score matching (PSM) was used for group balancing. RESULTS: PRA was associated with shorter operative times (p = 0.004), shorter hospital stay (p < 0.001), fewer dr

Balancing the clock: Time thresholds for angioembolization and patient outcomes in pelvic trauma.

INTRODUCTION: In the management of patients with pelvic fractures associated with retroperitoneal hemorrhage, prompt angioembolization is typically required to achieve hemostasis. However, not all institutions are equipped to perform this procedure in a timely manner. This study assessed the impact of time to angioembolization on the outcomes of patients with pelvic fractures. We hypothesize that there is a time threshold for the performance of angioembolization, beyond which patient outcomes may worsen. METHODS: The trauma registry system was retrospectively queried for patients treated for

Comments on "transperitoneal vs retroperitoneal robotic partial nephrectomy: a meta-analysis and systematic review of propensity-matched studies".

This letter provides a review of the meta-analysis by Rong et al. comparing transperitoneal (TPPN) and retroperitoneal (RPPN) robotic partial nephrectomy. While the study offers valuable insights into perioperative outcomes, such as operating time, hospital stay, blood loss, and complications, there are several methodological concerns. These include the use of non-randomized controlled trials, which may introduce bias, the lack of long-term follow-up, and high heterogeneity across studies. The letter also addresses the evaluation of serious complications and data extraction methods. It suggest

Donor-derived carbapenem-resistant Klebsiella Pneumoniae infections leading to retroperitoneal abscesses in kidney transplant recipients: a report of two cases.

BACKGROUND: Retroperitoneal abscesses caused by donor-derived Carbapenem-Resistant Klebsiella Pneumoniae (CRKP) infections are rare and often challenging to diagnose early due to a lack of specific symptoms. CASE PRESENTATION: In case one, a 64-year-old male presented with unexplained fever and emaciation three months after undergoing a kidney transplant for end-stage renal disease. Metagenomic Next-Generation Sequencing identified CRKP in peripheral blood samples, and CT scans confirmed a retroperitoneal abscess. Considering the infectious status of the donor and other recipients, donor-deri

Complete Vascular Replacement of the Infrarenal Inferior Vena Cava and Abdominal Aorta during Post-Chemotherapy Retroperitoneal Lymph Node Dissection for a Non-Seminomatous Germ Cell Tumor.

Testicular germ cell tumors (TGCTs) are the leading cause of cancer-related death in males between the ages of 20 and 40. In the advanced stages, the combination of cisplatin-based chemotherapy and surgical excision of the remaining tumor can cure many of these patients. Vascular procedures may be required during retroperitoneal lymph node dissection (RPLND) in order to achieve the complete excision of all residual retroperitoneal masses. Careful assessment of pre-operative imaging and the identification of patients who could benefit from additional procedures are important for minimizing peri
