Cost-effectiveness of MRI in managing suspected scaphoid fractures.

In a cost-effectiveness study, we compared a treatment algorithm using repeated radiological examination with an algorithm using subacute MRI in patients with clinical signs of scaphoid fracture but normal initial radiography. Twenty-seven patients were included in both groups, and MRI reduced the immobilisation time from 20 days (range, 6-54) to 4 days (range, 1-19) and sick leave from 27 days (1-92) to 11 days (0-28). Use of MRI increased hospital costs by 151 euro (P<0.05), but reduced non-hospital costs by 2869 euro (P<0.05), making MRI cost-effective in the treatment of suspected scaphoid

A comparative analysis of the accuracy, diagnostic uncertainty and cost of imaging modalities in suspected scaphoid fractures.

INTRODUCTION: Clinical examination of suspected scaphoid fractures is sensitive, but not specific, and initial plain radiographs lack sensitivity. A variety of second-line imaging techniques have been proposed to improve immediate diagnosis and reduce overtreatment. Different sample populations and methods of reporting test performance have made side-by-side comparison difficult. The aim of this study is to describe the epidemiology of clinically suspected scaphoid fracture and determine the prevalence of true fracture. These data are used to compare second-line imaging techniques in our popul

A cost/utility analysis of open reduction and internal fixation versus cast immobilization for acute nondisplaced mid-waist scaphoid fractures.

BACKGROUND: Open reduction and internal fixation and cast immobilization are both acceptable treatment options for nondisplaced waist fractures of the scaphoid. The authors conducted a cost/utility analysis to weigh open reduction and internal fixation against cast immobilization in the treatment of acute nondisplaced mid-waist scaphoid fractures. METHODS: The authors used a decision-analytic model to calculate the outcomes and costs of open reduction and internal fixation and cast immobilization, assuming the societal perspective. Utilities were assessed from 50 randomly selected medical stu

Isoelastic resurfacing prosthesis for distal radius fractures: Outcomes in 24 cases with at least 2 years' follow-up.

BACKGROUND: Unicompartmental resurfacing prostheses have been reported to be useful for treating comminuted fractures involving the distal radial joint surface in elderly patients with osteoporosis. However, the data on this method remain scarce and further evaluations are needed. The objective of this study was to evaluate the clinical and radiological outcomes after unicompartmental resurfacing prosthesis implantation in patients with distal radius fractures and at least 2 years' follow-up. HYPOTHESIS: Outcomes with Prosthelast® are as good in the mid-term as in the short term. MATERIALS A

A fresh cadaveric study of the paratubal muscles: implications for eustachian tube function in cleft palate.

The aims of this anatomic investigation were to examine the levator veli palatini, tensor veli palatini, and salpingopharyngeus muscles in relation to normal eustachian tube function and to analyze the clinical implications of these data for tubal physiology in cleft palate individuals. Detailed dissections under 3.2x loupe magnification were conducted on the paratubal muscles of 15 fresh human adult cadaveric head specimens, paying particular attention to their cranial base anatomy. Each half of the cadaveric heads was examined separately, giving a sample size of 30. The cranial base origin o

[Results after minimally invasive therapy of acute scapholunate dissociation].

In 27 cases with acute scapholunate ligament injury, minimal invasive treatment was performed. After closed reduction scapholunate transfixation was achieved by Kirschner wires. Postoperatively, a short-arm cast was applied for six weeks. All patients have been reexamined on average 26 months after operation. Follow-up results have been good concerning clinical examination and patients' satisfaction. Radiological examination has shown recurrence of scapholunate instability in 15 cases. In fact, 11% of the patients had dynamic and 44% static instability at follow-up. Recurrence has been most fr

Treatment of wrist and hand fractures with natural magnets: preliminary report.

The Authors, after having defined the phenomenon and the biological characteristics of natural magnets, evaluate their ability in accelerating the formation of bone callus in hand and wrist fractures compared to treatment with immobilization in a plaster cast. Forty patients (4 females and 37 males) between 20 and 86 years of age were treated. A small natural magnet was inserted in each of the plaster casts (diameter: 2cm, height: 0.5cm) made of 4 blocks in Neodymium-Iron-Boron, capable of generating 4 magnetic poles (2 positive and 2 negative) of diagonal alternate polarity that produced a sy

Bilateral trans-radial trans-scaphoid perilunate fracture-dislocation with bilateral dorsal elbow dislocation: a case report.

BACKGROUND: Bilateral trans-radial trans-scaphoid perilunate fracture-dislocation is one of the less common injuries of the wrist, occurring as a consequence of high-energy trauma. Its coincidence with bilateral elbow dislocation is extremely rare. CASE PRESENTATION: The authors present a case of a 24-year old male of Czech origin who attained this unique injury by falling from a height of 4 m onto both upper extremities. Bilateral open reduction of the wrists using both palmar and dorsal approaches and closed reduction of both elbow joints was performed. The arms were immobilized in casts fo

Subtalar dislocations: Analysis of a case series. Our urgent treatment algorithm.

INTRODUCTION AND OBJECTIVES: Subtalar dislocations are a rare entity (<1%) in traumatic foot injuries. There is a loss of anatomical relationship between the talus, calcaneus and scaphoid. There are only small-published series. MATERIAL AND METHODS: We present a series of patients (N=13) with subtalar dislocations in which a descriptive analysis of the main epidemiological, clinical and radiological variables was carried out, from which an urgent treatment algorithm is proposed. Cases with fractures of the neck of the talus, body of the calcaneus, or isolated Chopart fracture-dislocations wer

Failure of Wrist Hemiarthroplasty.

BACKGROUND: Wrist hemiarthroplasty has emerged as a motion-sparing option for severe wrist arthritis. It is technically easy with advantages of limited bone resection and no risk of nonunion. Given the relative infancy of the procedure in clinical practice, there are limited data on patient outcomes. METHODS: Eleven patients were treated with wrist reconstructive hemiarthroplasty. Indications included 1 patient with scaphoid nonunion advanced collapse, 9 patients with scapholunate advanced collapse, and 1 patient with capitolunate arthritis. Average age was 63 years; average follow-up was 4 y

Wrist fractures: sensitivity of radiography, prevalence, and patterns in MDCT.

The aims of this study are to evaluate diagnostic performance of conventional radiographs for wrist fractures using multidetector computerized tomography (MDCT) as a reference standard, to determine prevalence, demographic risk factors including age and sex, and associations among various wrist fractures. A retrospective study was performed, finding a total 455 patients (457 wrists) who had wrist trauma and who had undergone a radiography and subsequent MDCT examination during a 45-month period. The MDCT and radiographs of the patients were reviewed by two radiologists, and a consensus was obt

Wrist anatomy: incidence, distribution, and correlation of anatomic variations, tears, and arthrosis.

We dissected 393 wrists to evaluate the incidence and distribution of anatomic features, arthrosis, chondromalacia, and soft tissue lesions. The data were then analyzed for any statistically significant associations among the different variables. The most common (73%) lunate morphology had a separate medial facet on its distal surface for the hamate. The capitate had a separate facet for the fourth metacarpal in 86% of the wrists. Fourth metacarpals with a dorsal radial facet, either separate from or connected to the rest of the fourth metacarpal base, were the most common types of fourth meta

Silicone rubber carpal implants: a study of the incidence of late osseous complications.

This clinical review of 53 silicone rubber carpal implants done by the senior author (P. R. C.) between 1976 and 1983 determines the incidence of so-called "silicone rubber synovitis." Although previously reported in small series, the true incidence of this complication of wear of the implant has not been documented. In this study late radiographic follow-up demonstrated lytic lesions adjacent to the carpal implants in 75% of the scaphoid implants, 55% of the lunate implants, and 75% of the scapholunate implants. In patients with x-ray film evidence of lytic lesions, more than half (56%) compl

Patient expectations and outcomes of successful scaphoid surgery: a prospective cohort analysis using the trauma expectation and trauma outcome measure (TEFTOM).

BACKGROUND: Scaphoid fractures are common carpal injuries that are commonly treated operatively. Increasingly, patient-reported outcome measures (PROMs) highlight the influence of expectations on perceived success. The Trauma Expectation and Trauma Outcome Measure (TEFTOM) is a validated instrument assessing expectations (TEF) and outcomes (TOM) across domains of pain, function, disability, injury satisfaction, and overall satisfaction. While applied in other orthopaedic trauma conditions, TEFTOM has not been previously studied in scaphoid surgery. METHODS: This prospective cohort study enrol

Assessment of scaphoid mobility in patients with suspected scapholunate instability: a radiographic and four-dimensional computed tomography study.

INTRODUCTION: This study was performed to evaluate the validity and reliability of the anteroposterior intrascaphoid angle (APISA) on radiographs, and the radioscaphoid angle (RSA) on dynamic four-dimensional computed tomography (4DCT) as indicators of the extent of scaphoid flexion-extension. We also evaluated the contribution of these two measurements relative to the stage of scapholunate instability for identifying reduced scaphoid mobility. METHODS: One-hundred and fifty patients referred with suspected chronic scapholunate dissociation were investigated with anteroposterior radiograph of
