In order to describe the clinical manifestations of erythema chronicum migrans Afzelius (ECMA) in Sweden and to compare them with those in Lyme disease in the United States, 231 patients with ECMA were investigated. Although there are many similarities between the two disorders the findings also point to differences. The skin lesions were of longer duration (median 5-6 weeks) than those in Lyme disease but less often multiple (8%). General symptoms were found in about half of the patients with a short disease duration (less than or equal to 3 weeks), but were usually mild. Laboratory abnormali

The recent discovery that spirochetes transmitted by the tick Ixodes ricinus are involved in the etiology of erythema chronicum migrans Afzelius (ECMA), Bannwarth's syndrome, and acrodermatitis chronica atrophicans (ACA) has thrown new light upon these disorders. Thirty-two patients showing clinical and serological evidence of ACA were investigated. Histologically, constant findings in active ACA lesions were telangiectases and a lymphocytic infiltrate with a moderate to rich admixture of plasma cells. Clinically, besides ACA lesions, lichen sclerosus et atrophicus (LSA)-like lesions were foun

The clinical features in eight patients with neurologic abnormalities typical of Lyme disease and elevated titers of antibody to the spirochete, Borrelia burgdorferi, its causative agent, are described. None of the patients had the diagnostic skin lesion, erythema chronicum migrans. Lyme arthritis, the other clinical marker for the disease, developed subsequently in only three. The neurologic abnormalities included aseptic meningitis, encephalitis, cranial neuritis, motor and sensory radiculitis, and myelitis in various combinations. The occurrence of severe encephalitis resulting in dementia

The paper discusses the proven and probable role of different tick-borne pathogens and their combinations in the occurrence of zoonotic diseases. The variants of coexistence of various combinations of 7 proved disease agents (3 species of Borrelia, 2 species of Ehrlichia, Babesia, and TBE virus) with different, if might to be, disease agents, such as rickettsiae of two groups, which do not belong to the Borrelia burgdorferi sensu lato group spirochetes, are analyzed. The difference of their development and the time of transmission after of the vector Ixodes persulcatus Schulze attachment are c

AIM: Crimean-Congo haemorrhagic fever (CCHF) generally affects many organs. In this study a retrospective evaluation of the pulmonary findings of CCHF patients was made.
MATERIALS AND METHODS: The records of patients were retrospectively analysed. For each patient an evaluation was made of age, sex, occupation, place of residence, contact with ticks, smoking history, and pulmonary radiological and laboratory findings. A confirmed diagnosis of CCHF was made on determination by enzyme-linked immunosorbent assay or polymerase chain reaction.
RESULTS: Of the 128 patients, 48.4% were female. Symp

Rocky Mountain Spotted Fever (RMSF) is a tick-bourne illness, which can be fatal if unrecognized. We discuss the case of a patient treated with an anti-TNF-alpha inhibitor for rheumatoid arthritis who later developed a generalized erythematous macular eruption accompanied by fever. The clinical findings were suggestive of RMSF, which was later confirmed with serology. Prompt treatment with doxyclycine is recommended for all patients with clinical suspicion of RMSF.

BACKGROUND: Many vector-borne pathogens including viruses, bacteria, protozoa and nematodes occur in northeast Italy, representing a potential threat to animal and human populations. Little information is available on the circulation of the above vector-borne pathogens in dogs. This work aims to (i) assess exposure to and circulation of pathogens transmitted to dogs in northeast Italy by ticks, sandflies, and mosquitoes, and (ii) drive blood donor screening at the newly established canine blood bank of the Istituto Zooprofilattico Sperimentale delle Venezie.
METHODS: Blood samples from 150 pr

Tularemia has become a re-emerging zoonotic disease in Turkey recently. The aims of this study were to determine the seroprevalence of tularemia in humans and their animals living in rural risky areas of our region and to investigate the risk factors. Between January and July 2012, people living in rural areas of Van province (located at eastern part of Turkey) and their domestic animals were included in the study. The sample size was determined by using cluster sampling method like in an event with known prevalence and planned as a cross-sectional epidemiological study. Proportional random sa

BACKGROUND: Sand flies are small hematophagous insects known as leishmaniasis vectors. Similar to most arthropods, they harbor nonobligate endosymbionts that may influence host adaptation and pathogen transmission, but these symbiont communities remain poorly characterized in Leishmania-endemic African sand flies.
METHODS: We screened 1700 wild-caught phlebotomine sand flies (1266 females, 434 males) from Kenya's Baringo, Nakuru, and Kajiado counties, and 253 colony Phlebotomus duboscqi, for Rickettsia, Wolbachia, Spiroplasma, Cardinium, Arsenophonus, Microsporidia, and Leishmania by high-res

Tick-borne borreliosis (Lyme disease-LD) is caused by pathogenic Borrelia spirochetes that is transmitted through bite of Ixodes ticks to humans and animals. In the Russian Federation, borreliosis registered with an index of 6-7 per 100,000 people annually. In reality, LD morbidity in Russia is much higher because Russian strains develop less erythematous rashes compared to North American strains, thus missed by physicians in most of the early cases, and current serology tests have insufficient sensitivity as well. The aim of this work was to improve the sensitivity and specificity of serology

INTRODUCTION: Mediterranean spotted fever (MSF) is a tick-borne disease caused by Rickettsia conorii subspp. conorii. It is transmitted by the bite of the tick Rhipicephalus sanguineus. Modified by the rickettsial invasion, the micro-vascular endothelium acquires an activated inflammatory phenotype and initiates secretion of cytokines and expression of cell adhesion molecules (CAMs) and chemoattractans.
AIM: This study aims at investigating the alterations in the soluble cellular adhesion molecules (sCAMs) and chemokine MCP-1 levels in patients with MSF of varying severity in the acute and co

Tularemia is a zoonotic disease caused by the highly contagious bacterium Francisella tularensis, with rabbits, hares, and rodents considered as primary reservoirs. Clinical manifestations in humans can range from mild to life-threatening, depending on the mode of exposure (ingestion, inhalation, skin contact, injection, or tick bite), with high fever being a common feature. The primary aim of the present research was to evaluate the circulation of F. tularensis and the potential infection among wild boar (Sus scrofa) and red deer (Cervus elaphus) from the central region of Portugal. BACKGROUND: Acute Q fever caused by Coxiella burnetii is often missed because clinical manifestations are nonspecific, routine cultures are unrevealing, and typical zoonotic exposure may be absent. Persistent fever after correction of diabetic ketoacidosis (DKA) should prompt reassessment for atypical or intracellular pathogens.
CASE PRESENTATION: A 56-year-old man with type 2 diabetes mellitus was admitted with polyuria, polydipsia, and DKA but was afebrile and had no obvious infectious symptoms on admission. Metabolic acidosis resolved within 24 hours, but persistent high-grade fever subseq

BACKGROUND: Central nervous system (CNS) Lyme neuroborreliosis (LNB) rarely occurs. Clinical and radiological findings are heterogenous and considered unspecific, often delaying the time to diagnosis and treatment. Recently, specific clinical courses and MRI patterns have been proposed for patients presenting with brainstem encephalitis, but their relevance remains unclear.
OBJECTIVE: To describe radio-clinical features of patients with CNS-LNB encephalomyelitis, and to further investigate potential specific imaging patterns that could facilitate the diagnosis.
METHODS: We report a case of l

Herein we present the first-known case report of a structural infestation by the bird flea (Dasypsyllus gallinulae perpinnatus) (Dale 1878) in the United States. In March of 2023, the San Mateo County Mosquito and Vector Control District was contacted by a resident in South San Francisco, CA who reported the presence of fleas inside their condominium. The resident had 2 dogs who were on oral flea medication and only 1 inhabitant out of 4 reported receiving flea bites. The front walkway, backyard, and garage were flagged and a small passerine nest was removed from a fire alarm bell in the front

BACKGROUND: Serological testing of patients consulting for typical erythema migrans (EM) is not recommended in European recommendations for diagnosis of Lyme borreliosis (LB). Little is known on the level of adherence of French general practitioners to these recommendations. The objectives were to estimate the proportion of Lyme borreliosis serological test prescription in patients with erythema migrans seen in general practice consultations in France, and to study the factors associated with this prescription.
METHODS: LB cases with an EM reported by the French general practitioners (GPs) of

Tularemia caused by Gram-negative, coccobacillus bacterium, Francisella tularensis, is a highly infectious zoonotic disease. Human cases have been reported mainly from the United States, Nordic countries like Sweden and Finland, and some European and Asian countries. Naturally, the disease occurs in several vertebrates, particularly lagomorphs. Type A (subspecies tularensis) is more virulent and causes disease mainly in North America; type B (subspecies holarctica) is widespread, while subspecies mediasiatica is present in central Asia. F. tularensis is a possible bioweapon due to its lethalit
