Cervical slide tracheoplasty in adults with laryngotracheal stenosis.

OBJECTIVES: 1) Evaluate success rates for adults undergoing cervical slide tracheoplasty. 2) Examine complication rates of slide tracheoplasty in adults. METHODS: A retrospective cohort of adults > 21 years of age undergoing cervical slide tracheoplasty for tracheal stenosis between October 2011 and August 2017 was reviewed. Comorbidities, stenosis grade, etiology of stenosis, primary versus revision surgery, complications, and number of adjunct endoscopic procedures required postoperatively were evaluated. RESULTS: Nineteen patients (63% female) underwent cervical slide tracheoplasty during

Early and late outcome after surgical treatment of acquired non-malignant tracheo-oesophageal fistulae.

OBJECTIVES: Tracheo-oesophageal fistula (TOF) is a rare, life-threatening condition. We report our results of surgical treatment and evaluation of the outcome of acquired non-malignant TOF. METHODS: Twenty-five patients (aged 49 ± 21 years) with TOF were operated on between 2001 and 2011. Tracheo-oesophageal fistula was due to prolonged intubation/tracheostomy (84%), was secondary to other surgery (8%) or trauma (4%) or was idiopathic (4%). The tracheal defect was 2.4 ± 1.3 cm long and was associated with tracheal stenosis in seven (28%) patients. Surgical treatment consisted of direct suturi

T-tube insertion for sclerotic subglottic stenosis.

T-tube insertion is effective treatment for subglottic stenosis, but it is generally difficult due to bending of the T-tube. In a 52-year-old woman with relapsing polychondritis, a T-tube was inserted after predilatation using Hegar dilators. We describe the details of our T-tube insertion methods for sclerotic subglottic stenosis.

New interdisciplinary approach for removing large clivus and upper cervical spine tumors.

We have developed a new interdisciplinary approach for removing large clivus and upper cervical spine tumors. This approach is a combination of the Le Fort I osteotomy, midfacial degloving, and median labiomandibular glossotomy. Our approach gives an excellent, wide surgical field from the nasopharynx, including the base of the skull, to the base of the tongue and permits sufficiently safe extirpation of clivus and upper cervical spine tumors that may not be removed by transoral or transcervical approaches. Our approach not only incorporates the merits of each approach but also creates a large

Tetanus and its complications: intensive care and management experience in 150 Indian patients.

A total of 150 patients were treated for tetanus in the tetanus ward of the J. J. Hospital, Bombay, between October 1983 and January 1986. The complications of tetanus and the mode of management in the presence of restricted resources are outlined. Intensive care, proper nutrition, early tracheostomy and ventilator support in severe tetanus were chiefly responsible for an overall reduction in mortality from 30 to 12%. The mortality in severe tetanus was reduced from 70 to 23%. Sudden death due to unexpected cardiac arrest was an important complication in severe tetanus. We observed that an abr

Pulmonary edema complicating relief of upper airway obstruction.

Treatment by endotracheal intubation or tracheostomy in children with severe and prolonged upper airway obstruction usually results in dramatic improvement; in some rare instances, it is complicated by the development of pulmonary edema. During an eight-year period, the author observed this complication in 20 children. The mechanism of this edema is complex and not yet fully understood. In addition to hypoxia, profound hemodynamic changes occur during the inspiratory phase of the obstruction; highly negative transpulmonary pressure may lead to an increase in pulmonary blood volume and biventri

Pulmonary edema and acute upper airway obstruction.

Pulmonary edema following the relief of acute upper airway obstruction is unusual and unpredictable. The cause is usually attributed to pulmonary and hemodynamic changes resulting from high negative intrathoracic pressures during obstructed respiration. The incidence of this problem is not known. Why pulmonary edema develops in only certain patients is also unclear. We reviewed the records of 27 consecutive adult patients admitted and treated for acute upper airway obstruction. Three of 27 (11%) developed pulmonary edema. Age, sex, diagnosis, duration of obstruction, history of cardiopulmonary

Management of airway trauma. I: Tracheobronchial injuries.

One hundred six consecutive patients with injuries to the tracheobronchial tree who were admitted to the emergency room of the Tulane Medical Center Hospital or the Charity Hospital of Louisiana at New Orleans over a period of almost 20 years were analyzed retrospectively. Penetrating trauma of the neck or chest was reported in 100 of the patients, and only 6 had blunt trauma to the neck or thorax as the cause of injury. There were 18 deaths among the 106 patients (16.98%), including 11 (13.75%) of 80 with injuries of the cervical trachea. Seven (53.8%) of 13 with principal injuries of the tho

[Clinical manifestations and therapeutic tactics in acute lung abscess].

Under analysis were data of 107 patients with acute pulmonary abscesses. Such patients need a complex intensive conservative therapy. Indications for surgical treatment of acute pulmonary abscesses were considered to be concomitant complications (hemorrhage, pyopneumothorax, empyema of the pleura), atypical forms badly differentiated from a cavitary form of carcinoma, as well as suppurative processes not treated conservatively and disposed to turn into a chronic stage.

Respiratory complications of achondroplasia.

Nine patients with achondroplasia who were seen over a three-year period developed significant respiratory complications. Eight had sleep-disordered breathing, including obstructive sleep apnea in five, for which two required tracheostomy. Of the seven patients with significant hypoxemia, five had clinical evidence of cor pulmonale and recurrent pulmonary infiltrates. Two patients died, one with autopsy findings of compression of the medulla at the level of the foramen magnum and one with respiratory and cardiac failure. Appropriate therapy for our patients depended on recognition of the mecha

Laryngeal papillomatosis: anesthetic management.

We reviewed the anesthetic management of all children admitted with laryngeal papillomatosis during a six-year period. This included 147 endoscopies on 16 patients, most referred with advanced disease after initial care in other hospitals. Onset of the disease before age 2 was associated with the most severe upper airway involvement. Eight patients required tracheostomy at some time during treatment, and management of the shared airway during surgical manipulation was particularly difficult in these children. Inhalational induction with halothane was used rountinely, with surprisingly few inst

Evaluation of dynamic compliance of lungs and disturbances of breathing mechanics in patients following laryngectomy for carcinoma of the larynx.

The purpose of study was to observe changes which may develop in the mechanics of breathing following laryngectomy in patients with carcinoma of the larynx as compared with the pre-operative conditions. The studies were carried out on a group of 29 patients, performing the measurements twice, i.e. before operation and after laryngectomy and tracheostomy. The measurements were done using a Godart Compliance-Test type 121 apparatus. The value of the coefficient of dynamic compliance of the lungs was determined together with the magnitude and type of work necessary to overcome respiratory resista

Timing of tracheostomy in geriatric patients with isolated severe traumatic brain injury: A nationwide analysis.

BACKGROUND: The impact of tracheostomy timing on outcomes is unclear in geriatric patients with traumatic brain injury (TBI). METHODS: Patients aged≥65 with isolated severe TBI who underwent tracheostomy were identified in the ACS-TQIP 2017-2020 database. Patients were grouped by early (<10 days) vs. late (≥10 days) tracheostomy. Propensity score matching accounted for confounders. Outcomes included mortality, hospital (H-LOS) and ICU length of stay (ICU-LOS), ventilator days, and complications. RESULTS: Among 1385 patients, 637 (46.0 %) had an early tracheostomy. Following propensity score
