UNLABELLED: The aim of the work was an evaluation of vestibular organ as well as static and dynamic balance condition in menopausal women. We have examined 107 women aged 46 to 55 years, 47 among them with complaints of vertigo (Group I), 30 women aged 45 to 50 years with symptoms of menopause diagnosed in Outpatient of Endocrinology Clinic (Group II) and 30 healthy women aged 45-54 (control Group). Blood concentration of sexual hormones (estradiol, LH, FSH, progesterone), sodium and potassium blood level, electronystagmography (ENG), optokinesis, eye tracking test, static and dynamic posturog

OBJECTIVE/HYPOTHESIS: Vestibular evoked myogenic potential (VEMP) testing has become a well-established test to explore the sacculo-collic pathways in human. The aim of the present study is to establish a clinical indication for saccular hydrops by VEMP testing.
STUDY DESIGN: Prospective study.
METHODS: Ten patients (three men and seven women) with Meniere's disease and 10 patients (four men and six women) with sudden deafness were admitted and given pure-tone audiometry and VEMP testing daily. Interaural amplitude difference over the sum of amplitudes of both ears was measured, and when the

Vestibular evoked myogenic potentials (VEMPs) are short-latency electromyograms evoked by high-level acoustic stimuli recorded from surface electrodes over the tonically contracted sternocleidomastoid (SCM) muscle. These responses are presumed to originate in the saccule. The purpose of this preliminary report is to provide an overview of our initial experience with the VEMP by describing the responses obtained in five subjects. Click-evoked VEMPs were present at short latencies in two normal-hearing subjects, one patient with profound congenital sensorineural hearing loss, and one patient wit

OBJECTIVE: We reviewed the features of nystagmus in 24 patients with horizontal canal benign positional vertigo (BPV). Patients were treated with canalith repositioning maneuvers. Our goal was to develop a framework for distinguishing horizontal from posterior canal BPV and to further develop a mechanistic model explaining the horizontal canal variant of BPV.
STUDY DESIGN: The study design was a retrospective case review with prospective treatment outcome and follow-up.
SETTING: The study was performed at a secondary and tertiary referral center for vertigo and dizziness.
PATIENTS: The diag

BACKGROUND: The cochlear aqueduct is a route for direct pressure transfer between intracranial and intracochlear fluids. In patients with Menière's disease, intracochlear pressure is presumably disturbed. The "Tympanic Membrane Displacement Analyser (TDA)" is a new system which provides a useful noninvasive method of detecting intracranial and intracochlear pressure changes.
PATIENTS: In this study TDA measurements in combination with a glycerol test were performed in nine patients with Menière's disease and in seven normal persons.
RESULTS: Before ingestion of glycerol, no significant diffe

BACKGROUND: Cochleovestibular lesions in patients with acute myeloid leukemia or the blastic phase of chronic myeloid leukemia are usually due to leukemic infiltration, infection or hemorrhage. In contrast, the most likely cause of cochleovestibular lesions in the chronic phase of the chronic myeloid leukemia seems to be disturbed microvascular perfusion and vascular occlusion.
CASE REPORT: A 45-year old patient with a chronic myeloid leukemia presented with the sudden onset of profound deafness in his right ear combined with a total loss of vestibular function on the same side. The hemoglobi

Off-vertical axis rotation (OVAR) stimulates the otolith organs in a manner that is suitable for assessment of the otolith-ocular reflex. To further assess the potential clinical usefulness of OVAR, the eye movement responses of seven patients with surgically confirmed unilateral peripheral vestibular lesions were compared with the eye movement responses of a group of age-matched, healthy, asymptomatic control subjects. Patients and controls were tested with constant velocity rotations that followed a brief period of angular acceleration (velocity trapezoid) using either earth-vertical axis (E

Sudden cochlear hearing loss, occurring successively or simultaneously in both ears, was observed in four unrelated preschool children. Vestibular testing could be carried out in three patients and showed complete bilateral loss of function. All patients had a mildly retarded motor development due to nonprogressive muscular weakness. On MR imaging all patients showed multiple periventricular and subcortical white matter lesions. These lesions were not clearly progressive in one patient examined repeatedly over 6 years. Virological, bacteriological, immunological, and metabolic examinations wer

As many as 47 patients with different extrapyramidal hyperkinesias were examined. In addition to the methods commonly accepted in otoneurology, use was made of the method of recording short-wave auditory evoked potentials (SAEP). The parameters of SAEP were correlated to the degree of auditory and vestibular function impairment. The data obtained indicate that the majority of patients with hyperkinesias manifest a well-defined reduction of the amplitude and an increase of the latent period of the late waves of SAEP, which attests to the interest of the cerebral structures.

The statokinesigram of the 16 benign paroxysmal positional vertigo (BPPV) patients and the 28 vestibular deficits (VDS) patients were analyzed in comparison with the 20 normal subjects (NS). Velocity of the track (LNG/T), root mean square (RMS), and standard deviation area (SD-Area) were used as parameters. Fourteen different tests were done. When the result for each subject group were analyzed there were interesting differences between the compensated VDS and BPPV patients and between the untreated period and the compensated stage in each patient group. Comparing the data between NS and BPPV

A new application of the glycerin test in the diagnosis of post-traumatic perilymphatic fistulas is described. Temporary disappearance of abnormal responses to the fistula and Quix tests and improvement in the hearing occurred. The glycerin test was useful in confirming the diagnosis of post-traumatic perilymphatic fistula in 13 patients in whom fistulas were found at middle ear exploration.

A careful history is the most important part of a medical examination for vertigo, especially to establish whether it is acute, chronic or recurrent. If the patient spontaneously volunteers ear complaints, such as fullness, tinnitus, or hearing loss, the cause is probably otologic and deserves immediate referral. If no otologic complaints are volunteered, the whole body becomes a source of investigation. All nonvital drugs should be withheld while the vertigo study is progressing. A complete physical examination and blood profile should be carried out. Roentgenograms of the skull and chest sho

The main aim was to evaluate the relative importance of sensory interactions for postural stability in 45 patients with insulin-dependent diabetes mellitus (IDDM) with and without peripheral neuropathy. All subjects had normal electronystagmography. Dynamic posturography provides functional, selective testing of three sensory modalities in maintenance of balance, i.e., vestibular, visual, and somatosensory. The Sensory Organization Test (SOT) includes six test conditions during which the subject tries to maintain an upright stance with as little sway as possible. The subject stands on a movabl

According to international statistics, the first examination of 25% of patients with vertigo is carried out in Emergency Departments. The most important task of the examining physician is to diagnose life threatening pathologic processes. One of the most difficult otoneurological diagnostic challange in Emergency Departments is to differentiate between dangerous posterior scale stroke presenting with isolated vertigo and the benign vestibular neuritis. These two disorders can be safely differentiated using fast, non-invasive, evidence based bedside tests which have been introduced in the past f
