Case Report: De novo multiple intracranial aneurysms following extracranial-intracranial bypass and proximal occlusion for a giant serpentine aneurysm.

BACKGROUND: Carotid artery occlusion, whether therapeutic, iatrogenic, atherosclerotic, or congenital, induces profound hemodynamic changes in the cerebral circulation. Collateral channels within the circle of Willis compensate to maintain cerebral perfusion; however, the resulting increases in flow velocity, wall shear stress, and pressure gradients are believed to ultimately contribute to aneurysm formation. While de novo aneurysms after hunterian ligation have been described, they typically occur years after treatment. The early occurrence of multiple aneurysms following parent artery occlu

A Vessel Wall MRI Investigation in Patients With Moyamoya or Quasi-Moyamoya Disease: Diagnosis, Features, and Outcomes.

BACKGROUND AND PURPOSE: None of the previous studies have investigated the pathologic authenticity of affected arteries in moyamoya disease (MMD) and Quasi-MMD diagnosed by angiography. This study aimed to confirm the angiographic diagnosis of moyamoya as well as investigate the pathologic mechanisms in angiographically proven MMD and Quasi-MMD using high-resolution magnetic resonance imaging (MRI) in a large sample. METHODS: We prospectively studied 116 patients who had angiographically proven MMD and Quasi-MMD. Each affected internal carotid artery, and middle cerebral artery was independen

Assessment of cerebrovascular reserve with N-isopropyl-p-[123I]-iodoamphetamine time series analysis in patients with cerebrovascular disease.

Using N-isopropyl-p-[123I]-iodoamphetamine(123I-IMP) and single-photon emission computed tomography (SPECT), the relationship between cerebrovascular reserve and the 123I-IMP redistribution phenomenon was investigated. The 50 patients who matched the inclusion criteria were divided into control and ischemia groups, and the redistribution phenomenon was examined on resting images. The delayed images showed higher 123I-IMP accumulation in lesions in the middle cerebral artery(MCA) area and anterior cerebral artery(ACA) area, these watershed areas in the ischemia group than in the control group, c

[Transposition of the Left Subclavian Artery Before a Thoracic Endovascular Aortic Repair for Type B Aortic Dissection].

We encountered a patient who had undergone a transposition procedure, which encompassed the performance of subclavian-common carotid arterial end-to-side anastomosis before a thoracic endovascular aortic repair (TEVAR) when carrying out Stanford type B aortic dissection. A 50-year-old man was admitted to our hospital with a diagnosis of chronic Stanford type B aortic dissection that tended to expand. Transposition was performed in advance of zone 2 TEVAR. Lymphorrhea was observed under the skin following surgery. A decreasing tendency for lymphorrhea was confirmed at six months following the o

[Comparing the risks for the development of perioperative complications in carotid endarterectomy and carotid angioplasty].

The article is a literature review presenting a comparative analysis of 30-day risks of mortality and complications after carotid endarterectomy and carotid angioplasty with stenting. The risks studied were as follows: myocardial infarction, stroke, transitory ischaemic attacks, bradycardia, hypotension, postoperative haematomas, and damages to the craniocerebral nerves. The authors analysed a series of recently published foreign studies and meta-analyses dedicated to the problem concerned. The obtained findings revealed that carotid endarterectomy turned out to be associated with a higher per

Limb-shaking transient ischemic attacks successfully treated with external carotid artery stenting.

The external carotid artery (ECA) is one of the most important extracranial-to-intracranial sources of collateral circulation, contributing significantly to the cerebral blood flow especially when perfusion through the internal carotid artery (ICA) is compromised. Most of the endovascular studies so far have been dedicated to ICA, with little focus on the ECA. Limb-shaking transient ischemic attacks (TIAs) are a relatively rare manifestation of carotid artery disease that may present with repetitive shaking movements of the affected limbs. We report a case of an 80-year-old male with bilateral

Proximal Middle Cerebral Artery Aneurysms: Microsurgical Management and Therapeutic Results.

BACKGROUND: Management of proximal (M1) middle cerebral artery (MCA) aneurysms can be challenging. Because of the diversity of aneurysm morphology and location of M1 aneurysms, the surgical strategy should be tailored to each individual case, especially for complex cases. We assessed the safety and efficacy of an algorithmic strategy for the management of M1 aneurysms. METHODS: A prospective single-center study of M1 aneurysms treated with microsurgery was performed. Our algorithmic strategy was determined by the location and complexity (e.g., size, shape) of the aneurysms. The surgical modal

Left Vertebral Artery to Common Carotid Artery Transposition in a Patient With Bilateral Vertebral Insufficiency: 3-Dimensional Operative Video.

Extracranial vertebral artery (VA) atherosclerosis is responsible for 14% to 32% of posterior circulation infarctions.1 In the posterior circulation, narrowing of the VA > 30% is significantly associated with strokes. Subclavian artery (SCA) atherosclerosis can produce subclavian steal. Retrograde VA flow around an occluded SCA decreases blood flow to the posterior circulation and causes vertebrobasilar insufficiency (VBI). Flow augmentation to the posterior circulation can be achieved by VA endarterectomy, arterial stenting, VA-common carotid artery (CCA) transposition, or bypass using an int

Occipital Artery to Posterior Cerebral Artery Bypass Using Descending Branch of the Lateral Circumflex Femoral Artery Graft for Treatment of Fusiform, Unruptured Posterior Cerebral Artery Aneurysm: 3-Dimensional Operative Video.

Posterior cerebral artery (PCA) aneurysms can be technically challenging lesions due to the intricacy of perforating branches and the relationship to cranial nerves and the brainstem. Fusiform aneurysms of the perimesencephalic segment of the PCA are a rare finding which does not favor direct clip occlusion or reconstruction. In such cases, proximal parent vessel occlusion is an option for aneurysm treatment. Extracranial-intracranial (EC-IC) bypass can be used to revascularize beyond the lesion when considering proximal occlusion. Based on previous literature for occipital artery (OA) bypass

Novel insights into symptomatology of moyamoya disease in pediatric patients: survey of symptoms suggestive of orthostatic intolerance.

OBJECTIVE A specific population of young patients with moyamoya disease (MMD) persistently experience physical symptoms not attributable to focal ischemia. These symptoms, highly suggestive of orthostatic intolerance (also termed "orthostatic dysregulation"), were investigated and reported as potential determinants of quality of life in young MMD patients. METHODS Forty-six patients (6-30 years of age) were selected from a group of 122 patients who were diagnosed with MMD before 18 years of age. The authors administered a structured questionnaire consisting of 11 items based on screening check

Silent Cerebral Microbleeds and Longitudinal Risk of Renal and Cardiovascular Events in Patients with CKD.

BACKGROUND AND OBJECTIVES: In the general population, the presence of cerebral microbleeds on T2*-weighted magnetic resonance imaging has been reported to be a predictor of future stroke. Patients with CKD have a high prevalence of microbleeds and are at higher risk of ESRD as well as cardiovascular disease, including stroke. Because endothelial dysfunction is the common pathophysiology among microbleeds, CKD, and cardiovascular disease, we hypothesized that the presence of microbleeds would be an important predictor of composite outcome, including both cardiovascular disease and renal events,

Intra-arterial thrombolysis of occluded middle cerebral artery by use of collateral pathways in patients with tandem cervical carotid artery/middle cerebral artery occlusion.

BACKGROUND AND PURPOSE: Cervical internal carotid artery (ICA) occlusion with middle cerebral artery (MCA) embolic occlusion is associated with a low rate of recanalization and poor outcome after intravenous thrombolysis. Prompt revascularization is required to prevent disabling stroke. We report our experience on acute ischemic stroke patients with tandem ICA or MCA occlusions treated with microcathether navigation and intra-arterial thrombolysis by use of collateral pathways including the posterior or anterior communicating arteries, or both pathways. MATERIALS AND METHODS: We retrospective

Endovascular Treatment of Urgent Carotid Occlusion. Case selection & Technique.

Emergency revascularization of acute carotid artery occlusion is still controversial. We treated 15 patients (13 men and two women, mean age of 67.3 years) with acute atherosclerotic carotid occlusion by endovascular procedures and evaluated the usefulness of this treatment. All of the patients were evaluated with emergency MRI and MRA before treatment. Intracranial tandem arterial occlusion due to distal embolism was observed in nine patients, and contralateral carotid stenosis (> 70%) was observed in seven. The mean NIHSS score of the patients was 15.4 +/- 7.4 (mean +/- SD) before treatment.

Aspirin in the primary prevention of angina pectoris in a randomized trial of United States physicians.

PURPOSE: The objective of this study was to examine the effect of low-dose aspirin (325 mg on alternate days) on the primary prevention of angina pectoris in the United States Physicians' Health Study. Despite a postulated role of platelets in atherogenesis and myocardial ischemia, the effect of chronic platelet inhibition on the development of clinical angina pectoris is unknown. SUBJECTS AND METHODS: The Physicians' Health Study is a randomized, double-blind, placebo-controlled trial among 22,071 male physicians aged 40 to 84 years, free from previous myocardial infarction, stroke, and tran

Impact of genetic profiles on periventricular anastomosis following bypass surgery in moyamoya disease.

Postoperative regression of periventricular anastomosis (PA) may have important implications for preventing hemorrhagic events in patients with moyamoya disease (MMD). We aimed to identify the genetic factors associated with postoperative PA regression. This retrospective cohort study enrolled 81 patients (102 hemispheres) who underwent combined revascularization surgery. PA types (lenticulostriate, choroidal, and thalamic) were scored from 0 to 2 preoperatively and postoperatively using magnetic resonance angiography, and postoperative regression was evaluated. We extracted all exonic variant

Association between pre-operative long-term aspirin use and post-operative outcomes following extracranial-intracranial bypass for moyamoya disease: National Inpatient Sample analysis.

OBJECTIVE: Moyamoya disease (MMD) carries significant morbidity. While management focuses on treatment with aspirin and Extracranial-intracranial (EC-IC) bypass, the impact of pre-operative long-term aspirin use on outcomes remains unclear. Our objective was to evaluate the association of long-term aspirin use with in-hospital outcomes in patients with MMD undergoing EC-IC bypass. METHODS: We conducted a retrospective study using the National Inpatient Sample (2002-2020) to identify adult patients with MMD who underwent EC-IC bypass. Patients were stratified into 2 groups based on long-term a
