In cases where a deceased child exhibits trauma as a result of a physical abuse blunt impact load, a parent/caregiver may provide a simple short fall (SSF) as the justification for that trauma. The skeletal fractures remain difficult to differentiate between a SSF and physical abuse however, as both are the result of a blunt impact load, and are therefore biomechanically alike, and the rare nature of these fatalities means only anecdotal research has been available to validate such claims. The aim of this pilot study was to investigate if there may be differences in the skeletal fracture patte

BACKGROUND: The number of elderly patients sustaining long bone fractures is increasing with the rise of elderly population in the western world. Management of distal femur fractures is particularly difficult due to osteoporosis and other associated comorbidities. The key to management would be by surgical stabilization, which allows early mobilization. This study was devised to look into the radiological outcome and complication rate in a series of elderly patients who were treated with retrograde nail using spiral locking blade system for extra articular distal femur fractures.

Supracondylar humerus fractures are common fractures around the elbow. Open fractures, comminuted metaphyseal fractures and also fractures with bone loss may need to be treated with external fixators. The aim of this study is to compare two different external fixators, tubular external fixators and polymethyl methacrylate with K wires external fixators, with regard to stiffness and stability for metaphyseal-diaphyseal osteotomies of distal humerus close to the joint. Six matched pairs (24 specimens) of second-generation sawbone humerus were prepared in a standard fashion to create a metaphysea…

REASONS FOR PERFORMING STUDY: To enhance understanding of the nature and pathogenesis of scapular fractures in racehorses.
HYPOTHESIS: Scapular fractures in racehorses have a consistent configuration related to sites of pre-existing stress modelling and remodelling.
METHODS: Fractured and intact scapulae collected post mortem were examined visually and with computed tomography (CT). Scapular fracture configuration, bone modelling changes and standardised CT morphometry and density measurements were recorded. Statistical comparisons were made between fractured, nonfractured contralateral and

BACKGROUND/AIM: Mandibular condylar fractures represent 25%-35% of all mandibular fractures. Despite profound research, there is still a controverse debate about treating these fractures conservatively or by open reduction and internal fixation (ORIF). The aim of this study is to analyse the outcome after open and closed treatment of extracapsular mandibular condyle fractures regarding general characteristics, post-treatment malocclusion, facial nerve palsy (FNP), maximum mouth opening (MMO) and parotid complications.
METHODS: A retrospective cohort of 377 fractures (350 open, 27 closed treat

UNLABELLED: Coronal shear fractures of the distal femoral condyles are rare intra-articular injuries, and the simultaneous occurrence of a non-conjoint bicondylar Hoffa fracture with a partial anterior tibial tubercle avulsion and a medial epicondyle avulsion constitutes an exceptionally complex traumatic triad. We report the case of a 35-year-old male motorcyclist who sustained this "floating" articular block injury (AO/OTA 33-B3.3) following a high-velocity accident. Diagnostic assessment utilizing Computed Tomography with 3D reconstruction revealed a vertical Letenneur Type II fracture of t

PURPOSE: This study evaluates the clinical and radiological outcomes of a hybrid surgical strategy combining separate vertical wiring (SVW) with tension-band wiring (TBW) for complex patellar fractures involving both inferior-pole comminution and articular-body extension. METHODS: Fourteen consecutive patients treated with the hybrid SVW+TBW technique were retrospectively reviewed. Outcomes included radiographic union, patellar height (Insall–Salvati ratio), and functional recovery (Lysholm scores and ROM) at serial follow-ups up to 12 months. RESULTS: All fractures achieved bony union at a me

Intra-articular fracture dislocations of the base of the middle phalanx are complex and debilitating injuries that present a management conundrum when nonreconstructable. Hemi-hamate arthroplasty (HHA) is a treatment modality of particular use in the setting of highly comminuted fractures. This systematic review aims to summarize the reported outcomes of HHA in this context. A literature search was conducted using MEDLINE, Embase, and PubMed, yielding 22 studies with 235 patients for inclusion. The weighted mean postoperative range of movement at the proximal interphalangeal joint was 74.3° (r

Maintaining the calcaneal length after calcaneal fractures is vital to restoring the normal biomechanics of the foot, because it acts as an important lever arm to the plantarflexors of the foot. However, estimation of the length of the calcaneus to be reconstructed in comminuted calcaneal fractures can be difficult. We propose a new method to reliably estimate the calcaneal length radiographically by defining the calcaneotalar length ratio. A total of 100 ankle radiographs with no fracture in the calcaneus or talus taken in skeletally mature patients were reviewed by 6 observers. The anteropos

Extensor mechanism disruption, whether due to patella fracture or tendon rupture, generally occurs after low-energy trauma and frequently involves an indirect mechanism. When the fracture is comminuted and reconstruction is impossible, a partial or total patellectomy may be indicated. Although some authors advocate total patellectomy, partial patellectomy remains the standard treatment, especially for young and active patients. In the rare instance of a failed tendon repair after partial or total patellectomy, inadequate tissue is usually available for adequate restoration of the extensor mech

Fluoroscopy monitors intra-articular distal radial fracture reduction and stabilisation. The reduction is guided by traction, ligamentotaxis and manipulation, and when necessary, completed by minimally invasive percutaneous or limited open instrumentation. Kirschner wires effectively splint the reduction until fracture callus is visualised on X-ray. An occasional mini plate is required to buttress a displaced volar medial lunate facet (die punch) fragment into position. First, major metaphyseal articular fragments are approximated to restore the articular surface. Smaller fragments follow thei

CASE: A 17-year-old boy presented after a gunshot wound; computed tomography showed a comminuted posterior-wall acetabular fracture and a bullet 90% embedded. Through a Ganz surgical dislocation, a standard 2.5 mm drill bit was advanced 8 to 10 mm into the projectile, its flutes functioning as a self-tapping screw to rock and extract the bullet en bloc, followed by trochanteric osteotomy fixation. He regained painless full activity by 2 months; 33-month imaging confirmed durable union with no osteolysis.
CONCLUSION: The "Motown Corkscrew" technique provides a cost-neutral, controlled method t

Surgeons periodically encounter challenging clinical scenarios that require them to develop nuanced management strategies to achieve the best outcome for the patient. This is especially true in medically underserved patient populations, where follow-up and proper recovery protocols are often not accomplished. In this report, we discuss the case of a 26-year-old female with a history of medical non-compliance who presented to the emergency department with signs and symptoms of surgical site infection two months following the repair of her comminuted ulna fracture caused by a gunshot wound. The

An 18-year-old male driving an off-road motorcycle died from complete decapitation when his motorcycle tore a roadblock chain from its attachment. The decapitation injuries of the head and the torso corresponded perfectly, without apparent loss of tissue. The severance plane passed horizontally through the upper cervical region and C4, which sustained a comminuted fracture. The sharply delineated severance edge had a band-like abrasion. The decapitation resulted from the rotational movement of the unstrung chain, which struck and strangled the driver's neck. We were able to explain the movemen

Three patients were treated with external fixation with iliofemoral distraction as an alternative treatment of comminuted central fracture-dislocations of the hip. The patients were mobilized on the first postoperative day and discharged from the hospital an average of 10 days postaccident. The frame was left in place for 6 weeks while the traction was maintained. Full weight bearing was started 3 months after the injury. A full range of motion in the injured hip was achieved at 4 months postoperation. No signs of avascular bone necrosis were seen during follow-up for up to 2 years. The clinic

INTRODUCTION: Acetabular fractures in elderly patients are often the result of low-energy trauma and are complicated by osteopenia, comorbidities, and limited physiologic reserve. Minimally invasive approaches, such as percutaneous fixation have emerged as viable alternatives to open reduction in selected fracture patterns.
CASE REPORT: We report the case of a 76-year-old male with a history of coronary artery disease, hypertension, hyperlipidemia, and osteopenia who sustained a comminuted right anterior column and posterior hemitransverse acetabular fracture following a ground-level fall. Hi
