Cerebrospinal fluid vasopressin in neurological and psychiatric disorders.

Vasopressin was determined in CSF and plasma of 243 patients with different neurological and psychiatric disorders, including control patients. CSF vasopressin was significantly higher in patients with high pressure hydrocephalus, intracranial tumour, benign intracranial hypertension, intracranial haemorrhage, ischaemic stroke, and craniocerebral trauma. In patients with primary degenerative dementia, CSF vasopressin was lower than in control patients. Among patients with psychiatric disorders, CSF vasopressin was increased in manic patients, while in patients with depression CSF concentration

Serum neurofilament light chain in hydrocephalus and surgical controls: baseline comparison and 24-hour perioperative dynamics under general anesthesia.

Background/Objectives neurofilament light chain (NfL) is a blood-accessible marker of neuroaxonal injury. We assessed the clinical utility of serum NfL in adults with idiopathic normal-pressure hydrocephalus (iNPH). Methods in a single-center prospective cohort (registered at ClinicalTrials.gov; partial overlap with a prior registry detailed in Supplementary Table S1), we enrolled 41 adults aged ≥ 50 years. The hydrocephalus arm included only anticipated ventriculoperitoneal (VP) shunt responders (Group A); non-hydrocephalus surgical patients served as controls (Group B). The primary analysis

MRI imaging characteristics of brainstem and midbrain aqueduct in patients with iNPH.

Idiopathic normal pressure hydrocephalus (iNPH) is frequently difficult to diagnose due to the absence of specific symptoms, yet early detection and surgical intervention are essential for preventing sequela such as irreversible dementia. This study explores the specific magnetic resonance imaging (MRI) features of the brainstem and mesencephalic aqueduct in patients with iNPH. Head MRI data of 50 iNPH patients and 30 healthy matched controls were compared for mesencephalic aqueduct length, diameter, and angle, structural features of the brainstem at the sagittal plane, brainstem component vol

Comparison of comfortable and maximum walking speed in the 10-meter walk test during the cerebrospinal fluid tap test in iNPH patients: A retrospective study.

BACKGROUND: The 10-meter walking test (10 MWT) is widely used during a cerebrospinal fluid tap test (CSFTT) for idiopathic normal-pressure hydrocephalus (iNPH). However, various previous studies and guidelines do not specify whether to adopt a comfortable walking speed or maximum walking speed when implementing the 10 MWT. In this study, we analyzed the values of comfortable and maximum walking speeds during the CSFTT in patients who underwent shunt surgery to determine which walking form is desirable for evaluation. METHODS: The patients were 29 consecutive cases in which a CSFTT was perform

Usefulness of Brain Positron Emission Tomography with Different Tracers in the Evaluation of Patients with Idiopathic Normal Pressure Hydrocephalous.

Idiopathic normal pressure hydrocephalus (iNPH) is the only form of dementia that can be cured by surgery. Its diagnosis relies on clinical and radiological criteria. Identifying patients who can benefit from surgery is challenging, as other neurological diseases can be concomitant or mimic iNPH. We performed a systematic review on the role of positron emission tomography (PET) in iNPH. We retrieved 35 papers evaluating four main functional aspects with different PET radiotracers: (1) PET with amyloid tracers, revealing Alzheimer's disease (AD) pathology in 20-57% of suspected iNPH patients, c

Are gait changes linked to CSF flow changes in the sagittal sinus?

PURPOSE: To identify if specific findings on magnetic resonance imaging (MRI) cerebrospinal fluid (CSF) flow studies can be utilised to identify which patients with idiopathic normal pressure hydrocephalus (iNPH) will have improved gait following a CSF tap test (TT). METHODS: Prospective study of patients undergoing a CSF TT for iNPH. Functional gait was assessed using the timed up and go (TUG) test before and after the CSF TT. MRI CSF flow studies accompanied the CSF TT. The minimum clinically important difference for the TUG (3.63 s) was used as a cutoff value to categorise patients as resp

Fluoroscopic-Guided Paramedian Approach for Lumbar Catheter Placement in Cerebrospinal Fluid Shunting: Assessment of Safety and Accuracy.

BACKGROUND: Spinal catheter insertion in lumboperitoneal (LP) shunt surgery for idiopathic normal pressure hydrocephalus (iNPH) is frequently associated with technical difficulties especially in patients with obesity and elderly patients with vertebral deformities. OBJECTIVE: To elucidate the accuracy and safety of image-guided spinal catheter placement using a paramedian approach (PMA). METHODS: We retrospectively analyzed 39 consecutive iNPH patients treated by LP shunting with spinal catheter insertion via the PMA. The success rate of catheter placement and the number of changes in punctu

A unique glycan-isoform of transferrin in cerebrospinal fluid: A potential diagnostic marker for neurological diseases.

BACKGROUND: Cerebrospinal fluid (CSF) is sequestered from blood by the blood-brain barrier and directly communicates with brain parenchymal interstitial fluid, leading to contain specific biomarkers of neurological diseases. SCOPE OF REVIEW: CSF contains glycan isoforms of transferrin (Tf): one appears to be derived from the brain and the other from blood. MAJOR CONCLUSIONS: CSF contains two glycan-isoforms; brain-type Tf and serum-type Tf. Glycan analysis and immunohistochemistry suggest that serum-type Tf having α2, 6sialylated glycans is derived from blood whereas brain-type Tf having Glc

Intraventricular hematoma, subarachnoid hematoma and spinal epidural hematoma caused by lumbar puncture: an unusual complication.

Lumbar puncture is a commonly practiced bedside technique for acquiring cerebrospinal fluid for the purposes of examination, spinal anesthesia, and as therapeutic trial for normal pressure hydrocephalus. Headache and backache after lumbar puncture are not uncommon. We report an elderly woman who suffered from altered consciousness and acute neurologic deficit after a difficult lumbar puncture. Serial imaging studies revealed active bleeding from the left first lumbar artery with the formation of spinal epidural hematoma and coexisting acute cranial intraventricular hematoma and subarachnoid he

CSF somatostatin-like immunoreactivity in dementia.

Concentrations of somatostatin-like immunoreactivity (SLI) in CSF were reduced in Alzheimer's disease (AD) and multi-infarct dementia (p less than 0.01), but not in normal-pressure hydrocephalus, Parkinson's disease, and Huntington's disease. This suggests that reduced SLI content in AD cerebral cortex is reflected in CSF. Chromatographic characterization of CSF SLI showed no differences between AD and controls. Concentrations of SLI in AD patients overlapped those in both normal subjects and patients with multi-infarct dementia, so that changes in CSF SLI have no diagnostic specificity.

Our initial experience with ventriculo-epiplooic shunt in treatment of hydrocephalus in two centers.

INTRODUCTION: Hydrocephalus represents impairment in cerebrospinal fluid (CSF) dynamics. If the treatment of hydrocephalus is considered difficult, the repeated revisions of ventriculo-peritoneal (VP) shunts are even more challenging. OBJECTIVE: The aim of this article is to evaluate the efficiency of ventriculo-epiplooic (VEp) shunt as a feasible alternative in hydrocephalic patients. MATERIAL AND METHODS: A technical modification regarding the insertion of peritoneal catheter was imagined: midline laparotomy 8-10cm long was performed in order to open the peritoneal cavity; the great omentu

Surgical management of hydrocephalic dementia in Paget's disease of bone: the 6-year outcome of ventriculo-peritoneal shunting.

Paget's disease of bone is a chronic progressive skeletal disorder usually occurring in the long bones and skull of older adults and elderly persons. In the skull, softening of the skull base may lead to basilar impression and consequently obstruction of the cerebrospinal fluid through the basilar cisterns, resulting ventricular enlargement in association with gait difficulties, incontinence and dementia: a syndrome resembling normal pressure hydrocephalus. The optimal management of hydrocephalus associated with Paget's disease of the skull is not well documented and is still debated. We repor

Phase contrast cine magnetic resonance imaging: normal cerebrospinal fluid oscillation and applications to hydrocephalus.

Two-dimensional cine phase contrast (2D-cine PC) magnetic resonance (MR) imaging is an imaging technique that sensitizes MR images to velocity changes in a specific direction while cancelling signals from stationary protons and from motion in other directions. This article reviews the technique, conventions, and applications of this imaging technique to hydrocephalus. 2D-Cine PC and additional more rapid MR imaging methods now in development will allow clinicians to measure CSF velocity, pulsation, and bulk flow noninvasively in normal individuals and in patients with a variety of pathologic c

A novel technology to model pressure-induced cellular injuries in the brain.

BACKGROUND: Elevated intracranial pressure (ICP) accompanying a number of neurological emergencies is poorly understood, and lacks a model to determine cellular pathophysiology. This limits our ability to identify cellular and molecular biomarkers associated with the pathological progression from physiologic to pathologic ICP. NEW METHOD: We developed an ex vivo model of pressure-induced brain injury, which combines 3D neural cell cultures and a newly developed Pressure Controlled Cell Culture Incubator (PC3I). Human astrocytes and neurons maintained in 3D peptide-conjugated alginate hydrogel

Prolonged intrauterine transabdominal ventricular external drainage. A method to decompress dilated fetal ventricles.

Advances in diagnostic procedures to detect intrauterine hydrocephalus have compelled neurosurgeons to intervene in order to preserve precious neural tissue. We present the first case (to the best of our knowledge) of a successful decrease in intracranial pressure and head circumference by repeated transabdominal intrauterine external drainage. The mother tolerated the procedure well. No complication occurred from the procedure. This method may temporarily decrease the intracranial pressure and head circumference in utero, preserve neural tissue and facilitate normal vaginal delivery. The lite

ECT in the presence of increased intracranial pressure and respiratory failure: case report.

Despite the safety of electroconvulsive therapy (ECT) in the presence of such potentially dangerous CNS disorders as CNS infections, brain tumor, and normal pressure hydrocephalus, increased intracranial pressure is still considered an absolute contraindication. In addition, although ECT has been administered to patients with pneumonia, it has never been used when respiratory failure is present. The safe and effective use of ECT in a patient in whom life-threatening refusal to cooperate with medical therapy appeared to be caused by a combination of depression and organic brain disease is repor

Optic glioma with progressive occlusion of the aqueduct of Sylvius in monozygotic twins with neurofibromatosis.

Monozygotic twins sisters with optic glioma "in mirror image" (one with involvement of the left optic nerve and the other with the right optic nerve) and hydrocephalus secondary to progressive stenosis of the aqueduct have been found in a series of 128 cases below 14 years of age with neurofibromatosis. The optic glioma was diagnosed in each of the twins at 2 years of age. In one twin the tumor involved only the optic nerve but in the other the glioma affected the optic nerve and spread to the homolateral zone of the optic chiasm. First symptoms of hydrocephalus appeared at 8 years and 11 year

Experimental transcerebral fistula. Perineural olfactory CSF flow in the normal, hydrocephalic, and postoperative hydrocephalic dog shown by radionuclide ventriculography.

Cerebrospinal fluid dynamics were studied in eight dogs during normal, hydrocephalic, and postoperative phases. Radionuclide-labeled substances introduced into the normal ventricular system flow out of the exits from the fourth ventricle to the convexity subarachnoid spaces superiorly to be absorbed in the sagittal sinus, and basorostrally to the exits from the perineural olfactory sheath into the nose to produce physiological cerebrospinal fluid (CSF) rhinorrhea. Venous opthalmodynamometry: a noninvasive method for assessment of intracranial pressure.

OBJECT: The goal of this study was to examine the potential use of ophthalmodynamometry in the noninvasive assessment of intracranial pressure (ICP). Under normal conditions, pressure within the central retinal vein is equal to or greater than ICP, because the central retinal vein basses through the optic nerve before it drains into the cavernous sinus. The optic nerve sheath is the place where ICP affects retinal venous pressure. Suction ophthalmodynamometry is an established method of investigation in ophthalmology to determine the pressure of the central retinal artery. Although observation

Why don't ventricles dilate in pseudotumor cerebri? A circuit model of the cerebral windkessel.

OBJECTIVE: Pseudotumor cerebri is a disorder of intracranial dynamics characterized by elevated intracranial pressure (ICP) and chronic cerebral venous hypertension without structural abnormalities. A perplexing feature of pseudotumor is the absence of the ventriculomegaly found in obstructive hydrocephalus, although both diseases are associated with increased resistance to cerebrospinal fluid (CSF) resorption. Traditionally, the pathophysiology of ventricular dilation and obstructive hydrocephalus has been attributed to the backup of CSF due to impaired absorption, and it is unclear why backu
