A lightweight TcrLM model predicts T cell receptor and epitope binding specificity.

Immune responses depend on specific interactions between T cell receptors (TCRs) and peptides presented by antigen-presenting cells (APCs). The vast diversity of the TCR repertoire makes accurate prediction of TCR-antigen binding specificity highly challenging. Here, we present a lightweight, masked language model, tcrLM, to address this problem. We pretrain tcrLM on a large-scale TCR CDR3 sequence dataset and use the pretrained encoder to extract informative features for pTCR binding prediction. tcrLM achieves competitive performance on hold-out and external test sets and shows comparatively

Language Recovery after Brain Injury: A Structural Network Control Theory Study.

Aphasia recovery after stroke depends on the condition of the remaining, extralesional brain network. Network control theory (NCT) provides a unique, quantitative approach to assess the interaction between brain networks. In this longitudinal, large-scale, whole-brain connectome study, we evaluated whether controllability measures of language-related regions are associated with treated aphasia recovery. Using probabilistic tractography and controlling for the effects of structural lesions, we reconstructed whole-brain diffusion tensor imaging (DTI) connectomes from 68 individuals (20 female, 4

SurgAnt-ViVQA: learning to anticipate surgical events through GRU-driven temporal cross-attention.

PURPOSE: Anticipating forthcoming surgical events is vital for real-time assistance in endonasal transsphenoidal pituitary surgery, where visibility is limited and workflow changes rapidly. Most visual question answering (VQA) systems reason on isolated frames with static vision-language alignment, providing limited support for forecasting forthcoming steps, instrument needs, or remaining procedure time. Existing surgical VQA datasets likewise focus primarily on the current scene rather than the near future. METHODS: We introduce PitVQA-Anticipation, a VQA dataset designed for forward-looking

Patient Characteristics Associated With Telehealth Use in a Large Federally Qualified Health Center.

INTRODUCTION: The COVID-19 pandemic rapidly expanded telehealth services when in-person care was limited. Using primary care data from a large federally qualified health center in Washington state, this study aimed to: 1) describe trends in telehealth use among adult patients, and 2) provide disaggregated patient characteristic data associated with telehealth use, with a focus on patients speaking Asian Indian languages. METHODS: The authors conducted a retrospective cross-sectional study using electronic health data from adult patients seen at the partner health center between January 1, 202

Cross-Modal self-supervised vision language pre-training with multiple objectives for medical visual question answering.

Medical Visual Question Answering (VQA) is a task that aims to provide answers to questions about medical images, which utilizes both visual and textual information in the reasoning process. The absence of large-scale annotated medical VQA datasets presents a formidable obstacle to training a medical VQA model from scratch in an end-to-end manner. Existing works have been using image captioning dataset in the pre-training stage and fine-tuning to downstream VQA tasks. Following the same paradigm, we use a collection of public medical image captioning datasets to pre-train multimodality models

Eye movement measures of word-level and text-level fluency in disordered reading: A comparison of schizophrenia and dyslexia.

Deficits in language processing, executive functioning, and oculomotor control have been extensively reported in schizophrenia, all of which are crucial for skilled reading. However, only recently have studies identified reading impairments in schizophrenia, including more effortful eye movement reading behaviour relative to controls. Notably, similar deficits across these domains have also been reported in dyslexia (reviewed in Whitford et al., 2018). Given this overlap in reading-related systems, a recent eye-tracking study from our group directly compared global (sentence-level) reading beh

Generative AI in Medicine and Healthcare: A Comprehensive Review of Foundational Technologies, Clinical Applications, and Future Perspectives.

Generative Artificial Intelligence (AI) is poised to induce a paradigm shift in medicine and healthcare. This review provides a comprehensive overview of its foundational technologies, clinical applications, and pathways to practical integration. We first explain the principles of three core technologies: Transformers, exemplified by Large Language Models (LLMs); Diffusion Models for high-fidelity data generation; and Neural Radiance Fields (NeRF) and 3D Gaussian Splatting (3DGS) for 3D scene synthesis. We then systematically review their applications across diverse domains: automating clinica…

Future-proofing agricultural research: FAIR principles for agriculture AI agents (FAIR4AG2).

The rapid shift of large language models from conversational use to agentic reasoning is changing how scientific outputs must be structured for machine consumption. Agriculture stands to gain the most from this transition but currently has the least of the centralized, machine-ready infrastructure that biomedicine has built over decades. Agricultural knowledge remains dispersed across peer-reviewed journals, extension bulletins, technical reports, and multimedia field demonstrations, with associated code, data, and models often inaccessible to autonomous agents. We argue that the foundational …

Evidence-based Knowledge Synthesis and Hypothesis Validation: Navigating Biomedical Knowledge Bases via Explainable AI and Agentic Systems.

The scale of biomedical knowledge, spanning scientific literature and curated knowledge bases, poses a significant challenge for investigators in processing, evaluating, and interpreting findings effectively. Large Language Models (LLMs) have emerged as powerful tools for navigating this complex knowledge landscape but may produce hallucinatory responses. Retrieval-Augmented Generation (RAG) is essential for identifying relevant information to enhance accuracy and reliability. Generative AI in hepatology: Transforming multimodal patient-generated data into actionable insights.

Cirrhosis care is inherently complex, marked by a high risk of acute decompensation and significant morbidity and mortality. Traditional episodic care models provide static snapshots of a patient's condition, limiting the ability to address dynamic changes in clinical status. Emerging at-home monitoring technologies and wearable devices present numerous opportunities to generate continuous clinical data, but the integration of this data into clinical workflows remains challenging. Large language models (LLMs) and generative AI (GenAI) technologies offer innovative solutions by enabling the acq…

On-device query intent prediction with lightweight LLMs to support ubiquitous conversations.

Conversational Agents (CAs) have made their way to providing interactive assistance to users. However, the current dialogue modelling techniques for CAs are predominantly based on hard-coded rules and rigid interaction flows, which negatively affects their flexibility and scalability. Large Language Models (LLMs) can be used as an alternative, but unfortunately they do not always provide good levels of privacy protection for end-users since most of them are running on cloud services. To address these problems, we leverage the potential of transfer learning and study how to best fine-tune light…

The precision ceiling: information-theoretic accuracy bounds for responsible AI health screening.

Large language models pass licensing examinations and foundation models match human diagnostic accuracy, yet the sensing modalities that supply these algorithms with data obey unchanged physical limits. This article introduces the precision ceiling: the maximum accuracy attainable by any model operating on a given sensing modality, bounded by the information-theoretic capacity of the measurement channel rather than by algorithmic capacity. Grounded in the data processing inequality, the ceiling is an a priori constraint whose clinical magnitude must be empirically characterized per modality. W…

Fine-Tuning DeepSeek-OCR‑2 for Molecular Structure Recognition.

Optical chemical structure recognition (OCSR) is critical for converting 2D molecular diagrams from printed literature into machine-readable formats. While vision-language models have shown promise in end-to-end OCR tasks, their direct application to OCSR remains challenging, and direct full-parameter supervised fine-tuning often fails. In this work, we adapt DeepSeek-OCR-2 for molecular optical recognition by formulating the task as an image-conditioned SMILES generation. To overcome training instabilities, we propose a two-stage progressive supervised fine-tuning strategy: starting with para
