Variants and mimics of Guillain Barré Syndrome.

BACKGROUND: The clinical and pathologic spectrum of Guillain Barre Syndrome (GBS) has expanded to include both demyelinating and axon loss forms. GBS may also have atypical presentations. For these reasons, clinicians are more likely to overlook unrelated disorders that mimic GBS. REVIEW SUMMARY: In this article, the classic presentation and variants of GBS are briefly reviewed. Disorders that mimic GBS are reviewed in detail, including those caused by neurotoxins, heavy metals, chemical toxins, drugs, vasculitis, hereditary disorders, infections, critical illness, and myelopathy. Clinical manifestations of photosensitivity in patients with human immunodeficiency virus infection.

A number of light-induced pathologic changes in the skin of individuals with HIV infection have been reported in the dermatologic literature. The relationship between HIV and one of these more well-defined types of photosensitivity, PCT, while still uncertain seems definable and related to an infectious origin. The relationship of retrovirus infection and photosensitivities that are of idiopathic origin in HIV-infected individuals, as well as non-infected individuals, is much more conjectural. The association seems, however, to be more frequent than co-incidental, and the link probably resides

Erythropoietic protoporphyria: therapeutic response to combined erythrocyte exchange and plasmapheresis.

Three patients with erythropoietic protoporphyria were treated with combined plasmapheresis and neocyte exchange transfusions that induced and maintained clinical and biochemical remissions. Red cell and plasma porphyrin levels normalized and patients developed a more normal tolerance to ultraviolet exposure. This treatment modality was an effective method of reversing other newly described abnormalities such as excessive erythropoiesis, abnormal RBC osmotic fragilities, elevated plasma viscosities, and hepatic and renal impairments.

Coexistence of Mycosis Fungoides and Photosensitive or Autoimmune Diseases. The Therapeutic Challenge: A retrospective Case Series from a Tertiary Referral Center.

BACKGROUND: Mycosis fungoides (MF) combined with photosensitive/autoimmune diseases has been reported, yet there are limited data regarding the therapeutic considerations in these patients, specifically phototherapy, a mainstay skin-directed treatment (SDT), being a relative or complete contra-indication. OBJECTIVES: To outline therapeutic considerations for patients with MF who had also been diagnosed with photosensitive/autoimmune diseases. METHODS: We conducted a retrospective analysis of patients with MF who were treated at our center between January 2008 and December 2024with photosensi

Nanoparticles and siRNA: A new era in therapeutics?

Since its discovery in 1998, the use of small interfering RNA (siRNA) has been increasing in biomedical studies because of its ability to very selectively inhibit the expression of any target gene. Thus, siRNAs can be used to generate therapeutic compounds for different diseases, including those that are currently 'undruggable'. This has led siRNA-based therapeutic compounds to break into clinical settings, with them holding the promise to potentially revolutionise therapeutic approaches. To date, the United States Food and Drug Administration (FDA) have approved 5 compounds for treating diffe

Premature Calcification of Costochondral Cartilage: A Scoping Review of the Literature.

Premature costochondral calcification is a rare finding occurring in young patients and is associated with metabolic and endocrinological conditions. Significant heterogeneity exists with regard to its definition and assessment. This scoping review aimed to summarize the prevalence, risk factors, etiology, evaluation, and management of patients with premature costochondral calcification. A scoping review was conducted in PubMed, Embase, CINAHL, CENTRAL, Scopus, and Web of Science, as well as grey literature databases, including OpenGrey and Google Scholar up to 7 December 2024 in accordance wi

Murine models of erythroid 5ALA synthesis disorders and their conditional synthetic lethal dependency on pyridoxine.

X-linked sideroblastic anemia (XLSA) and X-linked protoporphyria (XLPP) are uncommon diseases caused by loss-of-function and gain-of-functio … to pyridoxine supplementation, we also demonstrate the relative insensitivity of the XLPP/EPP protoporphyrias, severe sensitivity of the XLSA models, and an extreme hypersensitivity of the SLC25A38-CSA model to pyridoxine deficiency, a phenotype that is not shared with another mouse hereditary anemia model, Hbbth3/+ β-thalassemia intermedia. Thus, in additi …

Role of Pharmacogenetics in Adverse Drug Reactions: An Update towards Personalized Medicine.

Adverse drug reactions (ADRs) are an important and frequent cause of morbidity and mortality. ADR can be related to a variety of drugs, including anticonvulsants, anaesthetics, antibiotics, antiretroviral, anticancer, and antiarrhythmics, and can involve every organ or apparatus. The causes of ADRs are still poorly understood due to their clinical heterogeneity and complexity. In this scenario, genetic predisposition toward ADRs is an emerging issue, not only in anticancer chemotherapy, but also in many other fields of medicine, including hemolytic anemia due to glucose-6-phosphate dehydrogena

Loss of Wnt Secretion by Macrophages Promotes Hepatobiliary Injury after Administration of 3,5-Diethoxycarbonyl-1, 4-Dihydrocollidine Diet.

Exposure of mice to a diet containing 3,5-diethoxycarbonyl-1, 4-dihydrocollidine (DDC) induces porphyrin accumulation, cholestasis, immune response, and hepatobiliary damage mimicking hepatic porphyria and sclerosing cholangitis. Although β-catenin signaling promotes hepatocyte proliferation, and macrophages are a source of Wnts, the role of macrophage-derived Wnts in modulating hepatobiliary injury/repair remains unresolved. We investigated the effect of macrophage-specific deletion of Wntless, a cargo protein critical for cellular Wnt secretion, by feeding macrophage-Wntless-knockout (Mac-KO

Bullosis Diabeticorum: A Rare Presentation with Immunoglobulin G (IgG) Deposition Related Vasculopathy. Case Report and Focused Review.

BACKGROUND Bullosis diabeticorum (BD) is a condition characterized by recurrent, spontaneous, and non-inflammatory blistering in patients with poorly controlled diabetes mellitus. While etiopathogenesis remains unclear, roles of neuropathy, vasculopathy and UV light are hypothesized. Most literature reports negative direct and indirect immunofluorescence findings in diabetics with bullous eruptions. Porphyria cutanea tarda, bullous pemphigoid, epidermolysis bullosa, and pseudoporphyria are other differential diagnoses of bullous lesions, and they must be excluded. CASE REPORT We present a 42-y

Challenge of liver disease in systemic lupus erythematosus: Clues for diagnosis and hints for pathogenesis.

Systemic lupus erythematosus (SLE) encompass a broad spectrum of liver diseases. We propose here to classify them as follows: (1) immunological comorbilities (overlap syndromes); (2) non-immunological comorbilities associated to SLE; and (3) a putative liver damage induced by SLE itself, referred to as "lupus hepatitis". In the first group, liver injury can be ascribed to overlapping hepatopathies triggered by autoimmune mechanisms other than SLE occurring with higher incidence in the context of lupus (e.g., autoimmune hepatitis, primary biliary cirrhosis). The second group includes non-autoim

Emergency management of acute abdomen in children.

Acute abdomen can be defined as a medical emergency in which there is sudden and severe pain in abdomen with accompanying signs and symptoms that focus on an abdominal involvement. It accounts for about 8 % of all children attending the emergency department. The goal of emergency management is to identify and treat any life-threatening medical or surgical disease condition and relief from pain. In mild cases often the cause is gastritis or gastroenteritis, colic, constipation, pharyngo-tonsilitis, viral syndromes or acute febrile illnesses. The common surgical causes are malrotation and Volvul
