Virtual Reality for Preprocedure Planning of Covered Stent Correction of Superior Sinus Venosus Atrial Septal Defects.

BACKGROUND: Covered stent correction (CSC) of a superior sinus venosus atrial septal defect is an alternative to surgery in selected patients, but anatomic variation means that assessment for CSC requires a 3-dimensional anatomic understanding. Heart VR is a virtual reality (VR) system that rapidly displays and renders multimodality imaging without prior image segmentation. The aim of this study was to evaluate the performance of the Heart VR system to assess patient suitability for CSC. METHODS: In a blinded fashion, 2 interventionalists reviewed preprocedural computed tomography scans using

Stent Graft Sizing for Endovascular Abdominal Aneurysm Repair Using Open Source Image Processing Software.

INTRODUCTION: An important step to reach a favorable outcome of abdominal endovascular aneurysm repair (EVAR) is preoperative sizing of the stent graft using computed tomography angiography (CTA) images of the abdominal aorta. A variety of costly image processing software options is available to obtain the necessary aortic measurements. A package that can be used for EVAR sizing is OsiriX Lite®-an open source, freely downloadable image processing option. This study assesses the concurrent validity of OsiriX Lite® when compared with commercially available 3Mensio Vascular® and Siemens Syngo.via

Dual compression is not an uncommon type of iliac vein compression syndrome.

Typical iliac vein compression syndrome (IVCS) is characterized by compression of left common iliac vein (LCIV) by the overlying right common iliac artery (RCIA). We described an underestimated type of IVCS with dual compression by right and left common iliac arteries (LCIA) simultaneously. Thirty-one patients with IVCS were retrospectively included. All patients received trans-catheter venography and computed tomography (CT) examinations for diagnosing and evaluating IVCS. Late venography and reconstructed CT were used for evaluating the anatomical relationship among LCIV, RCIA and LCIA. Imag

Coronary artery calcium in breast cancer survivors after radiation therapy.

The purpose of the current study is to investigate whether breast cancer survivors after radiation therapy have a higher burden of coronary artery calcium as a potential surrogate of radiation-induced accelerated coronary artery disease. 333 patients were included. 54 patients underwent chest CT ≥ 6 months after the start of radiation therapy (radiation therapy group), while 279 patients had a CT scan either prior to or without undergoing radiation therapy (RT). Coronary artery calcium was quantified from CT by applying a threshold-based automated algorithm. Mean age at diagnosis was similar (

Interaction of biomechanics with extracellular matrix components in abdominal aortic aneurysm wall.

OBJECTIVE: Little is known about the interactions between extracellular matrix (ECM) proteins and locally acting mechanical conditions and material macroscopic properties in abdominal aortic aneurysm (AAA). In this study, ECM components were investigated with correlation to corresponding biomechanical properties and loads in aneurysmal arterial wall tissue. METHODS: Fifty-four tissue samples from 31 AAA patients (30♂; max. diameter Dmax 5.98 ± 1.42 cm) were excised from the aneurysm sac. Samples were divided for corresponding immunohistological and mechanical analysis. Collagen I and III, tot

Practical tips and tricks for assessing prosthetic valves and detecting paravalvular regurgitation using cardiac CT.

Paravalvular leaks are an uncommon but serious complication of prosthetic valves. Transthoracic echocardiography is used in the assessment of prosthetic valves but can be limited by acoustic shadowing from the prosthesis and poor acoustic windowing. Small case series have previously shown cardiac CT to have promising results in detecting paravalvular leaks. We assessed 32 valves in our institution on cardiac CT using echocardiography results as standard and developed methods for improved evaluation of prosthetic valves. These include optimizing prescan drug therapy for heart rate control, opti

Anatomic characteristics of aortic transection: centerline analysis to facilitate graft selection.

BACKGROUND: Traumatic transection of the thoracic aorta is a life-threatening complication that most commonly occurs after high-speed motor vehicle collisions. Although such injuries were previously treated with open surgical reconstruction, they are now more commonly being treated with endovascularly placed stent grafts. Unfortunately, most stent grafts are designed for treating aortic aneurysmal disease instead of traumatic injury. Further refinements in stent graft technology depend on a thorough anatomic understanding of the transection injury process. METHODS: All patients with computed

Virtual fractional flow reserve from coronary angiography: modeling the significance of coronary lesions: results from the VIRTU-1 (VIRTUal Fractional Flow Reserve From Coronary Angiography) study.

OBJECTIVES: The aim of this study was to develop a computer model that accurately predicts myocardial fractional flow reserve (FFR) from angiographic images alone, in patients with coronary artery disease. BACKGROUND: Percutaneous coronary intervention (PCI) guided by FFR is superior to standard assessment alone. FFR-guided PCI results in improved clinical outcomes, a reduction in the number of stents implanted, and reduced cost. Currently FFR is used in few patients. A less invasive FFR would be a valuable tool. METHODS: Nineteen patients with stable coronary artery disease awaiting electiv

New radiographic index for evaluating acetabular version.

BACKGROUND: Several qualitative radiographic signs have been described to assess acetabular retroversion. However, quantitative assessment of acetabular version would be useful for more rigorous research purposes and perhaps to diagnose and treat hip disorders. QUESTIONS/PURPOSES: We developed a new quantitative index for acetabular version (p/a ratio). We determined the average p/a, compared it with previous radiographic signs for acetabular retroversion, and evaluated its relationship with anatomic acetabular version. METHODS: We calculated the p/a ratio by measuring p (distance from aceta

Quantitative MRI measurements of the Achilles tendon in spondyloarthritis using ultrashort echo times.

OBJECTIVES: Tendon involvement is common in spondyloarthritis. The MRI signal from the Achilles tendon has been used to quantify mechanical tendinopathy; however, conventional MRI is limited by the short T(2) of normal tendon. Short and ultrashort echo time (UTE) MRI have the potential to better measure signal intensity reflecting changes in T(2) or gadolinium enhancement. Furthermore, UTE images could be used for normalisation to reduce variability. The aim of this work was to investigate such techniques in patients with spondyloarthritis (SpA). METHODS: The Achilles tendons of 14 healthy vo

Combining Non-Contrast CT Radiomics with MR PREDICTS Improves Prediction of Futile Recanalization after Mechanical Thrombectomy.

PURPOSE: Despite complete recanalization (eTICI 3) following mechanical thrombectomy (MT), many patients with acute ischemic stroke (AIS) due to large vessel occlusion (LVO) experience futile recanalization (FR). We evaluated whether baseline non-contrast CT (NCCT) radiomics, alone or combined with MR PREDICTS variables, improves prediction of post-MT functional and imaging outcome. MATERIALS AND METHODS: We retrospectively analyzed 167 consecutive patients with anterior-circulation AIS-LVO who achieved eTICI 3 reperfusion after MT and underwent baseline NCCT, multiphase CTA and 24-hour NCCT.
